Line shortage [2008-08-04]
Ok, so I got a note from my PS that my line count was at 147 lph instead of the 150 lph needed for the FT, full bene status. Ok, so a week where you fall short. Life happens.....However.....the line count that he has and the line count that I have are off by 195 lines. Not only do I check my line count for the previous day at the beginning of every new session, but I also figure out how much I made doing transcription and ASR and then I also figure out how much I would have made on an hourly rate for the day. I just like to know where I am.
I guess I am wondering if I need to watch payablelines on my check. Does anyone else have problems like this or does anyone check their pay check closely? I don't and now I am wondering how many other times I have been shorted.
E-Mail shortage [2008-06-23]
Anybody notice they are not getting much E-mail on the new system? My setup is fine, but I very rarely get E-mails from PS re workflow or anything else. Used to be at least one every couple of days.

for MQ, anyhow. They continue to make decisions that give the MT no choice but to quit. I sincerely feel like they want us to quit. Why else would they continue to shove 'it' in our faces? Yeah, shortage of US MTs because US MTs are wise to MQ's manipulation and the national pool, etc. Who would really want to work for a company with this mentality anyhow?

There is a HUGE shortage of qualified US MTs who are willing to work for 3-6 cpl. There is NO shortage of qualified US MTs willing to work for a fair wage by US standards--and I mean at least 10 cpl! Take this MQ job and shove it if I can't get over 10 cpl--oh wait, I already have....shoved it, that is.

Articles in my paper recently point to shortage in doctors because we are allowing less foreign MDs in since 9/11 (yeah, right) and nurses are in extremely high demand (that's because they are all dictating discharge summaries)!!

Then, we are supposed to have primary and just a few secondary accts according to what I was told...but still with backups to those also, much the same way it is right now.
Probably nothing will change. I just don't know.
They're trying to recreate the wheel and I just don't understand. All I see is wasted resources from the people who made this decision (the time and money spent on the convoluted bonus program and the time and money spent on this decision for realignment) down to the utilization of my *talent*.

I learned MTing bec a hospital hired me as a trainee and gave me 6 months to learn. I am not a genius. I don't have a course in MT. I have a high school education, a summer of typing, and 2 years of Latin (which I think helped somewhat). The rest I learned by the seat of my pants and because I was given work to do. You can't learn MT any other way than to DO IT! You can take classes until you are blue in the face but this is a FLUID profession -- the words change, the favorite diseases change with the wind, the drugs are constantly changing. You can only learn through doing it.
Hospital belly ache bec they just have never wanted to acknowledge that what we do is necessary -- if they don't want to write it themselves. I was in the field just as they were going from docs writing their notes to us typing them.
Hospitals are getting ripped royally and yet they are so stupid they don't stop to look -- all they know is they want the reports to mysteriously appear.
Well, they get short of nurses and they can find the grants to open scholarships for nurses and programs to trains nurses -- they should realize that it's better, cheaper, and it turns out better pt care because there is tighter control -- if they will have it done in house -- WHICH BASICALLY MEANS YOU CAN STILL DO IT AT HOME because of technology today -- they can see every key we hit, everytime we have our foot on the pedal, they can see us clock in and clock out -- it's not a big problem to have a staff of MTs just like they have everything else in a hospital.
I just wish they would get burned enough to learn that. And also to learn to give basic speaking skills to their ON STAFF PHYSICIANS to see if they can speak the language (even if they are from Alabama!) well enough to be understood by all people, not just us -- and that they are given a course in what the hospital EXPECTS as quality of the dictation as it reflects on pt care.....and that along with checking the charts for timeliness -- they also LISTEN to the dictation to verify that what is typed is what was said......and if what was said was INTELLIGIBLE.
That's my 2/cents worth.

You know what really astounds me. That these managers can tell you it's just this or that when they have absolutely no Qlue. Do they have spread sheets that average incoming work from their accounts that they can compare to MTs who are scheduled to work? Do they see the average from last year declining or is it the same meaning too many MTs on one account. Come on PS and WFC coordinators. There are ways to distribute work and it's your job to do it. Blindly approving everyone's schedule and then telling everyone to make up lost time due to your poor plan on the weekend does not make a good PS or WFC.

What I meant was - this quarterly report is a legal document filed with the SEC, govt, whatever/whomever. So, MQ just states as fact that there is a shortage of US MTs as their sole excuse for using ILPs. That is a bunch of hooey. Its like Turkey Hill ice cream getting rid of vanilla, and only producing chocolate due to the shortage of vanilla beans. Doesn't anyone in govt hold companies accountable for the rubbish they publish, let alone file with the SEC. That's what I meant - when things are filed as fact in legal documents to the government - is that legal? And, yes, that was my point - we need to do something about this. If we could reduce the offshoring by pointing out the lies - anything. Would that help us? What can we do?

and tired of hearing this jabber about the shortage of US MTs. You have to read this long convoluted mess to appreciate it, but the bottom line is this... First MQ openly admits to a huge loss of revenue due to lost accounts. So, no rumor or innuendo there. Straight out of the donkey's mouth, soto speak. Second, they blabber on again about how theyHAVE to use overseas help due to the severe shortage of US MTs. I am so sick and tired of hearing this little motto thrown around all the time. Seriously, I want to do something about this, as I believe it is 100% bunk.It just dawned on metoday that nobody challenges this, even the government. MQ just states this as fact, and nobody questions where they get this supposed fact. We offshore our own accounts and then ourUS MTs don't have work. Period. We all know this, as we live it 24/7. If you still have your primary, just give it time. And now the offshores are ever so cleverly disguised as US entities, and MQ certainly knows this. In fact, we probably own them somewhere down the pike, under another company name or whatever. I'm sure all legitimate, but done in the shadows, so as not to let US MTs know the truth. So, my question is - does anybody know for sure where they are getting this FACT from? Is it a report by the lovely AAMT, or whatever they are called now - AHDI? This makes me so very sick to hear all of this garbage. There is no shortage of MTs, and as you stated our work here is being undercut and taken away by offshoring our work. MQ is making out big time by turning us into editors, and it doesn't take a rocket scientist to see that. Our pay has been drastically cut, and now that we cannot make our line counts they are taking away our medical benefits, which obviously works out fine for the company. I feel so low these days, and I have said this before that I feel our wages are being taken away from us. How can we possibly stand for this, especially in this country. No, I don't think the government is aware of this. I had to give my employer the other day at the doctor's office because of the change in employment and guess what, they have heard of MedQuist, and know they are the biggest transcription company around, yet when I told them they offshore they found it hard to believe.

Anyway, I think it's just going to get worse and worse. There are so many ESL docs now (I read somewhere upwards of 60-70% at this point?) because of the crisis in terms of lack of American doctors, and there seems to be no plan to build more American medical schools and encourage more Americans to become doctors. In other words, the doc shortage just seems to lead to more and more behind-kissing of whomever they can get.

While I was listening to the radio today, I heard an interesting commercial advertising a free seminar in a local town regarding working at home doing medical transcription. It went on to say how there is a shortage of medical transcriptionists. They statedthat good candidates would beif you know how to type or want to learn how to type and have the luxury ofworking from your home and making more $ than in an office. They did not say if they were a MT company or an MT school, but they sounded very excited about it. They must not be transcriptionists! If I had the time, I would go. When they were spewing out all of the lovely adjuectives about being a medical transcriptionist, I wanted to puke.
I have not worked for a month now and do not miss it a bit. It would be very hard to come back after my FMLA is over and deal with the emotional stress this job gives me on a daily basis. I would rather be at Walmart greeting customers!

According to the video by the CEO on Qnet, there is a shortage of qualified MTs. This is the reason why they have to outsource. They must think we are stupid. If this was the case, our wages would reflect the shortage. Supply and demand therapy. This whole industry has just headed south. I know I am making what I made probably 10 years ago.

If they are now short many thousands of transcriptionists, even with speech recognition and other technologies, it seems like there shouldstill be plenty of jobs available.How big a portion of the Transcriptionist industry does Medquist have? If Medquist off-shored all work,there still might be work available for us in US transcription companies. I wonder why there is such a shortage of labor here in transcriptionists, physicians- seems like a large percentage of ESL.

I for one am riding the storm out. I have to work, so until they give me a pink slip or shut the doors, I am hanging. Not to say I am not gonna dust off my resume to be prepared. Compared to what I personally have been through in the past year, this is small potatoes. I do promise this is not the worst thing that is going to happen in your life. Just keep doing the great jobs you all do, and it will all work out.
For those of you about to flame me for being a cheerleader, look at this way. If you hang in there when the times are tough, it will look that much better on your resume as a dedicated employee. There is no shortage of jobs out there, just look at the other boards alone, not to mention good old fashioned newspapers, and have you ever Googled? One thing I have done is started a little savings, instead of buying a takeout pizza, I toss some burgers on the grill and put the pizza money in savings. It adds up quickly.
Something my dear husband had always taught me, and I still hold tightly in my heart, was prepare for the worst and expect the best. Make the best of what you have. Little did he know how those words would carry his children and me through much tougher times than if a company is being sold.

Ya know, this is the sort of thing that makes me nuts.... someone posts some vague, but clearly knowledgeable stream that sends people into the panic stage. How fair do you think that was, especially since you have no intention of elaborating on why we should get OUT NOW, etc.
the last rumor I heard about MQ, from ex-staffers, was that the MTs were starting to be paid at 25% less or so, because MQ will stop counting spaces in the 65 character line count. I can't fathom how they could do this, 65 characters including spaces is the industry standard. BUT, even if they did make this sort of change, they have to inform the staff at least 30 days in advance of the change.
I know from experience, with MQ and another MT company, that there is a shortage of 30,000 or so qualified MTs in this country... that is comforting.
If you want to elaborate on WHY we should not panic, but look for other jobs NOW, great... if not, its really not appropriate to start a panic with no backup info....

Someone below referred to the MT business as being'viciously competitive.' I get that. And Ibelieve that it is, from what I've seen and heard. But I guess I don't quite understand *why* that is.
I mean, do the hospitals need us to transcribe their reports or not? Yes, they do. If they didn't, we wouldn't be here. So why do *they* get to dictate (no pun intended LOL)how much (or should I say, how *little*) they're willing to pay for the professional service we provide? (And you know that saying, 'you get what you pay for.')
If we didn't transcribe their reports, what would they do? Have a backlog, be in violation, lose their JCHO accreditation, etc., right? (Never mind what it would do to patient care.)
And we're always hearing about how there is a shortage of qualified MTs. So, what I can't get my mind around is the whole 'supply and demand' aspect of all of this. If there aren't enough of us to do the work, and they musthave the work done, why do the hosptialsseem to have all the power in setting the price lower and lower andlower.
I know, I know, the technology of the Internet, ASR, the cheap labor of outsourcing, etc. all play into this too, but where will it end...?

PSs. There were not more than a hundred or so in all. The statistics are broken down below. Basically most PSs were laid off and I guess all WFCs were, too. Unceremoniously by a phone call out of nowhere. QA, too, while there is an urgent shortage of QA. Do you think we're gonna get our Quality Knowledge Leaders now, and all those supervisors to help intervene with QA feedback? They are eliminating all humans, replacing with TSO for scheduling and QASAR for feedback and QA scores. When they have their dream, ASR will have replaced all MTs. Seriously, ya all don't think that is not the ultimate goal? They are beefing up ASR with classes and reclasses to be sure we all get it right, which is to get it out of here and over to our International Labor Partners for final ASR editing. This is the plan, ladies and gentlemen, and all your previous managers and supervisors can vouch for this. They just didn't think they were in the line for the firing squad. And it serves some of them right - hanging MTs and QA out for slaughter, thinking they were safe. That's the hoot with this company, and any company. Nobody is safe anymore. I have a second job, but now am so outta here with a new full time one. I just needed this kick in the can to get me off the fence, and I am off it. I can't work for people with these scruples any longer. I got my TUDE back - the abuse is over. I'm tired of it and I'm outta here! But not yet, moderator, so I'm still safe to post!
I Don't Have An Answer [2007-09-11]
I really don't have an answer for that one. I do know that in the past several months (perhaps longer) there has been a QA shortage and the PSs were told to tell all MTs who were interested in QA to go and take the test.
I do not know what the day-to-day duties of a PS entail, but I am under the impression that they already had more than they could handle.
In a way, I wish that I had been one of the 'lucky ones' who got to keep their job, but at the same time, those who are left are really going to have to step it up and add even more duties to their already-overcrowded day. I don't know how any of them will handle this.
My husband's employer did this a couple of years ago. They were 'downsizing' and 'cutting costs,' so they combined 2 positions into 1 and laid off those who were left over. Six months later, they realized what a major mistake this had been and they went into a scramble to try to undo what they had done. It took nearly 8 months to recover and get things back to a 'normal pace.'

Otherwise, there are now 2. I can totally see the PS restructuring. The new program that came out for scheduling obviously was going to replace them. But with the huge shortage of QA and backlog in QA, what on earth excuse are they giving for laying off QA staff? This makes no sense and sounds very fishy to me. QA dept is vital and already in trouble.

Okay, there is a shortage of nurses and apparently too many of them are used to dictate. We can do that! We know how to put sentences together, how to pronounce drugs, the correct meaning of words, and the fact that every noun cannot be made into a verb. We can look through charts for lab values and dates. I think we could do a much better job of this and free the nurses up to use their skills.

I think what might happen is that so many people are going to leave the MT/ME field because of the lower wages, thatthere is going to be a really severe shortageof experienced MT/MEs, even worse than now. Hopefully, that will mean our wages will be driven upwards again to keep thereally good people from leaving the field.
There could be severe ramifications for the health care field if there are mostly inexperienced MT/MEs, so I think they are going to be crying for us oldies but goodies and maybe realize we are worth more than they thought.
From experience, I have found that most new people take at least a year to get anywhere near being very good and/or fastIF they are going to atall, and IF they do not poop out before then(no offense toyou newbies).
Like was said in an earlier post, a lot of the new people are just wanting to go into this field because of the promise of $40-$50,000 peryear. When they find out this is notnecessarily true, they are not going to stick around unless they really, really love their job.
Therefore, again, wages will need to increase to get and keep really good and dedicatedpeople.

Articles in my paper recently point to shortage in doctors because we are allowing less foreign MDs in since 9/11 (yeah, right) and nurses are in extremely high demand (that's because they are all dictating discharge summaries)!!
Profession in decline for years [2007-07-24]
I've been an MT for 30 years. Our profession's problems go back well before the work at home boom. Thirty years ago our hospital bosses, the ARTs and RRAs, had no respect for what we did. According to them, anybody off the street who could type and use a dictionary could do the job. Oh, and any minute now there wouldbe a machine the docs could talk into and perfect work would come out the other end. So sit down, shut up and type. Be grateful for any crumbs you might be thrown because at least youhad a job. Even my brother, who worked for a railroad, had more sense than to think that what I did was typing pool stuff. He knew I spoke and understood doctor and said it with the same respect he'd use for someone who could speak Russian, German, Chinese or whatever.As a profession, we never did gain respect.Without respect, we never gained in pay. We've always been seen as amommy track job, a good way to earn a little mad money. Being able to work at home made it worse. Out of sight, out of mind. You're not even a person anymore. There has been a shortage of MTs for some time, but even the vaunted law of supply and demand doesn't seem to apply to our industry. Back when, if there weren't enough engineers or teachers or whatever, you paid more to draw people into the field. That never happened with MT, and with offshoring, now it never will. If you don't respect workers, if you don't pay them well, if you don't offer decent benefits, you will not attract quality people. The best and the brightest have other options, and that is particularly so for women when compared to 30 years ago. If we don't have enough quality MTs, it certainly isn't solely the fault of MTs themselves.

I have only been on this board for a few weeks now but during this time I have seen quite a few reactions to everything that is going on. I have been fighting the urge to post here because I know I will accidentally offend someone here and/or stir something up even though this is not my intention. Before anyone questions it, yes I am an MT and have been for 10+ years.
I am the first to agree that the current changes are hard to swallow, especially all happening at once: shortage of work, change in bonus, cut in ASR pay, nationwide workpool, etc.the bonus changeand ASR cuts are very difficult to understand as it hurts our pocketbook, especially when we see our hard earned money go to a 120,000 month salary. I am not condoning this; however, I know I cannot change this decision.
As far as the shortage of work/nationwide workpool I am a bit torn here. I actually work a shift that I never had a huge shortage of work; however, I have always asked to be put on a new account any time I had a shortage of work. I have also worked for a company who used to have a nationwide workpool and while it takes some time to adjust to the new accounts I believe it makes us better transcriptionists.
Yes, our pay will suffer in the beginning but this will improve, it is overwhelming at times to have to look at yet another CP and try to remember all the specifics but I believe we are selling ourselves short to say we cannot do it, not to mention making ourselves look silly fighting amongst each other over this.
I believe we are in control here. We are the ones with the power to make the best quality report possible, we are the ones with the power to decide whether to continue to work for Q or move on and we should be the ones supporting each other with whatever decision each individual finds necessary for their own situation. I personally am staying right now. I have worked for several companies and I haveseen good and bad in each one ofthemI am chosing to focus on the good. When the time comes I am going to walk away from the Q being that much more of a well rounded MT with that much more experience making me an asset to whatever company I may go to in the future. But please, can we stop beating each other up and agree to disagree.

this is the way the corporate world seems to work now. We, the experienced MTs, believe they are taking a loss if we leave. Unfortunately, companies see it as more $$ if they can pay a newbie much less. They figure they still have enough QA in place to cover the inadequacies, and whatever MT shortage remains after the good MTs leave, will simply be compensated for by the offshore MTs, who will work for even less than a new US MT. It's all about the almighty dollar. So, if anyone who has invested many years with a company believes they or their work will be missed, I beg to differ here..I believe we are all just a number.
