that this is a board set up FOR medical transcriptionists to vent, to socialize, and to network.  I'm sure that as management you have your own means of networking.  If you are visiting please know you are welcomed, but please do realize that the comments made here, while some may very well be 'Jerry Springer' like, are made out of frustration for our field in general.  With the majority being remote transcriptionists, we do not have the normal venue for getting our things off our collective chest at the water cooler or in the break room with our colleagues.  Most of the time we don't even know the names or emails of our coworkers in our own companies.  So we come here and sometimes the venting gets rough.


Please, do not take it personally, as some feel you may, but also please keep in mind the context of the posts and the fact that you will have Jerry Springer-esque behavior anywhere and everywhere.  It is not limited to the MTs, the residents, the PA's, this board, etc., and it is probably less than 5% of the total number of posters.  I'm also hoping, as a former manager myself, you're not thinking the same term I am about those who feel the need to run up and apologize where an apology isn't necessary.  After all, we are professionals and we do know how to take things in context.

The opinions expressed on this board represent only a fraction of the MTs that may be servicing your accounts.  As an MT who is proud of the MTSO for which she works, I apologize for the appalling Jerry Springer-like antics of this chat board and wish to assure that most MTs that I work with are thoughtful, hard-working professionals who do know the difference between venting a frustration and turning into an ugly mob of pitchfork-carrying villagers. Though I defend their right to an opinion no matter how churlish and embarrassing it is, I also assure you that there are still MTs working on your dictation who realize that there are live patients who depend upon us.  I just thought you needed to hear this.

you are seeing come from years of being treated badly by the companies you sign transcription contracts with.


We are treated like indentured servants.  Every year, our benefits disappear or cost much more.  Every year, we are not only not given any kind of a raise in pay, but we are forced to accept pay CUTS with little to no notice given by our employers.


Part of the responsibility for this mess is in your lap also - for demanding that services provided to you come cheaply.  Would you demand that your own employees suddenly be paid 1/3 less for a portion of their jobs?  What would happen if you did?


If you're feeling really bold, why don't you experiment with that concept?  Ask your nurses to be paid 1/3 less for the portion of their job that requires paperwork, for example. They aren't providing direct patient care, so that portion of the job should cost you less, right? 


Tell all of your employees that you will no longer offer direct deposit for their paychecks.  Then tell them that they have to pay $25 to have their checks FedExed to them or depend on snail mail for free.  How well do you think that would go over?


Just a slice of what we MTs have to accept every day.

I am not paranoid because I already do VR at another place. I thought the reason for the question was trying to figure out by having so many or so little as to how VR would work. Well, gotta go as time for me to start work.

Things get blown way up. It was just a question and only a couple just answered the question without judging me and getting into things they have no knowledge of. WOW I think you folks really need to get out more. !!!!!!

Don't know about turnover but new clients coming on board - GREAT company

What happens, is MTSOs coddle the clients and let the clients call all of the shots.

Then the MTs get stuck with account specs that are contradictory and confusing. 


It should be simple.  When an MTSO is wooing a client, the MTSO should present with a copy of the style guidelines they intend to follow or if it is the BOS, let the client know that.  It should be made clear that with the exception of report format, the guidelines are not flexible.  Either we use apostrophes or we don't.  Don't do it one way for one client and the exact opposite for the other client and then hold the MT responsible when she gets confused as to what account she is typing that day!

Maybe the reason you have caught a lot of grief on this board is because you apparently know how to dish it out but you can't take it. What goes around, comes around. I hate to tell you this but you come off sounding rather bitter yourself.

I'm just wondering why you people from Keystrokes feel the need to defend yourselves so much. There must be some truth to what is posted here about you. They can't all be transcriptionists who can't cut it or don't fit. Maybe the bad fit lies within your organization. How did all those bitter people get hired in the first place?

There is an awful lot of pettiness and ugliness in those who post from your company, dear. Those who live in glass houses shouldn't cast the first stone.

I do wish you the best and I hope those growing pains that you seem to have had for so long feel better soon.

Usually on visiting doc's office, they ask if I am in the medical field because
I can speak right along with them on most they say, don’t have to have it put in laymen's terms. I do not say I am but am asked if not in the medical field, countless times.
in addition, while the office staff are partying, visiting with each, breaking whenever they want to
with pay, we are struggling to pound those cold keys with our cold fingers with our break drinks, meals at our desk . . . no time to take off due to no pay for any time taking off for a break or lunch.   We are grabbing all the work we can get now before another MT gets it, as our paychecks continue to dwindle due to VR/ASR and leads and others taking the easier dictators from us (those who have ever transcribed a word in their life) and passing all the other down to the piecemeal worker, the MT.

I think you all need to realize
That MDI would not have never been created if not for Liz and Sue McGrogan....remember this is a company that they built from scratch.   So without Sue's input MDI would not be what it is today.  Kudos for her and the $$$ her business has made over the years.  I don't care where you work you are not going to be able to please EVERYONE all of the time. It just doesn't work that way...Oh and by the way...I am not management and I don't even work for MDI...I work for hospital in Florida.

most likely had to sell out because of their poor management abilities! Management is where all the troubles lie!

...that much of what I said was tongue in cheek, as there really AREN'T questions that will identify someone who has a personality problem. Unless, or course, the person is severely troubled and is speaking tangentially, or is using word salad, or has flight of ideation. I've come to realize that grandiosity is an uncommon, but workable trait in some MTs. Lol....

I appreciate your thoughts and wisdom on the topic. Hopefully others will begin to recognize that, short of tests that are designed to help identify problems, there isn't anything a recruiter can do to weed out the problem folks. To infer that there is someone bigger and better than a recruiter ("the MTSO")to accomplish this isn't going to be easy, or cost-effective.

I don't think most realize that you don't have
to be consistent. I stick with it consistently.

It's not that hard to make this money. You're right - don't work for peanuts. Find the line rate you need.

It can still pay well if you're good at this work. Some people just aren't as good as others.

There were companies I worked for that didn't require me to send invoices because I know that if the IRS were to question them, all they have to do is show them the contract that I signed as an IC and it saves their butts. I wasn't disputing.

but the 7th was a Sunday. Of course you were paid on the 8th.

I don't work for Cardioscribes, so it's really none of my business, but you're making yourself look pretty bad here. Banks aren't open on Sunday for DD, and mail doesn't run on Sunday...so you do the math there.
You might not appreciate it, but do you realize that
every day there are people wondering how they are going to feed their kids, that people are losing their houses, and $100 would be a major blessing to them.

Maybe not your clients, but many are.
Your clients may be behind the times, but many are not. VR has gotten much, much better, and I've spoken with many, many MTSOs who are moving their accounts over.

The average seems to be about 60% to 80% VR and 20% to 40% transcription. There will always be those you cannot put on VR, but it is growing rapidly.
Not all clients DO know--that's just it!
Yes, some clients DO know, and the almighty dollar is the only thing they think of. But some clients DO care, and they don't know their records are going overseas. They would be apalled if they knew. Come to think of it, so would the patients.

Do not get me wrong. I do not care for it either, but somewhere down the line, the companies will have to suck it up and so will we. Otherwise, we move to a different field. Unfortunately, that is just the way it is.
Becoming an IC with own clients
How does a person become an IC with their own clients, not working for an MTSO? From what I have read on this board, it seems that is the way to make the most money per line/word/page, etc. so I am thinking about doing this after being an employee for 15 years (with things going downhill now). I would appreciate knowing several things:
1. How do you get the clients.
2. Are there certain types of docs that tend to prefer this situation rather than going to an MTSO?
3. How do you determine what to charge?
4. How can someone get the dictation, type it and return to the doctor without having to pick it up and drop it off?
5. Without QA other than yourself, how does an IC handle blanks? Do doctor's offices tend to get upset with blanks?
6. Is there a particular software system that is better to use when you have your own IC service and want to get work over the Internet and return it over the Internet?

This and any other advice greatly appreciated!

hide it from the MTSO, just like the MTSOs hide the fact that their clients' work is being generated by VR.

being sold it at every AHIMA conference they go to? Being cold-called by companies doing VR already? That doc don't get info about it at their little retreats and conferences?

It's better to be proactive and have VR sold as part of an MTSO's offering (with MTs doing the editing) than being undercut by other VR-only companies.
I think once you get into it you'll realize just

how little med-speak you know.   I have always soaked up anything medical, starting in junior high and terminology came very easy to me, but I have been in this business 20 years and I still learn new info all the time.   I may learn a new piece of equipment or new drug, or have a doctor from a different part of the country and he may use different terminology, not necessarily new, but new to me.  I may have a day go by where I don't have to look up anything, but I rarely have 2 days that I don't.  


The speed will come with practice.  I barely passed typing in junior high and never really typed aftewards until I started in this profession.  If I had to take a typing test where I had to type from a book I'd fail, but I can fly when typing dictation.  


I didn't realize there was such a
company where everyone is happy and no negative comments.  Point me in the right direction - I certainly would appreciate it.
You need to realize that about 80 pc of the docs sm

practicing acute care in this country are ESL.  It is what it is. 


I like them a lot better than the native English speakers, who slur words, like to play with the language.  ESLs learn one way to say it, and they say it that way every time.  You just have to get used to it.


You have to realize that there are some people that you cannot sm
make happy no matter what you do and that those same people take their negative selves to the boards and attack their latest company. It IS two-sided. MTSOs have as many problems with MTs as MTs do with MTSOs, but we are supposed to just sit back and be attacked without voice.

There are some people who have miserable lives and like to spread their poison everywhere they go. The problem is that they seem nice and normal when being hired and then their true self shines through within days or weeks.

what I am talking about.  I worked on it for 2 weeks.  I gave it my best shot.  My Expander was loaded.  The account was super-easy, but Meditech ran so slow I was typing faster than it could run and I was typing a word, pausing, typing a word, pausing.  I have worked on more than 50 accounts in my career and 10 or so different software systems.  I can say, without exaggeration, I would not work on that particular account if they offered me $25/hr salary.  I was so stiff with tension at the end of the day I walked like Frankenstein.

Someone on here did that to me too, was back and forth, back and forth, had a confusing pay scale, just had another ad on the boards this past week.  Don't want to say too much here because I KNOW they come here although it is not a well known place. 


If it is the same place, I'd say run because I think the problem is just total chaos and disorganization - nothing to be a part of.


While I realize we all need a Plan B, I mean...

I realize...I just want a job that has a ton of work that
I can do as I please. There is one company I've been hearing about that has a ton of work and will let me go part-time, which helps (since I have a current job as well). I have little kids, it's just so hard...not what it used to be.

IC - FINDING CLIENTS
Do you have to pick up tapes and deliver the hard copies with your accounts? I used to have an account where they would dictate into my Sony system (a special system that allowed docs to dictate from their phones, pause, rewind, make changes, etc), and then I would just e-mail the completed work back at the end of the day for them to print out in the office.
I wonder if the clients are leaving
because they are being sold and that is why I have no work? I can't wait until they make the announcement - then I will know if I need to pack up and leave.

Are their clients aware?
If I were a client of theirs, I would look for someone else to do my transcription. If I were a patient of this client and if I found out that the client knowingly supports this sort of crap, I'd look for healthcare elsewhere. Heck, docs come a dime a dozen!!!

It all comes down to the customers--that's US!!! The American citizens who trust that our medical records are (1) kept confidential and (2) are done correctly IN THE USA!!!

There should be a way to inform the customers of HIS's clients. First and foremost, we, as customers, as Americans, need to boycott the physicians first--the ones who are sending American money overseas!!! There are PLENTY of other docs out there who support the USA and want to keep the work at home. They'll gladly take my money!

Persnonally, I don't want my medical record entrusted to overseas folks who, for the most part, hate us to begin with. They barely speak a word of English and they certainly do a pathetic job at trascribing. I've seen the damage these folks can do to medical records.

I think that we should boycott MDs who are paying HIS to transcribe for them.

If we don't stand up for ourselves now, what's next????
