old forms [2008-05-09]
ah ha ha ha ha, good one!
Oh get real; these guys make me fill out most of the reports from the patient intake forms. [2008-04-02]
It's been done this way for 22 yrs. I'm supposed to compose a lot of his correspondence myself, too. There are MTs and there are MTs who went to college on National Merit Scholarships and got BS and MS degrees and IQs of 131 and previously worked as technical writers.
Would anyone know which forms? [2008-01-19]
certain forms you have to use when claiming deductions instead of or in addition to the usual 1040? thank you
W2 forms [2008-01-15]
does anyone remember when our W2 forms are mailed out? TIA
finding forms on Qnet [2008-01-05]
It states that when you want to go part-time, you have to fill out a form. Can you tell me where I can find the form?
why would you mess with the forms, just [2007-12-02]
have the dental office send it in for you!! Maybe they don't do that? I've never personally had to handle insurance forms, figure that's what the offices pay their insurance employees for...
Not insurance forms. The flex spending medical forms. We have SM [2007-12-02]
to do that ourselves.
May I please vent. I am so tired of ESL docs who don't know how to use singular vs. plural forms [2007-11-28]
Example: The patient's vaccinations IS up to date. This is a 35 yearS old man. The patient HAVE history of hyperglycemia. Drives me absolutely nuts. If you read this, thankS you for letting me complain.
going from FT to flex.....forms? [2007-10-26]
Going to flex PT 11/01. PS stated over a month ago she was taking care of the change but have never heard any more on this. Should I be receiving forms or some other verification? What about COBRA information? Don't want to bother PS unless I have to and besides she often doesn't reply and seems to receive only some of my emails.
Yes you are. They sent me the forms, Expensive though. nm [2007-09-20]

BOS says do not abbreviate in headers, even if dictated that way, no abbreviations, short forms, etc [2007-06-25]

Thanks for posting. I have the Tax Forms to show how much $ I lost from 2000 to 2006. [2006-06-18]
I went from making $42,000 a year to the last year I was with MQ I made $14,000
and that is no lie and I have the papers to prove it. I can show you when I signed on, what I typed, and my line counts.
I continually ran out of work the last 2 years, with never an explanation of where the work was.
In that 6 years we lost our bonuses among other things, which took a big chunk of change out of our pockets. In other words, my hourly rate was being cut incrementally every 6 months or so, in a back-door manner. No, they didn't lower my rate per line, they cut my pay by taking away built in bonuses.
Then when I was being made to commit to hours to be there, and yet there was no work to do, that lowered my hourly rate average even more, because I had nothing to type. Add to that, we lost the Sat and Sun bonuses.
So, that slippery hard slide from $42,000 to $14,000 was subliminal but it was definitely there. All the while I was being held responsible for my overhead.
I don't see from posters on here that much has changed in the work area - so what is going on with that company? And why can't they give straight answers?
"By law, employers must send out W-2 forms to their employees no later than January 31" [2006-01-21]
I believe they can postmark them January 31, which with mail time means you could receive them in February...
forms we took to manpower [2005-10-20]
I just called the MT/ME rewards desk at 4:45 p.m. EST. I wanted to know whether they ever got my papers from manpower. Well everybody that takes care of that leaves at 4:30 according to greg. Anyway you all will find the following quite interesting:
At first he didn't get what I was talking about and in the conversation he told me that yesterday a letter went out to everyone who has not turned in their stuff to manpower or they haven't received papers back from yet. He wouldn't tell me what was in that letter though. I don't know what is going on, we will see in the morniing when I talk to them. I don't know why they would be sending anything to anybody now. I thought we had til December 1 to get this stuff in.
Will let you all know what I find out!
forms we took to manpower II sm [2005-10-20]
I am wondering (now I sound paranoid) if they are going through the stuff sent back by manpower and taking the schedules they like and ignoring the ones they don't and saying they never got the forms back. Mine were went 11 days ago. I called the ME/MT rewards desk this afternoon and it was too late to talk to anyone. They were gone home. But Greg said that there was a letter went out yesterday to everyone whom they did not receive schedules back from. He wouldn't say what was in the letter though.
Why would they send a letter if we don't have to have these in until December anyway?
I can't get my supervisor to answer the question as to whether she got my schedule to start with. She dances all around the question, never answers it and the ME/MT rewards desk is totally screwed up!
Geeze I am sick of all of this!
cobra [2008-08-30]
I left MQ on July 10, and I did not get my papers to fill out until the second week of August, although I kept calling to find out what to do. I filled out the forms, sent in the check for $436 because it is just me. I got the check back a couple days ago, that I had made the check out to Cobra instead of Employee Benefits. They said they put the money on to my Vision section (which I did not order) and they needed me to send another check. Of course they will not get it unil Tuesday or Wednesday, and now not sure how long it will take to get credit on my health insurance. So I went to get my meds yesterday and was told that my insurance card did not go through. thank goodness it was just for my cheaper pills and it was only $21.95. Not sure what the other pills will cost me in a couple months. they are supposed to reimburse us for anything we pay out of pocket for.
Hopefully I will get some good insurance with the new job I went to. Hope you got a good job after MQ. I guess anything is better. I just figured out my line count and what my check will be BEFORE taxes and I USED TO get checks from MQ AFTER taxes about 2 years ago,that were more than my total I made at this new place, but it will be a WHOLE LOT more than what I made the last year at MQ.
The only bad thing is my new place is going to almost all ASR and I refused to do it at MQ after I tried it back in 2004 and ABSOLUTELY HATED IT. And it truly has not gotten any better anywhere else. I am truly much faster from scratch typing than trying to fix that ASR stuff. I think we need to be paid more to fix that ASR crap, cuz is sure takes longer to correct than to type from scratch.
Word Expander [2008-08-08]
I've used Short Cut for many years and like it a lot. It works with any program. I have over 5000 entries.
I have recently purchased Instant Text to see if it will help even more as it does a few more things.
I definitely recommend using an outside word Expander as that way you can take your short forms anywhere if you change jobs and not be stuck re-creating your word list.
Both programs are available at www.sylvansoftware.com
It depends on your state. We are "AT-Will" employees [2008-08-07]
which means MQ can let you go for NO REASON at all and there is nothing you can do about it. You signed that paper back in 2006 as I recall when you had to go to anoffice in your local area and fill out W2 forms, etc.
That's one way we started getting suspicious. [2008-08-05]
Our insurance company started writing us letters and denials and forms questioning medications, etc. My husband used to receive epidurals for his lower back region, and the insurance company wouldn't approve this for whiplash victims and told us we had to go thru auto insurance, etc. All the meds I had been on for over 10 years were suddenly being questioned, as they were not approved to treat TMJ, etc. It was a fiasco, and actually still is. However, we figured its gonna be worse trying to go to a new doctor and straighten this mess out for both of us, so right now, we're sort of in free fall, what with all the other disasters going on in the world right now, like trying to get used to a new job, figuring out how to find extra gas $$ and feed a houseful of teenagers.... so much going on at once that there's no time to fix all these messes. I'm sure we're all going thru garbage like this, right? If its not one things, its another - internet acting up, cable down, phone on the fritz, bank messing up, on and on and on. It seems to be escalating these days, and I need A PILL!!!! A large pill!
If you remember on the previous SEC filings and [2008-07-29]
annual or quarterly reports, MQ always reported their only profits fromo the reduction in wages paid for ASR, never a mention of charging clients less. Whenever I asked same in the past, I was told that ASR had cost so much to invent and manufacture, which is where Philips came in, that the 30% they saved from MT payments went towards recovering their investment. MQ smooth talk which I interpreted as as big NO to billing client less. It was always listed on formal financial forms the savings in salary to us by ASR.
I would escalate your request, until a policy is officially posted it isn't one. The supes (sm) [2008-07-10]
cannot make rules on their own or go by heresay... they can get in big trouble legally for that. I used to work in HR and if something was not listed in the handbook and distributed to the employees, it could not be interpreted as a policy. In fact we had to have signed understanding forms on file for the employees and if there was not one we had to be really careful about enforcing a policy (mostly talking about ones that might result in termination or pay). We had to word the policies very carefully so it could not be interpreted wrong... still often was! And the MQ Central has a listing of the polcies and they have to go by it... as do we. It is very serious business if supervisors are making their own policies. Possibly the PS is communicating something he/she heard may happen or might become a policy, but until then whatever is listed on MQ Central as a policy is a policy. If this is important to you, please escalate it immediately! Before they do change it!
no - never. I've been through 2 [2008-06-03]
computers with MQ. When they crashed, they paid for the shipping to return them. The forms I got when I quit are also prepaid. You shouldn't be paying for any shipping with MQ.
I gave my notice in mid April and [2008-06-02]
I just got the forms about a week ago. The return UPS slips come regular mail. I didn't have to call anyone for them; they just came.
If they're that concerned about $$$, how come [2008-05-09]
forms that should have been filed a year or 2 ago? That one still burns me!!!
Well I suffered through a transcription test without the foot pedal. Not much fun. I actually [2008-04-30]
took the word part of it, then took the transcription part of it. Thenthey sent this long form that took about a half hour to complete with millions of questions on it. Then they called and wanted me to take an oral test and another editing test and I thoughtis this nuts. I must have already spent 1-1/2 hours on all this stuff and then I find out they dont hire IC. Well enough for me. I passed all the tests very well and on its goes. I never saw so much testing and filling out forms and you dont even know what they pay. I can see testing but 2 hours worth. That is crazy.
I took LOA a year or so ago as a PT employee...SM [2008-04-29]
There are forms on QNET or where ever they may be now. Your supervisor still has to approve it, I believe, and I think the timeframe on LOA is 2 calendar weeks (regardless of what your schedule holds)
S1-S2 support for ASR [2008-04-23]
For those of you having an issue with S1-S2, there is a customization file we can apply that will allow you to toggle between the two forms. If this is an issue for you, please call support or email support@medquist.com with Route to ASR Support in the subject line, and place a Q-Care ticket. Tell them the issue and ask them to apply the customization that will allow you toggle back and forth.
Since both forms are pronounced exactly alike, ASR has no idea which to use and tries to match up as best it can.
Hope this helps.
bos [2008-04-17]
absolutely, I had one physician actually throw the book between all our desks in the office and said,I don't care what BOS states, I am the dictator and I am asking this department to stop making combined forms that do not exists.
What I remember about sales tax...sm [2008-04-11]
I think it varies from state to state, so check with your state's Dept of Revenue.
When I was an IC before, if I transcribed for an entity outside of my state, I did not have to charge a sales tax, but when I transcribed for anyone in my state, I did... but there were exceptions to that too, which included if the entity was a nonprofit organization or nonprofit hospital, thus making them exempt from paying sales tax. I still had to have a sales tax ID and file the state tax forms, but listed my income on the lines where they had out-of-state income and nontaxable income. I worked for a nonprofit clinic and didn't charge sales tax, and also did some freelance interviews, which I did tack on sales tax.
It's been a several years since I had to do this, so I may not remember exactly, and also I know in my state that over the past several years they've changed some tax codes regarding how ''services'' are taxed, but I haven't researched that since the changes as I don't do any IC work inside my state currently, so I don't know if they would or wouldn't affect MT services (probably considered under clerical).
I'd check with your state and/or your accountant, you should be able to find out about the laws and codes for your state. Good luck.
I agree. She just does not trust [2008-04-10]
this company as in the past W2 forms showed problems before.
Great book for credit and budgeting [2008-03-29]
I'm not sure how many of you have heard of Dave Ramsey. He at one time was into real estate and made a fortune. He eventually hit rock bottom and had nothing. He has a book and videos out called Financial Peace University. IHIGHLY recommend this book. He gives tips on how to handle your credit card companies and how to budget (his book has all the forms you will need). He also has a radio show that comes on in the afternoon (sorry, not sure which station as I haven't listed in a while). He also has a website, www.daveramsey.com. I promise, you won't be sorry for buying this book and putting it into practice. You won't gain financial freedom overnight, but it will happen. I only learned of this book after having to file bankruptcy a few years ago. Not a pleasant road to go down either. Best of luck!!!
taxes. how do i get MQ to start holding federal out [2008-02-27]
didnt even realize they havent been withholding federal taxes. is this the norm or is this something i overlooked when filling out my tax forms??
vision insurance [2008-02-19]
I thought we had to go to a Spectera provider which, in my area, is only Wal-Mart. I then learned from a friend that if obtain the forms, we can go anywhere and file our own claim, paying whatever the difference is on exams,glasses,etc.
Back at ya Rose and everyone else...sm [2008-02-14]
My treat to myself today was downloading the test files for a new job!! It's not in MT though, but I think that it's going to be good.
I think I'm getting burned out... I can't believe I'm saying that after 15+ years of loving being in the biz with 12+ of those here at MQ. It's not just the garbage we've been going through with MQ that has given me no will to work lately, I'm really getting tired of sloppy dictators! It seems anymore like every single report has something wrong with it... it's either an ESL dictator, a careless sloppy dictator, volume so low you can't hear, so much background noise, etc. It never used to be this bad.... even the old tapes used to be better than most of this digital stuff we have now. And sure, we'd get those few bad dictators with every facility, but this past year or so I have run across very few dictations that I have enjoyed transcribing... so if it isn't the MT field in general that's getting worse, then I am definitely getting burned out and it's time to move on.
Hopefully getting back into other forms of transcription will renew my love for my profession again.... but I am also aware that it could possibly send me running back to MT quickly, who knows?!!
I can't wait to get to my test files this weekend and see what I think.
You and all have a very Happy Valentine's Day and enjoy whatever ''treats'' you get or give to yourselves!!
Does anyone know if there is a place [2008-01-26]
online to do the forms to change your 401k contribution? Also, is there a place online to increase your W4 withholding? I have asked PS but no response. Figured I'd get a answer here sooner anyway. Thanks!
Would anyone know which forms? [2008-01-19]
certain forms you have to use when claiming deductions instead of or in addition to the usual 1040? thank you
Yes, Freebie I value your opinion. [2008-01-16]
Unions can take many forms and maybe we already have one (unofficially) right here.
W2 forms [2008-01-15]
does anyone remember when our W2 forms are mailed out? TIA
Not OP, but... sm [2007-12-17]
I was told to file by Jan 31 when my daughter was a senior in high school as we needed our current tax return to start filing for her financial aid. I don't think you HAVE to be filed by that date, but the federal forms for the finanical aid have to be in by February something or other and the earlier the better.
To OP - you should be okay using the YTD totals on your last pay stub. Mine have always equaled what came out on the W2.
what I do ... [2007-12-14]
I increased my speed when I took the time to put in my expansions.. wasn't easy but it definitely increased by lph by about 30-50 regularly.
I am an old PRD user, and kept the same rules ..
i.e.
start all meds with a v and 3 letters, i.e.
vpro = Procardia
vpen = penicillin
For three-word phrases, I use the first letter of each word .. chf = congestive heart failure
Be consistent with how you name abbreviation and expanded words .. i.e.
chf = congestive heart failure
chf2 = CHF
(all abbreviations would have the 2 or whichever way is easier for you)
Typically I stay away from 1 letter abbreviations ... too easy to make a mistake with.
Make short forms for ALL regular words .. and use them.
t = the
r = were
n = and
wo = without
wi = with
hh = has had
pt = patient
pa = the patient
hi = he is
si = she is
I put in words that my fingers just want to type wrong, i.e.
nad = and
onyl = only
If I think of any others, I will add ... good luck to you!
