It may be harsh, but it is true. Too many people come into transcription with false sm [2008-11-14]
expectations. They expect to get paid to stay home, watch their kids, work when they want, make a fortune after 6 months of correspondence courses.
It takes more than that to be a good MT and a heck of a lot more than that to be a great MT.
With this economy, it would pay to fine-tune skills, work on typing speed, learn new specialties. The best will survive and I intend to stay one of the best.
Not everyone is good at everything.

Acusis transcription test [2008-11-13]
Has anyone taken the test for Acusis? I attempted to take the test...first of all they warn you right off that it will take 3 hours, and that you have to complete the test in 3 hours, nonstop. So I thought I could handle that but once on the site I find out you cannot use a foot pedal but have to use function keys for the dictation (but of course the function keys didn't work for me) so I had to manually stop the dictation plus you could not slow the dictation down. After doing 1 report and an hour was up, I decided it just wasn't worth it. Did anyone else experience these problems. If not, how did you get through the test? Thanks for you help.

CTS (Central Transcription Service) [2008-11-04]
Again, and thankfully the last time I'll have to deal with this, I was not paid on-time (hopefully I will get paid considering I quit) I am awaiting my final paycheck from CTS which should have been, according to the payroll department, direct deposited on Friday the 31st. After complaining because my direct deposit was not posted I was told it was becaused I resigned, that they mailed the check yesterday at 12:00 p.m. Of the 3 months that I worked for them I was paid late all but 2 pay periods. If you want to be paid definitely do not consider working for them. Promises are made and not kept, yet they continued to ask for extra help when they were behind. I guess they don't get that most of us work for income, not because we are philanthropists and are working out of the goodness of our hearts.

This is the one based out of Watertown, WI. Christopher Stoiber is the information systems manager. They had an ad posted online so I responded. They ask for a resume, which I sent,then heasks me for a time at my convenience that day or the next to call me to discuss things. I sent the resume and a time - no call back allweek, no nothing. I get sick of waiting and forget about it, then happen to see on their website the position is no longer posted.
I'm reallydisgusted withrude and unprofessional, so-called business people.

The best way to get your own accounts [2008-11-20]
Come up with a flyer that states what you do (transcription), what your costs are (and don't sell yourself cheap - you may need to get new equipment or with working locally, it may involve going to pick up microcassettes and then taking the transcription back to the office), and what kind of Turn Around Time you can provide. Really sell your experience and the quality of the work you can do.
Then, start local. Go to your own doctor, OBGYN, etc and see if they need any help with transcription. If they don't, see if they can recommend anyone. You may even be able to get into the VA since you're a military wife - use any and all connections.
You can also do your flyers on postcards and mail them out to specialists in the area (use postcards because postage is cheaper).
If you decide to use any of your past reports to show as an example of what you can do, make sure to remember to delete any personal information about the patient from that file.
The next step is to make follow up phone calls - get the names of the office managers that deal with hiring transcriptionists - now would be a good time to even offer like the first 500 lines free or something like that, just to get your foot in the door. Since you said you already have Christmas pretty well covered, this would set you up for getting paid by December or January. You can also make personal visits to the doctor's offices, just to drop off a flyer and give them a face to put with the name. Any place that says they aren't looking right now, keep at them because eventually they will be and then they'll remember you.
I did all these things when first starting out and came up with three really good accounts, one of which I still work with today, seven years later. Good luck to you and I hope even just some of this helps you out. =)

Straight. My reports have no PHI none whatsoever on them or in them or anything to that nature. I know exactly how the confidentiality works, I have had 5 years in a hospital facility doing more than just medical records, so I should know the HIPAA law, legal and nonlegal regulations. Many of you don't know if these were reports saved from my schooling or nothing. Many of you are just jumping the gun as usual, and it don't surprise me none. My son's pediatrician and my own doctor have English as their second language and I would bet my life and my son's life on both of them. They are wonderful people. Both of them said they would love for me to tanscribe for them, too, if they did not have their own service. I guess we all get to do in life what we are destined to do. I do see these so-called ESLs as people who provide great care and honestly I have transcribed for so long, I don't even blink at who is dictating. It is the dictator in any language who disrepects the medical record by misspeaking terminology or just plain disrespects the Transcriptionist in that they think we read their minds that I think less of, in which case I leave a blank. They can fill it in, and read their own mind. LOL.

If you're saving reports, I hope there is ZERO identifying data in them. (Look up PHI and HIPAA)
Other than that, make up a flyer and cold call the medical buildings. Since it is the holiday, take a page from the Pharm Rep book and take a gift with you- cookies, etc-- they will remember that.
You also need to determine the rate being paid in your area. If you go in too low you'll be sorry and if you go in too high you'll likely get no work.
Good luck! Having your own accounts is 24/7 and the volumes are no more predictable, but you then at least have only yourself to hold accountable :)

There is an old saying that never attribute to malice what can be attributed to stupidity and I can only hope that it is just the function of being naive that causes people to save reports. Having worked for legal departments of large hospitals, I can tell you that this is a violation of HIPAA, of local laws, likely of hospital bylaws (or of the bylaws of the corporate entities that make up most medical practices these days) and that whoever is doing this is shooting themself in the foot. (As well a the pocketbook if they have to retain legal counsel to defend them in an invasion of privacy suit.) I really hope that people doing this will reconsider what they are doing. THINK, people!

Most of these folks haven't got a clue as to the seriousness of what they are going. God help anybody who stored my medical record on their private computer. I would sue them from here to eternity. This is one of the problems with people working at home. I myself have legal insurance covering me and have a very clear understanding of HIPAA. I have a feeling that many docs just hire the cheapest and do not realize that they are endangering their patents' privacy and their own incomes. If the privacy of any of these medical records is breached, the lawyers will come after the docs (who have the deepest pockets) but the MT will certainly be right on the firing line as well as he or she should be.

I really don't have anything bad to say about Transcend. they have been a great company for me, very flexible. In fact, any other company would probably not be as flexible as they have been. If you are struggling, they will work with you. They may outsource some work, but they are definitely not India. They are based out of Georgia. All of the team leads and managers are right here in the United States. IMO, one of the best and fairest transcription companies around.

I tried really hard on an account with Transcend that had 90+ plus ESL docs, and could not continually meeting the 98% error-free goal, or really make any money. I have been working many, many years in all phases of medical transcription, and this was the hardest account I had ever worked on. I liked the people, and the training was good, but just could not live up to their expectations with that account. At my age, I got frustrated and quit.

What is wrong with you to judge? Do you think every MT has to be just like you. Every business has the high achievers and the ones who struggle, possibly to find they have chosen the wrong field, or the wrong company, specialty expander.
Your ladies who toss off your smart a$$ comments at every chance are part of the reason MTs are considered difficult.
If you are like this 24 hours a day, I have a feeling you spend a good deal of time MTing because there is no one else in the house with you. Who'd want to spend time with a person who could cough up a post such as yours? A masochist possibly.
Find yourself a sense of humor and until then, cease posting in representation of medical transcriptions. We have more class than that where I am in this business.

Lee Perfect/Inscribe Emdat system [2008-11-17]
I was recently hired by Lee Perfect Transcription who utilizes the Inscribe Emdat platform. I am having a very difficult time trying to get used to working the foot pedal. When I am transcribing and I lift my foot off the pedal the dictation does not stop, just keeps going. I have to almost stomp on it to get it to stop. Is this just my foot pedal, or is there another way to get it to stop just when you lift your foot up and it automatically stops like on other platforms. I sure would appreciate any help regarding this. Thanks so much.

The best I've ever been offered was 8 cpl for standard transcription on an account that only had editing at half that. The MT business just does not seem fair anymore. Sometimes I think they don't want us to make a living. But it is not like they can survive without us. Besides greed, what are they (MTSOs) thinking these days?

Geez, I've been trying to find info on this company. I was hired in July and it's been non-stop confusion and change, just TODAY someone actually told me they are going through a transition from Indian to American support. My training was all messed up, hired as a QC and wasn't even trained to do transcription for this company, then got moved to do transcription on a second account because there wasn't any work for TWO weeks on my QC account. Now, I'm in TRAINING constantly again, still not actually working. Has anyone else had these problems?

The top speed we have observed was a type-up three times as long as the recording being transcribed. This record was achieved under the following conditions: a very rapid typing speed of over 250 strokes per minutes using the ten-finger system, USB footswitch support, a good quality recording with clear sound, simple transcription rules.
On average the transcription duration for a text with simple transcriptions rules is around five or ten times the length of the recording.
How can I transcribe as quickly as possible?
Typing up verbal data is a longwinded process. You will usually require between five and ten times the amount of time that the recording lasts.
You can speed up the process by up to 30% by using a footswitch. The slower your typing speed, the more the footswitch can help. Even if you type very efficiently (with a typing speed of over 220 strokes per minute) and have until now always used key combinations to operate your transcription software, a footswitch can still make the transcription process 10-20% quicker.
Do take breaks. This may sound trivial, but it is not possible to transcribe for eight hours at a time without requiring medical assistance! We recommend one five to ten minute break per one hour of computer work, which is a recognized standard. It is extremely taxing to transcribe for more than six hours a day.

I was trained by a mentor. She had, at at time, over 25 years' experience in the field, about nine years ago. I have no idea what they teach about this in the classroom, but I suppose my answer would have to be that it was chiseled in stone. LOL!!!
However, just for you, I found this informationprovided in the BOS2:
Inan independent benchmarking study of the MT profession conducted in 1999 by the Hay Management Consultants (HayGroup), three distinct professional levels for medical transcriptionists were identified and described.
The first item listed for the Professional Level 3 states, Recognized as possessing expert knowledge of medical terminology, anatomy and physiology, disease processes, signs and symptoms, medications, and laboratory values related to a specialty or specialties.
Level 2 refers to the sameknowledge, but as seasoned rather than expert.
Level 1 refers to the sameknowledge, but as basic rather than expert or seasoned.
I would think that an expert level, even a seasoned level, would include available dosages and what the medications are used to treat, wouldn't you?
Does this mean you have to know this? No, I don't think so, but I would also think the transcriptionists with expert and seasonedknowledge earn more than those with basic knowledge.

I just read this by a poster: No, MTs are NOT supposed to KNOW if a med is generic or brand. HOW COULD WE?
I sat here scratching my head thinking, is this person SERIOUS? Do they TRULY believe that they don't need to know simple things like if a drug name is brand v generic?
I read so many people here dissatisfied with the gig they are doing. Getting fired for no reason. Ticked off at QA when they only had 6-7 errors.
What is it you think and MT *DOES*? The job is to translate the spoken word into an accurate and coherent record.
That's what the job REALLY is. And to take pride in knowing you did just that with the emphasis on ACCURATE.
Yes, the pay sucks for some services. We all have the option to CHOOSE who we work for. If you can't tolerate the pay, then don't take the job.
Your JOB is to produce a chart-ready, accurate medical document.... and yes, Virginia sometimes that means you have to recognize that when the doc says, pulse 142/90, that the report needs flagged; and it might mean you have to look up new cancer drugs or whether or not something is brand or generic. If you're doing your JOB, then you learn what's brand or not and don't have to look it up; you know just where to go to grab oncology studies; you know what sites/references are accurate for surgical terms. You DON'T demand that it's not your JOB to KNOW how to make an accurate medical document.
And if your retort is thatservices pay crap so you give them crap? Well then please, PLEASE don't complain when you're also TREATED like crap.

Expectations [2008-11-16]
I work for Synernet. One of the best gigs I've had in a long while. Having been on both sides of the MT fence (as owner and now as grunt), I appreciated their up-front manner- COMMUNICATION IS KEY. There are expectations on both sides, and yours are no less or more important than are theirs.
If you were fired for medical reasons, contact an attorney.
But just reading the list you ticked off, sounds like you have lots of issues that would preclude them being able to meet client TAT. And sorry kiddo, but that means you're not cutting it, even if your reasons sound valid.
There aren't a lot of businesses who will tell ya, go be sick, hospitalized, have earthquakes, and have an imperfect family and we'll just wait here patiently for your problems to compound ours.
Being an MT at home doesn't mean you don't have the rules that ALL employees for businesses have... and just saying, sounds like your situation is not a good fit.
