Account versus Physician sm

I think it is more the account itself than the physicians that want the verbatim, Though of course, there are those physicians that actually speak English and want it done thier way, but I think the ones that don't, especially the ESLs, expect us to clean up after them.  Either way, it too, goes against my grain to leave it incorrect and I have a very hard time with it.

I'm getting ready to start applying and these are the companies that I'm looking at.  So give me the low-down.  Good, bad, India?


Here are my criteria:


9 cpl or better


Does NOT send work to India


Good accounts with at least 50/50 of ESL versus American dictators


Consistent work flow - no downtime or time spent waiting for work


Upper management should be professional, positive, and encouraging.


Pay for lines WITH spaces.


Is there such a job out there or am I just dreaming?  If there are other companies other than the ones I listed that would fit the bill, please feel free to let me know. I tested with TransHealth and I felt like I tanked.  It was a hard test and I had to use the mouse to stop and start the dictation as it was online test and you couldn't download the voice files.  So that made it a lot more difficult.  One of the dictators was cutting himself off through the report and I had to leave some blanks.  I'm very frustrated with this whole process of looking and testing.  Are all the tests like that?  Or are there companies that allow you download the test files and transcribe them using your pedal and software?  Ugh!  I am just so frustrated and discouraged.

rocks and, unfortunately, is probably a minority in a huge amount of immigrants who come to the US anymore and do not truly become Americans. It was always with immigrants before, like my great grandpa who came from Germany at 35 and recently passed, that they wanted to be Americans and to assimilate into American culture. There is an interesting article this morning in the LA Times that Nashville has a huge immigrant population and that it is somewhat of a problem because people are not assimilating and are not learning English. They are working on passing a law declaring English the official language. I know where I live in Northern CA that I have seen this happen myself. I was at McDonalds one day waiting in line to order and a customer went up to the counter and started ordering in Spanish. The poor girl behind the counter was Indian and told him in English she does not speak Spanish. Where I live, we have a huge population inclusive of Asian, Indian, and Middle Eastern people along with Hispanics. It is ridiculous and unnecessary to cater to 1 segment of the population. America's language has always been English and no one ever seemed to have a problem with that before.
Years ago I had this one physician who
in a rage, demanding that we type things exactly like he said it. Ok, Dr. Do What I Say---- he would mess up on his 1, 2, 3 and 4 and so like he said, when he said 1, 4, 3, 2, 9 and so on, that is EXACTLY what I gave him.
Story for you, went to the physician
a a workin on a Saturday, never do that, waited and when he asked why I was there, said well I have a couple of issues and he told me to tell him which more important, he did not have time for the 2. I chose the ribs which were really hurting (costochondritis) and what I did not get to talk with him about was had a breaking out of shingles. Anyone knows that can be really bad if it goes to nerve endings, horrific pain. I no longer see him by my choice.

Can anyone tell me how to change the dictating physician SM

in the gray field on the Spheris TWS screen. For example, if I need to change a dictating physician from a PA to the attending, the field is gray and I can't change those fields.

and rearing my children.  I had a phone interview with a company that I found here on stars and when I couldn't make the first attempt due to a problem with the kids, she clearly understood and yes, I did get hired. 


Not all companies and physicians are children haters like you.  Sorry to bust your bubble. 


Not all MTs think their you know what doesn't stink and are very down to earth as well as the people who hire them.   


P/S:  This profession is not looked upon as such a bad thing ya' know.  Who told you that?  Where did you read that?  Furthermore, if this was the case, I don't think there would so many MTSOs out here working from their home offices.  And believe me these MTSOs are a dime a dozen.  Most of them do have children and work from home.  It is not uncommon and they are very professional.


Agree that E&O rests on the physician -
not the transcriptionist. Looked into this many, many years ago, as I thought it was something that I needed. Found out that the liability indeed rests on the shoulders of the physician - who proofs and approves his reports before signing them. Although I feel responsible for the accuracy of every report I transcribe,the bottom line is that it is the physicians responsibility - legally - to ensure that it is !!

his letters, I was required to same them for 6 months. Would do me and him no good if I deleted the identifying info.

We are transcriptionists, not physicians.  We don't make any calls.  We transcribe what we hear, and they read the report and sign off.  It's that simple.  It always has been that simple.


No one here deserves the salary of anything more than what they get or make production wise.


Do you really think these MTSOs are charging a million dollars or what?  They make money by bidding accounts and then sub the work out whether that be by hiring employees or by contracting with an IC. 


I don't know where people come up with this idea that we are the bread and butter of these companies.  We are the worker bees.  Do a good job and you'll make some money.  I make twice as much now working from home than I did hourly with benefits and the benefits were crap.


I can set my own tone.  I transcribe to the best of my ability but by darn it if I can't understand what the dictator says, I leave a blank, and they fill it in.  That's what transcription is about. 


If your physician jumped off a bridge...
would you jump right behind him? The bottom line is that the only person you can control is yourself. If a physician is not performing his job correctly, guess what? He will eventually lose his license. Why do you continue to try and justify punishing patients simply because people do not want to do their jobs correctly? However you spin it, it's still wrong.

I think your physician gave you good advice

These MT schools are still cranking out the MTs as fast as they can (and taking their money).  I am sure they know that there already isn't enough work to go around and no work = no money.  Plus the fact that the MT trainee who could ever hit the ground running and make money right out of the gate is far and few between.


As for the coding, it is pretty obvious that is a huge candidate to be done by EMR and I'm sure it will happen in the not so distant future.  There will probably be someone an R.N.? LPN? EMT? who will do chart reviews to ensure that every possible diagnosis that could get another dime in reimbursement is put in the record.  That would be a few jobs depending on the size of the hospital.  I seriously doubt that these jobs will be offered to former MTs.


Another job opportunity might be that of what used to be called ART (acredited record technician), those people might be allowed to do chart reviews as they have always done it anyway, I even did it myself way back there.  The JCAH (forget what they're called  now) used to offer a study at home course but you had to be affiliated with or work for a hospital in order to have access to the records that were necessary to complete the course.  In some smaller hospitals ARTs (or whatever they are called now) were even heads of the medical record department.  I believe it is also offered as a 2 year course at some community colleges.


Sadly I think that those who have put themselves out for the expense and bother of getting and maintaining CMT credentials are just going to be SOL because I think those credentials and a buck might buy them a cup of coffee.


This is the way I feel and I could be wrong but I don't think so.

I had a horrible experience with this company.  I applied in for part time IC work in February but it was not until April that I rec'd my first very unprofessional email, but I had read nothing bad about the Co, so I came on board.  The first weekend I typed non stop too much work not enough typists.  She was so happy with me that she moved me from .07 cpl to .08 cpl, but nonetheless, the account ended up dropping us for the most part, meaning there was no work.  For nearly 2 weeks there was little or no work.  I had to take another company.  All of the sudden she had tons of work and demanded I do full time.  I responded I could not as I had to take another company, she told me I had to commit to part time hours for her, which I did, but there was no work during this time.  In the middle of all this was my 1 paycheck per month which is supposed to be mailed on the 10th from TX, I did not receive it until the 19th with a postmark of may 13th!  I rec’d countless phone calls and emails most of the email were junk emails and not concerning work.  In June she said that the 20 hours I worked for her that had no work was not enough I had to type 5000 lines a month, shouldn’t be a problem, but there was not work when I could type.  As I do have another account.  In June she sent out an email stating that she was changing pay from .08cpl to a sliding scale, the scale starting at 3000 lines.  I emailed her stated that I did not agree with it, but she was the boss :’) and I would make it work.  I asked her what if I type below 3000 lines, as when I am scheduled to type for her there is NO WORK.  She sent me a nasty email about her being the boss and making the decisions and it didn’t matter if I agreed… anyone typing under 3000 lines in a month was not worth her time.  I emailed her again, now I was upset stating I was joking, hence the :’) and that she still did not answer my question about typing under 3000 lines.  She sent me an email stating that she has tried to help me answer the question and she is done talking about this.  I did not respond.  The next day on my lunch break from my other account I logged on to Beth’s knowing I had to work extra hours to try to get my 3000 lines, when I logged off of her to go back to my other job she sent me yet another nasty email saying:  There is work, example you just logged off there is work.  I responded Example?  I am on my lunch from my other job and typing for you to try to make my 3000 lines.  She responded and said she has had it with my attitude, as of July 1st you will be no longer needed.  I responded are you kidding me you are firing me because I was typing for you on my break?


On July 5th I emailed her and said I will uninstall the program and what else is needed.  She asked why?  I said you fired me.  She asked for the email firing me, I sent it to her.  She responded back to me that she wanted me to continue working.  Against, my better judgment, I continued with her.  July 14th I rec’d my check, it was 27 +/- off.  I sent an email stating My check today was short 27 dollars, please remit payment immediately.  She sent me an email stating that I am not to demand money from her that is not how it works and I need some employee/employer skills.  I responded that I was not her employee, I run a business as an IC and that the statement “please remit payment immediately” is per NYS statute, payment has to be requested immediately and that I meant no disrespect by it; however, I did not appreciate her telling me my attitude would determine if I was paid or not.  She responded back that again I was fired.  Only this time she said that TX law says she can fire an employee after 3 counsels of work or attitude and she felt this was my 3rd.  She went on again to say how my QA was awful for the headings and dates and she was continually it.  I requested she be more specific and asked why didn’t she send me QA reports so I could correct it, she said that she “didn’t have the time”  I responded again she was confused as I was NOT her employee, she did not need a reason to fire me.


All in all even if we consider Beth as being right that my “heading skills and dates” are awful, what kind of a company keeps a typist working for them if the typist is horrible.  What does that say about the company AND what kind of a company doesn’t QA their employees or ICs and let them know in order to make them better MTs.  This company is very disorganized extremely disrespectful of its employees, IC.  I have emailed others who have had terrible experiences as well.


 


I would suggest run the other way, otherwise DO NOT have any personal emails with her in asking questions, be prepared for many phone calls, tons of junk mail and no feedback.

That account in Texas was not a MQ account. They did not use DQS on that account until KS started.
They are moving 3 other accounts to DQS between now and January 1st. I asked. The MQ accounts they have gotten in the past did not go to DQS. I followed my old account, which is how I know, and it was Meditech. The account is still run by KS and is a big account too.
Repeatedly told another new account would come soon, but it never did. In fact, our slow account is.
everyone else's backup account. What a disorganized mess.
Then when we MT's are **thrown on** an account because of lack of work on our account

What is so hard to understand about that? 


Then we have that account info filed away in a folder in our Outlook Express by name of account and can look at past emails sent out regarding that account.  Not fun to be tossed on an account that you have never seen NOR ever received an email on.  Sounds like business to me.

It happened quickly and was unexpected but it was really for the best. Keystrokes does not string people along or lie to you. After being with MQ for too long, I am happy there, although it is not for everyone.

I really would give them a call as the other poster suggested. That whole situation was a nightmare for everyone including management but the account pulled a fast one on them.

I have the facts on this because I have a good friend at the hospital and what she told me matched what my manager did.

I know that they are trying to give more when so many companies are giving less and taking away more.

I know the SS differences between the 2 but can anyone tell me is there any real advantage of being an SE versus an IC?  Why did MQ do away with SE?  Do you think this will be growing trend with companies?

Take the R.N. course. I wish I had taken it at an earlier age. You will have a job that you can depend on with good salary. Don't make the same mistake I made.

If you got 8 cents as an SE, what do you think this would calculate to in line rate as an IC - 9 cents?  By this, I mean SE helps with your taxes but by how much would you estimate?  Anyone know? 
QA versus MT?

I was just offered a QA position.  If I accept the position, it will be my first QA job.  According to my calculations, I would only make about a dollar more an hour on average by staying a production MT than I would as QA with this particular company, who pays their QA staff by the hour.  I am wondering, is being offered QA anything like a promotion in the industry, or is it more of a lateral move/matter of preference?  Would love to hear opinions on pros and cons of QA  Thanks!

Been and done MQ...now at Webmedx and enjoying my job again. Nice to feel valued.
SE versus IC. I know there is s simple

but I'm a math retard.  If an SE made 10 cpl and an IC also made 10 cpl, and everything else were equal, then the SE is actually making more because half of the 15% social security tax is paid by the employer.  So the SE makes 7.5% more than the IC, correct?


Now here's where I'm a math retard.  If you take that 7.5% into account, what is the SE's *real* line rate?  Sure, it's still cpl but when you figure the employer is paying 7.5% of the taxes then does that make the line rate actually more like 10.75 cpl?  Or am I off on my math?


MT VERSUS MTSO
I have to agree with you that job hopping can look unprofessional. I have a bit of dilemma though: I am on my second job and either I stay and continue to be unhappy or risk a third try that may put me in a similar/worse situation. Here's the kicker though - My problem is not the company...it is my coworkers. I am discouraged by a lot of my fellow MTs' work ethic and the every-man-for-himself attitude. I don't mind sharing my normals with everyone. If it means they are more productive, helps them get more lines in and our account is in turn, why wouldn't I want to share? We have all been new to an account...why not be helpful and supportive? Here is the resume I would love to send out:

MT with 3 years experience in acute care; ESLs not a problem; reliable, will work the schedule YOU need, will be on time and stay for entire shift(imagine that); no children to bring to doctor/football practice/exhusbands house/school play; actually does research, has an extensive library of research books, and does not just toss a job to Quality dept for them to finish; NEVER cherrypicks or skips jobs; will not poach, working outside my schedule trying to get extra lines to pay for Xmas/kids braces/second mortgage, taking work from others; does not mind working over/extra when needed; does not have a endless stream of mysterious health problems - in fact, has only called in sick once in 3 years; recognizes the Quality department's value and that they are an essential part of the job I do; does not whine about hard dictators/OPS/ESLs/short reports/long reports; have taken Anatomy and Physiology courses in addition to initial training and still taking continuing education courses; sitting for CMT next month; helpful and supportive of my TEAM (for those of you who do not know, that is my coworkers and my employer); does not have constant, vague-sounding internet/phone/electricity problems that prevent me from working.

Here's a thought (not for the original poster, but for those the shoe fits) - how about thinking about what you have to offer a company instead of what you can get out of them? What do you bring to the table? Selfishness? Laziness? A baby hanging off of your hip, a slew of pathetic excuses, and a victim's attitude? No wonder you are not happy anywhere...you get what you give.

I will not send out that resume, obviously. Why? Because those types of MTs are everywhere. I cannot hope to find a place where none of that exists, not with the pool MTSOs have to pull from. Plus, it is not professional. The reason I did it here was to illustrate a point. People do not always job hop with the hope of improving their situation, many do it for bonuses or to find a place where they get the most and can do the least.

Want change? The responsibility for change and growth within this industry lies with the individual MTs.
IC versus Employee
As an IC, you can deduct at 100% all of your expenses.  An an employee, you can only deduct the amount that exceeds 2% of your gross income (if you are married and filing jointly, it is yours and your sponse's combined gross income).  In other words, if your joint combined gross income is $100,000, you can only deduct your expensed over $2000.  That is a huge amount to have to not include.  Check with any tax accountant or IRS regarding this.
Holidays versus PTO

I think it all depends on the amount of PTO that you accrue per pay period.  Some companies allow you to accrue more, but then you are expected to use your PTO for holidays.  Some may pay for holidays separately, but then you don't accrue as much PTO; it basically all balances out in the end.


IC versus employee

This is a muliple-part question--hoping someone here can help with some or all of them! 


1.  Does anyone have experience working for Shriners' Children's Hospital inhouse in Norcross, GA?  Many ESLs, how are working conditions, etc?


2.  I've been recently sent home from my small office practice (didn't volunteer!) and changed from transcription/med records clerk employee to IC; lost benefits. I think a little further on I'll have to get another inhouse job--remains to be seen if I can afford to live on this reduced income.  Does anyone have solutions regarding health insurance for self-employed, single people?  In my state an individual BCBS premium for someone my age is $300.00/month w/$250.00 deductible. (Yikes!)


3.  How to tweak my (hopefully) next interview to reflect that in my last job I was told basically I wasn't a good "fit" for the office, even though they were happy with my work quality and quantity?  In their opinion I didn't pitch in and answer phones quickly enough, schedule patients and take on copying duties cheerfully enough--in short, lacked a positive attitude, even though I DID do these extra duties politely. I'm nearly 60 years old--and don't do perky very well.  Should I just explain this last job didn't suit my skills and personality?

VR versus straight

VR versus typing
Do you make more money with straight typing or VR?  Just a poll.  I do both and seem to make more straight typing.  You would think VR would be quicker.
VR versus typing
I make WAYYYYYYYY more money typing that VR. There is no question about that at all. On one particular account that I do I can type almost as fast as he speaks so that is a great moneymaker, but even on my fast talking doctors there is no comparison to doing them in VR.


SPI versus MedQuist
I tested with SPI yesterday -- no word yet on the results, but keeping fingers crossed very tightly!  Anyway, got an E-mail from Medquist today but I see I lot of MTs complaining about running out of work and a high ESL range of doctors.  Is MedQuist a place I should avoid?  I haven't tested or applied yet -- just looking to get out of where I am working now.  Not enough lines and just generally unhappy there. Thanks for all of your input!  Was offered a job with DocuMed -- called to accept and was asked to type a few more files only to never hear from that company again.  I guess I did some free work as I heard that I was not alone.
MQ versus Webmedx
Can anyone compare the hourly line rates between MQ and Webmedx for me?  I'm usually around 250 and 300 per hour at MQ but want to find a company with a better reputation.  Just want to know an average of what anyone can type at Wedmedx per hour.
OSi versus DeVenture

Hi.  There used to be a lot of negative posts on here about OSi.  What are you guys' current opinion on it?


Here's what I'm really contemplating:  contacting DeVenture, who had offered me a job at around the same time that OSi did.  Here's why:  I got put in a program that would advance me to acute care kind of transcription.  It did, but now I have to type for about 10 hospitals, thus I never can get good--or a good line count--at any ONE hospital's transcription.  So--my cents-per-line rate in theory has gone up but there's no way I can make the kind of line counts I made when I was just doing clinic transcription.


Theoretically, I have a primary account, but there is never enough work on it.  That's why I get work from 9 other difficult accounts instead.


Anyone else having this type experience?

Anybody left MQ for Webmedx?  Just curious about how they treat MTs as compared to the Q.  I was also told they do not offshore - can anybody clarify?

Has anybody got any feedback on MD-IT.  I am considering an offer from them and also from Amphion.  I've heard some not so positive things about Amphion and was wondering if anybody knew anything about MD-IT....

Made the change and can't believe the difference.  I knew there had to be a better company, couldn't have been worse, and boy am I glad I made this change!
hospital versus IC
If you are happy being an IC, stay there. I went from there to MQ and was happy for several years, but just quit MQ because of all the changes. If you stay your own boss, you obviously have much more control.
MQ versus Webmedx
I am wondering if anyone has left MQ for webmedx.  Is it any better there.  Another question, webmedx does not supply an Expander with their program, or am I confused.  Just looking for anyone that might have switched and how they are doing with webmedx.  Thanks!
US versus India
I have to agree, the qualityof the India MTs has been superior in many instances and some of the people who claim hey have 10-20 years experience must be flat out lying.  I have seen lap coli in reports..in a urology report the patient is impudent, it is embarrassing to say the least.
