How odd, I wonder why?

How very odd!  It seems to me that Instant Text is just a tool to help us get the type onto the screen/page.  And if Instant Text is compatible with their system so that one could technically use it for your abbreviation program, I don't see how that would affect their upload to their clients.  They're uploading the finished product, not the abbreviations that we use to get there.

It sounds like it's more likely to be a problem with their platform's line counting.  Since you're using their computer, I would bet that their program is probably logging your keystrokes, i.e. counting your Keystrokes where each keystroke equals a character, instead of counting the actual characters on the page of the finished product.  So if you type tp and it expands to the patient, you'll only be getting two characters based on your keystrokes, whereas in actuality it is 11 characters.

Oh well, whatever the reason--thanks for letting me know!
