Qwest "unlimited" long distance can start monitoring you after 5000 minutes a month on residential, 3000 minutes a month on small business and then can ask you to prove that you are making regular calls, i.e., person to person, not to a computer, etc. This doesn't happen to everyone, but it happened to me. MCI, Verizon, SBC... they all do the same thing. The only thing I have seen that doesn't specifically limit minutes is VoIP such as Vonage, but every plan has terms and conditions on what you can use it for and what you can't.
Maybe your company has overcome VR limitations
My own experience has been that the good, easy to transcribe, and consistent doctors end up on VR, be their name Smith, Smithnova, Smithahaji, Smithnguyen, or whatever. However, the limitation most companies' VR has is with the problem dictators and the rare occasion when the good dictators have a hiccup during their dictation (be that a proper noun, a choking spell, cell phone going off, whatever). Maybe your company has decided to just take everything and toss it into VR and see what comes out. Am I to assume then that your company has 100% VR? If not, there is some determination made as to who gets done VR, and believe me, it ain't Dr. Gadfiupiouruyrqeasdfhyrqqueasdjy, III.
