It is a little scary putting something that far down your ear canal.  However, it's no different than some types of ear protection, and the hardest part was getting used to the stone-cold silence when they do get put in there correctly.  The little silicone tips are surprisingly comfortable and they include a variety of sizes so you can choose which ones fit best.  I do find though that if I don't thoroughly dry the inside of my ears after a shower and put the ear plugs in, they will itch.  Nothing a kleenex or alcohol prep pad won't cure pretty quickly. 


I've had my pair for 2 years now and I use them all day for transcription and then plug them into my little MP3 player for tunes while walkin'.  They really kickass for music quality too! 


My only complaint is the teeny filters they have that go in them.  They give you a couple pair but it seems that even microscopic amounts of wax, I mean amounts so small you cannot see them with your naked eye, will occlude them and the sound will just stop.  I thought they were broken until I contacted Etymotic and they told me I had to replace the filters...more often too since I wear them all day.  I'd say I go through a pair of filters about every 2 months or so, but for those accounts where the dictator whispers, or it's hard to hear ESL, or anything where you wish you could fine tune the sound, these bad boys are just the ticket.


I wish I could let you try mine first.  You would be instantly sold.  I only got to try them beforehand because my husband had a pair for his MP3 player and he wanted me to try them after I wound up crying one day because I couldn't hear a bunch of reports due to the general crappiness of both the dictator and the recording quality.   


After I initially contacted them, I told the company what I was using them for and they had no idea that they would be such a benefit to a medical transcriptionist...they were intended for music listening only, so that was kinda cool.
