My company is in the process of moving to Chartnet and are saying the optimum internet speed is 3000 Kbs upload, 600 Kbs download. At best, I get 1200 Kbs upload, 400 Kbs download,but often less. I have no other internet options where I live. Am I going to have a lot of problems? Thanks in advance.

Unfortunately, I think that is why most services will not pay too high a rate to an IC, so the IC will not outsource the work to a third party.
I have satellite internet [2008-10-21]
As long as the only requirement is high-speed internet, I dont' see that there should be a problem. Sometimes you'll see that they say no satellite, probably because they work on an internet platform and satellite can sometimes be unreliable. Also check to see what thier method of sending and receiving dictation is - if it's through an FTP site, you're okay. I have found that the satellite takes a little longer with downloading and uploading, but still works just as good as cable or others. Good luck to you!

Healthcare, along with construction, education and retail are ranked as being the slowest adopters of IT. With more pressure than ever from government, insurers and patients, why is healthcare still so far behind?
As health care becomes more integrated, electronic medical records (EMRs) have become an integral part of any practice.
In its simplest form, the concept of EMR is electronic storage and instant availability of clinical information to authorized users at the point of care. Increased efficiency is one of the foremost benefits. Because of increasing technological advances, we are seeing the delivery and implementation model of EMR changing with the times.
Practitioners today have an option to choose either EMR on Client-Server or an ASP platform.
Traditionally, EMRs had been deployed directly in the clinic on a Client-Server platform using a main computer (a server) located in the clinic. The data is then accessed using a client i.e. another computer connected directly to the server through the local area network (LAN).
However, with the increased demand for cost-effective EMR solutions, and advances in Web based technology, many vendors are now offering EMR on ASP platform.
The Application Service Provider (ASP) model is a way of providing EMR to medical practices that takes advantage of the Internet. An ASP replaces the on-site server at a physician’s office with a server installed and managed at a central data center. Physicians and practice staff can access the software applications remotely via PCs connected through the Internet. It is an effective EMR solution for Solo and Medium Medical Practices.
The increase in demand and interest in the ASP model is attributed to the various advantages it offers over the traditional Client-Server model:
Low cost of ownership: The foremost advantage to the practice is the reduction of hardware, software and maintenance costs. Software installation, maintenance, upgrades, back-up etc. are carried at the vendors end. The clinic does not require hiring of specialized IT staff to manage the day to day activities related to EMR. The user has to pay a fixed or transaction-based fee, mostly on monthly basis which also reduces the cost.
Easier accessibility of data at multiple locations: The ASP based solution allows the physician to access patient data at multiple locations via PCs connected to the Internet. This is very useful for clinics having multiple locations: the physician does not have to stick to a particular PC or location to access patient data.
Easier Upgrades & maintenance: The EMR vendor can centrally update the existing software at his end rather than going to each site for upgrades. This results in fast and effortless upgrade without interfering with the day-to-day operations of the clinic. Upgrades to a Client-Server platform could also require upgrading the existing hardware. This adds to the cost further. With an ASP model, the EMR vendor upgrades the hardware as needed at his end rather than requiring the physician to purchase additional hardware.
Greater Flexibility: ASP platform is far more flexible than Client-Server systems. Most Client-Server EMRs require the practice to buy the application outright rather than choosing a step-by-step implementation. Once the clinic has chosen a Client Server platform, it is difficult to switch to a different vendor because the cost incurred during the initial implementation for hardware and software is high. As ASP based EMR platforms have enjoyed an increase in demand, vendors offering these solutions have to address certain issues in order to increase the acceptance of ASP based EMRs over Client-Server solutions:
Security & Privacy of Medical Records: Since all the medical data is transmitted over the Internet, patient privacy issues are vital. In fact, ASP systems can be more secure than most Client-Server systems. HIPAA Complaint ASP systems are professionally managed data centers with high level data security systems in place. Data moves through a secured and encrypted pathway across the internet with standards such as SSL-128 in use. Firewalls installed at both ends stop unauthorized access to the network. In addition, Digital Certificates and PINs are some of the other measures being implemented by vendors.
Ownership of Medical Data: Because the patient data is stored at a site outside of the clinic, prospective ASP clients often ask the question ‘Who owns the Data?’ In most cases Service Level Agreements or other legal agreements are formulated and put in place to ensure that the ASP users can retrieve their data upon the termination of services, if they chose to do so.
Availability of Internet Connection: Since ASP platform is web based, it requires a stable and fast Internet connection at the clients end. The physician might not be able to use the solution if the connectivity is slow or interrupted. Most vendors recommend T-1 or DSL lines, both readily available in most locations, to ensure seamless flow of information and uninterrupted workflow.
In summary, ASPs offer an effective alternative to traditional Client-Server applications with the promise of lower cost of ownership, greater flexibility, easier implementation and maintenance. Physicians have come to realize that ASPs have grown up, and more practices will begin to migrate to the ASP model in the future as the concept gains increased acceptance.

that Dell and several other companies were starting a new line with XP in a deal worked out with Microsoft because of all the problems with Vista. I want to say it was Santa Rose. Also, if you Google laptops with XP you get several sites to check out.

If I was looking on the internet at something and saw an address to respond to, I could click on the email address and it would pop open a box in my yahoo and put the address right in the compose a message part of my yahoo mail.
A couple of days ago, this feature stopped working, and now I am cutting and pasting the email addresses off of the internet and pasting them into my yahoo mail.
Does anyone know how to get this feature back.

But only from Andrews School or M-TEC. Many companies will waive the 2-years' experience requirement and will test and hire you if you go to either of those schools. Do NOT bother with At Home Professions, Penn Foster, US Career Institute, or ESPECIALLY Trans-Am, which is also known as Trans-SCAM and is a total ripoff. You will have a VERY difficult time getting hired without experience unless you choose Andrews School or M-TEC. Yes, they cost more, but that's because they are excellent training programs, not correspondence courses.

They were thinking of doing that or tried with a hospital I used to work for. Their rationale was that it was too much of a distraction. Could not believe that! MTs need the search assists on the internet. It just goes to show you how totally out-of-touch they are with job MTs perform.

Hospital Disabling Internet Access for MT's [2008-05-08]
I have a question and hope someone might be able to help me. I work at home as an MT. The company I currently work for does overflow work for a hospital; however, while on the hospitalc. It is my equipment, I pay the bills, yet Ic., but for them to dictate how and when I can use my own computer is ridiculous - like telling me I can drive my car, but canQA and are supposed to send blanks directly to the doctors - who just love to b**ch when you*I* am GOD!!! egos!!! Still, disabling a computer they own is one thing; disabling one YOU own is another. I think that's more of a power-trip on their part than anything that will truly protect them. If they connect with any other PC for any transmission of data, they can get a virus. Hobbling their MTs when it comes to research and communication via the Internet is going to prove counter-productive to them in the long run. They should invest in some professional grade antivirus software and consider joining the 21st Century like the rest of us.
I personally wouldn't work for such an employer. But you could: Ask them to provide you with THEIR computer to disable, if they feel that's so important. Check with your IP to see if you can simply set up a 2nd email account that you could toggle in and out of. Or, buy a laptop, have a different user-name & email address, and use that to stay connected with the outside world.

It is amazing how poor the sound quality of most dictation is. I experimented by listing to videos on the Internet. Those sounded like movie theater quality compared to the dictation.
I think the quality is just as good when plugged directly into the computer, but the volume capacity is higher if you use speakers. You don't even have to have good speakers when using them this way.

my daughter developed a horrible rash under her arms that started spreading over her sides, went to the doctor 3 times and got creams, antibiotics, and steroids, nothing helped. we finally discovered it was caused by aspartame (Splenda), and went away when she stopped using it in her coffee. researching on the internet, this appears to be a common reaction to aspartame.

Most MTs use the internet to keep updated on the latest pharmaceuticals. There are a gazillion sites out there that offer a lot more information that Quick Look drug books. They are relics of the past. Although you might find someone who wants one if you are lucky.

I don't usually post anything on the internet but I was hoping to maybe get some positive feedback.
I started as an MT in 1990. I worked for Kaiser in So CA for 15 years. I transcribed all work types and all specialties for hundreds of doctors.I took pride in my work and found it challenging on how to find ways to get the reportsdone faster. I had a very good job -$22.00per hour when I left - (they are union.) Ihad to leave because I had to have asubacromial decompression for impingement syndrome on my right and left shoulder. I still have flare-ups from pain. It was due to my job, and they could not honor my work restrictions when it was time to go back (I could only work 4 hurs a day)so I had to leave. That was okay with me - I had been commuting 100 miles round trip for years and I was burned out.I started a small transcription business of my own and had a few accounts - (I did them all myself.) I also have worked for a few transcription services throughout the years. I am not bragging by any means but I was always told that I did a very good job and I had very particular doctors.
About a year ago my husbandcontracting businesstook off and he told me I did not have to work anymore. That was fine with me. Only now, due to theeconomy, the contractingbusiness is down. Anyway, I have to go back to transcribing as we have no money. I mean NO money, and we owe a lot. I have never been in this situation in my life and I am very scared.
My point being - asyou all know when you stop transcribing hospital transcription for a time you lose a lot of your skillsI guess you would say. My confidence is way down. I have failed a few tests. I am 51 years old and I am scared to death. I have to find a job. Why can I am so down and I thought maybe some of you had been through this before. I live on a busy road AND they are building a 3-story building across the street. I just ordered a sleep mask (hope it works). Right now, I just blacken the room with black/out curtains (still not perfect, slivers of light still peek thru and I need black.
I hate ear plugs, uncomfortable and don't really work.
However, what I have found that does work is one of those sound maskers (I use waterfall sound). I also have a box fan running (if it's too cold I just face it somewhere it won't blow directly on me. These too things, along with the room-darkening curtain liners seem to do the trick pretty well. I'm hoping the mask will help some more.
I've also heard people had good luck with those air-whooshing-type sound machines. Right now I just have one of those little ones that play a variety of sounds...waterfall, rain, tropical forest, etc.

Depressing News followup [2008-11-13]
My PT job was at a hospital that used Meditech so I do have that training on that software, but I have worked via the Internet for years with a couple of diff hospitals so thank God I did have that experience in other software programs and knowledge of Internet.
Meditech is not a MT friendly program either but I did make good money so now I am going to have to find another PT job to help the gap. I sure am going to miss that account. I have been so lost since I lost my hourly paying hospital job and going back to working by the line for my total paycheck and I hate it. I really have thought about going back to school at age 58 and doing medical billing maybe. Hope it improves for all but I really do not think it will for a long long time.

I wanted financial help with going back to school for coding. The only training they offered was at a JVS school or a IDT type school as a medical assistant. Then the funds for training were cut. They offered (and pushed) this job assistance program which I thought was going to be more in depth than it is. I thought I would be sent on interviews they had arranged. Basically I meet with the counselor once a week and she gives me leads she found on the internet and I send in my resume. She supposedly knows the area and was going to provide local job leads. I lost one MT job in June due to VR and on my own found the job I have now in about a week. Yes I probably need another job in billing/coding so I can make more money, but now is not the time to be giving up a job you already have. IMHO
Ever call people you meet online? [2008-11-13]
I feel a little weird calling someone that I met online. Yet several people have left me their phone numbers to call them just to chat or some ask me to IM (instant message) them. Are they too trusting? Am I too mistrusting? It just feels weird to me. I am on my own. No kids or anything. Soon to be 45 years old. So I guess maybe that comes across in my e-mails that I am lonely and they are just trying to be nice. However, I would prefer just to leave a message on the message boards like this one or send an e-mail. I guess I want to know if others are exchanging phone numbers over the internet and IMing people they meet over the internet? The people I meet are usually people suffering from autoimmune disorders or people in chronic pain. They are usually responding to a message I left. I can't do anything about this condition so I try to focus on others andoffer whatever words of encouragement I can over the internet to those suffering.

I was (and still am) on a music board for a band and quite a few of us met first through emails, then phone calls and then concerts, and finally we had get togethers every year for about 4-5 years alternating at each other's houses. We had so much fun and I never would have met them if it wasn't for the internet.
We haven't seen each other in quite sometime, but we do correspond mostly by email now (phone once in a blue moon), but there are a few of them that live in the same state now that are very close and go to each others's weddings, baby showers, etc.

I've been thinking about a job change recently [2008-11-12]
I've only been in this biz for about 13 years, but now that my last one is off to school, I'm also thinking about a career change. I live in a very rural area as well, so I understand completely what you mean about people thinking $8 an hour is a good wage.
One thing I'd like to suggest is looking into a state job. We have 2 state prisons in my area (about a 20-minute commute) and both hire medical transcriptionists as well as medical records personnel. These jobs generally start out at about $12 to $15 an hour plus state benefits, which in my state are the best bennies anywhere. The jobs that require coding certification pay even more. I would highly recommend that you look on the internet for state civil service jobs in your state. You'd be surprised at the opportunities in the medical field.
I'm considering this option myself but want to wait until next fall, after I have the summer to spend with my kids (and so I don't start out in the hole because of babysitting expenses) and to have time to look into getting my coding cert. I graduated from a business school with an Associate's in medical secretarial and did a lot of coding prior to working from home. I used to do billing for a very large rehab facility, so I don't think it will be too hard to brush up on my skills.
Anyway, good luck and let me know if this (a state job) pans out for you. Most people have no idea these kinds of jobs are even out there (I didn't).

Why is it that podiatrists state MPJ (metacarpophalangeal joint) of the hands when they are dictating on the MTPJ (metatarsophalangeal joint)? I have podiatrists who dictate this and have seen it on the internet in articles. I have modified my abbreviation for MPJ for this reason so that I don't insert the wrong expansion. Just wondered if others have run across this.

I also have a home computer and all the same programs on a laptop to use when I travel. It's no different than my home computer. When I type at home, I am typing in my living room with my husband and son moving all around me. (We're remodeling right now.) They respect that what I am typing is no one's business and they have never even offered to look or ask what's going on. Furthermore, this is a family computer, not provided by work. They are on the computer quite alot when I'm not. The program in which I transcribe is password protected and the information on patients is not stored locally, but on a server 1000 miles from here. That's about as secure as you can get. Then, that is not to mention that I have a wireless network here at home, as well as when I travel. With a password and a good firewall, security is just not an issue. Also, see the post from Just Me a few down, about networks. She is correct. (I also have worked in IT for many years.) Hotels ARE secure networks with original passwords. The last one I worked in had to give me a new password daily. Working at McDonald's or an Internet Cafe is what wouldn't be safe.
As for the laptop... like I said for my main job, no patient information is stored on my hard drive, but at a server away from me. I have a local account also, and once I do the work and send it in, it is deleted. I personally don't see the concern. There's not any point when I'm traveling that I have patient data stored on the laptop. The only way anyone could get patient information off of it is if they bust in the door while I'm working and jerk it from me mid-note.

I had to change my hours. The main account I love working on seemed to dwindle. I had a heart-to-heart with my supervisor and asked her to look at the peak times for the account. So, I changed. I work Su-Thu evenings/nights. So, I also get some shift differential with it. But it also keeps me well stocked on my main account. I do get other accounts too and hate that part but it is all part of being an MT, in my opinion.
When I'm working, though, I am very serious and keep all distractions away. Work, get it done and then I can play, surf on the Internet, chat, go shopping, watch TV, whatever.
For my company, I do not save any [2008-11-05]
patient files on my laptop, so therefore no breech of security is possible and I transfer the files over the internet, just the same as with my PC.

ok.. cost for us is 89 for hubby phone as he is primary, and uses for internet also.. mine is 40.. internet,, text, circle of friends etc.. we have the Smart Phone. As far as footpedal and all this is how I do it.. we bought one of those converters that you plug into cigarette lighter and that is 110 power do I plug in laptop to that for AC power..I push the seat all the way back.. connect the footpedal and place it at my feet,, near the top of the carpet area.. put on the headphones and type away,, you should see the looks I get..LOL..

Sorry I cannot answer your question regarding internet speed but wondering if you can tell me what company is going to Chartnet. I was under the impression that SPI bought that program exclusively. I loved working on Chartnet for Cymed. Did not love working for SPI. Thanks in advance.
