Wow, you have been through a lot and I admire your ability to bounce back. I just know whatever company you choose to work with you will do well, and good luck on your move! Thanks too for the feedback on all the companies. I thought that E-Trans was very nice too, but decided in my heart I did not want shift work, and since it did not work out the first time (the ExText account), I would just pass. I am going to keep Encompass in mind for the future because they seem great like you said, and I have the quality but have been in this for so many years, don't want to be stressed out. Like you, I get insecure, and that is when I do worst. LOL. I wish you all the best. For me, I am now going with MDI-MD and know that if I maintain my quality and my flexibility I can stay away from shift work. I think we all need to put priorities and what we need to do in life and then hang onto our decision. Rough seas, but if you hang on to the life boat the storm always passes and we always seem to make it. Thanks for writing and all the best to you! :)

What is the % of ESL dictation you do on weekly basis? Why do people hate ESL dictation so much? I learned a long time ago thatit is better toput the time into learning each ESL dictator, then when they come around it isn't as painful and some of them are actually pretty good and give good line counts. It seems, no matter where I have worked, that when reports fell way out of TAT it was because there are a lot of ESL dictations that people didn't want to do so they just sit there.

Followup: I did test with them. There was a written test with 33 questions and then typing test with 5 reports. Sound quality was good. There was one foreign accent on a heart cath. The thing is that I know the recruiter from meeting her when she worked with SPi as theier recruiter and they were the company that my hospital outsourced too. Well the recruiter left SPi. I had to send two Emails and yesterday she said she would call me yesterday. Yesterday has come and gone and still no phone call. She said she had to hire 40 people. I just wanted to know if I passed or failed. I had to go to work for SPi but they do not pay for spaces. DSG test can be done with open book and was not bad. Good Luck if you go for it.

Can anyone give me the good/bad/ugly on OSI? Have offer, but contract is extremely odd, over the edge, another company pays (only guaranteeing minimum wage of OSi don't fork out!!). They are looking for VR. I've been told they outsource, but not sure if true.

I quit working a shift that what would happen was I would get all 1, 2 minute dictations but if I worked the next shift or after midnight all kinds of good dictation came my way, go figure. I hate being cynical but if you ask about it you are told, oh no, no one, not even the leads can do that. Common sense tells me if they can assign jobs to you, they can assign or take jobs for themselves. Doesn't take a brain surgery to figure that out.

The good: Rarely run out of work. Pay is always on time. The MT account managers are wonderful. Great platform. Generous PTO. The bad: Their pay is low. No extra pay for overtime or holidays. Production bonus that is set ridiculously high (5000 LPH for editing I believe).

Some information on TTS [2008-11-02]
They were very nice when I spoke with them. They let you set your own hours, so a split shift is okay. But, they do expect you to work the exact hours that you set. They are very upfront about the fact you have set hours. If you are looking for a flexible position without set hours, this may not be the company for you.

Dictation Services Group pays for spaces, but are wanting MTs mostly for second shift. I work a split shift.

If the dictators are easy to understand, then it may go quick but if they aren't or they back up a lot and correct themselves, the software isn't smart enough to pick that up so you must spend a lot of time editing. Editing is much more difficult than transcribing IMO and it doesn't go that fast. So they need to pay more for editing. Anyway, there are still some big facilities out there who won't be going to VR or VSR or ASR or whatever you want to call it because of the security issues. I also think that once someone hacks into the patient records that now reside on the internet, that will be the end of web based record keeping and the end of Escription such as it is now.

Keystrokes is hiring and has many openings. The ER account I am on needs 2 or 3 more people, the B4 account my SIL is on is short-handed and they are hiring like crazy because of new accounts. If you do not have enough work, contact your manager or go above her/his head. There is plenty of work there. I also know for a fact that they are hiring for orthopedics due to a large new account because they sent an email asking if anyone knew anyone who was looking for full-time ortho work. They do not overhire. Quite the opposite, actually. We are always asked to work extra. I know they are looking to balance this out but will not hurt current employees by overhiring.

The best way to get your own accounts [2008-11-20]
Come up with a flyer that states what you do (transcription), what your costs are (and don't sell yourself cheap - you may need to get new equipment or with working locally, it may involve going to pick up microcassettes and then taking the transcription back to the office), and what kind of Turn Around Time you can provide. Really sell your experience and the quality of the work you can do. Then, start local. Go to your own doctor, OBGYN, etc and see if they need any help with transcription. If they don't, see if they can recommend anyone. You may even be able to get into the VA since you're a military wife - use any and all connections. You can also do your flyers on postcards and mail them out to specialists in the area (use postcards because postage is cheaper). If you decide to use any of your past reports to show as an example of what you can do, make sure to remember to delete any personal information about the patient from that file. The next step is to make follow up phone calls - get the names of the office managers that deal with hiring transcriptionists - now would be a good time to even offer like the first 500 lines free or something like that, just to get your foot in the door. Since you said you already have Christmas pretty well covered, this would set you up for getting paid by December or January. You can also make personal visits to the doctor's offices, just to drop off a flyer and give them a face to put with the name. Any place that says they aren't looking right now, keep at them because eventually they will be and then they'll remember you. I did all these things when first starting out and came up with three really good accounts, one of which I still work with today, seven years later. Straight. My reports have no PHI none whatsoever on them or in them or anything to that nature. I know exactly how the confidentiality works, I have had 5 years in a hospital facility doing more than just medical records, so I should know the HIPAA law, legal and nonlegal regulations. Many of you don't know if these were reports saved from my schooling or nothing. Many of you are just jumping the gun as usual, and it don't surprise me none. I am not getting sassy, but I think I left out additional information to let you think that. My fault on that part. You all have a nice day

First, that would be a HIPAA violation. Second, I am positive the entity you worked for did not allow you to keep copies of work for personal use. Bad idea. Hopefully, any group you show that info to would be scared to work with you.

I save my reports and hang onto them for a while in case the doctor needs a copy, also so the next time they dictate on that patient I don't have to re-type everything, saves me tons of time as I am not the fastest typist in the world. Granted I don't show them to anyone as the OP infers, and she can do that IF she edits out all the information pertaining to patient name, drs. names, etc. I bet most MTs keep copies of their reports, it would be foolish not to. My one doc calls me at least once a week looking for a report, though it is up on the FTP they won't retrieve it themselves, I end up encrypting it and emailing it to them. So if I did not have a copy then what would I tell them, sorry, guess you will just have to tough it out and go to the FTP because I cannot help you since I save nothing. --- My work computer is just that, no one else uses it, and that is all I use it for.

Blackswan, not to split hairs but if you have reports with no PHI, they can be saved. I agree that I'd make sure I was covered, but the key is making sure there's no identifiable information included in the report (birthdate, name, address, phone, etc)

You can save reports but you have to make sure that all personal identifying information is deleted. Also, you can save them on disk or other media so that you don't have to store them on your hard drive.

I have a few options and don't want to make a mistake, though when I was asking for information about Trans Tech I got a lot of negative feedback about them hiring and firing and not enough work and I could not be happier there. I would not leave [2008-11-19]
I work for a small company as an IC. The company knows IC means I work when I can or want to. I have no set schedule. That is #1 importance to me. But aside from that, the account I work on is really nice. I have run out of work on occasion, but it is not a regular thing and seems to happen mostly when someone new is hired. The new people seem to go in and work around the clock, taking work from those of us who have been there longer and tending to be the cherry pickers, which annoys me somewhat, but that usually dies down within a couple of weeks and then things get back to normal, with enough work for everyone. There are some ESL dictators but not a ton, and many of those are easily understood. The owners of the company are really easy people to work with, very understanding, etc. The pay is always on time, and I feel I earn decent pay for what I do and I feel like I am valued there. I would not leave them now or otherwise. Now my second job is a whole different story. I work as a coder for a large hospital, and i would leave in a heartbeat if I could. I work from home and again have no set schedule but do have to work xx number of hours each week; I just pick the times to work. That is it's only advantage. The work is demanding and difficult. We get no respect for it at all. The stress is incredible as we work on a production standard that is virtually impossible to meet. We have 0, that's right 0, people in our group who meet the standard, and we have 27 coders. The expectations are unachievable, and we are constantly pressured to do more and more and more but we get less and less and less in return. Sounds familiar? However, this is the job that provides benefits for myself and my hubby, so I am stuck there for time eternal (or until Medicare kicks in for us). In my lifetime, I have always enjoyed working, but this is truly the first time I could say I hate my job and mean it wholeheartedly. That is why I feel for so many of the MTs out there who are stuck in situations that are making them miserable. Considering the amount of time an individual spends working, shouldn't we be able to feel good about what we do?

I work for MQ and DocQScribe is one of the best things about MQ. I think it is very easy to use. I use ShortHand with it easily. You cannot really go back and look up out charts but that might be just with MQ. I have used a couple of other platforms and found it more user friendly than the other ones. I have found that most of the ADT information is automated, and I only have to read through it, which is par for the course with ADT information.

Lee Perfect/Inscribe Emdat system [2008-11-17]
I was recently hired by Lee Perfect Transcription who utilizes the Inscribe Emdat platform. I am having a very difficult time trying to get used to working the foot pedal. When I am transcribing and I lift my foot off the pedal the dictation does not stop, just keeps going. I have to almost stomp on it to get it to stop. Is this just my foot pedal, or is there another way to get it to stop just when you lift your foot up and it automatically stops like on other platforms. I sure would appreciate any help regarding this. Thanks so much.

I went that route a long time ago and vowed never again! I want to get a W2, go to H&R Block and get a refund after what I went through as an IC and with my own accounts, I was working way too many hours and got stiffed for $2800 in the end. The company I work for has mixed reviews on this site but they treat me well and continue to add to us rather than take away from us. The benefits have improved over the past few years and I heard they are improving again with a 401k match next year a possibility. I am at the top of the pay scale for now, but that's OK with me too. It is Keystrokes, and I am very happy there. I am sure that there will be a few people who will disagree with me, and there always are those who do not share my opinion, but I have been treated as a person instead of a number. I continue to enjoy working every day, which is very important to me. You won't find a better group of caring individuals. Then again, my experience is good but I make it that way and am proud of what I do!

I was trained by a mentor. She had, at at time, over 25 years' experience in the field, about nine years ago. I have no idea what they teach about this in the classroom, but I suppose my answer would have to be that it was chiseled in stone. LOL!!! However, just for you, I found this informationprovided in the BOS2: Inan independent benchmarking study of the MT profession conducted in 1999 by the Hay Management Consultants (HayGroup), three distinct professional levels for medical transcriptionists were identified and described. The first item listed for the Professional Level 3 states, Recognized as possessing expert knowledge of medical terminology, anatomy and physiology, disease processes, signs and symptoms, medications, and laboratory values related to a specialty or specialties. Level 2 refers to the sameknowledge, but as seasoned rather than expert. Level 1 refers to the sameknowledge, but as basic rather than expert or seasoned. I would think that an expert level, even a seasoned level, would include available dosages and what the medications are used to treat, wouldn't you? Does this mean you have to know this? No, I don't think so, but I would also think the transcriptionists with expert and seasonedknowledge earn more than those with basic knowledge. Hope this helps!

I just read this by a poster: No, MTs are NOT supposed to KNOW if a med is generic or brand. HOW COULD WE? I sat here scratching my head thinking, is this person SERIOUS? Do they TRULY believe that they don't need to know simple things like if a drug name is brand v generic? I read so many people here dissatisfied with the gig they are doing. Getting fired for no reason. Ticked off at QA when they only had 6-7 errors. What is it you think and MT *DOES*? The job is to translate the spoken word into an accurate and coherent record. That's what the job REALLY is. And to take pride in knowing you did just that with the emphasis on ACCURATE. Yes, the pay sucks for some services. We all have the option to CHOOSE who we work for. If you can't tolerate the pay, then don't take the job. Your JOB is to produce a chart-ready, accurate medical document.... and yes, Virginia sometimes that means you have to recognize that when the doc says, pulse 142/90, that the report needs flagged; and it might mean you have to look up new cancer drugs or whether or not something is brand or generic. If you're doing your JOB, then you learn what's brand or not and don't have to look it up; you know just where to go to grab oncology studies; you know what sites/references are accurate for surgical terms. You DON'T demand that it's not your JOB to KNOW how to make an accurate medical document. And if your retort is thatservices pay crap so you give them crap? Well then please, PLEASE don't complain when you're also TREATED like crap. PRIDE - it's gotta start somewhere.

the owner sends out UGLY and NASTY emails to the MTs. The communication is just a disorganized mess and they hire you as an IC, but pretty much treats you like an employee. Emdat sucks and pretty much the accounts are not that great. VERY hard to get your lines in Emdat platform. They LOVE to FIRE....when in actuality.....(that is the term they use)...you cannot FIRE an IC, you can lock them out of the system and I guess that is firing, but those two words have always been a pet peeve of mine....hearing when MTs are fired as ICs....Just my opinion ******* DISCLAIMER *****************Postings of this nature are the poster's personal opinion should not be considered as substantiated by MTStars.
