Do you transcribe for a hospital or clinics?  The job I am currently in I do about six hospitals, I have one primary hospital, but they always get caught up and I get put on my several back-up hospitals.  This makes it very difficult to get my line count up as I do such a variety of doctors and have new ones almost every day that I have never done before.  I am used to making 2000 lines a day and on this job am only making 1200 at the most.  I am seriously considering a change.
