New Doctor, some patients already typed [2008-12-01]
I got a new doctor. They girl who had been working for him left him in a big mess, undone dictations, etc. The officegave me a flashdrive to do with about 30 patients on it over the holiday weekend. Today they've told me ten patients had already been typed, how did I want to handle that? It seems they don't want to pay me for these. My husband says to bite the bullet and not charge them, since this is a new account and I'd like to keep it, is possible. What do you think? I know it's a land of confusion there from what is done to what is not done. Thanks!

the quality and production will improve, & turnover will go down. Doctors keep raising prices to US, so why are we so squeamish about raising our prices to THEM?

for our records to be handled outside of the US. I think the terrorism thing is everywhere; as another poster said, we have had them here. This issue is about the US taking money out of our hands, then expecting us to work for beans AND improve a failing economy? Impossible. Hopefully at some point the people who can do anything about outsourcing (government) will realize this and bring MT and many other industries back where they belong.

Doctor's hate to dictate. I can't tell you how many docs [2008-11-28]
have told me over the years how dictating is their least favorite part of their job. This is why so many of them suck at it. I used to the transcription supervisor at a teaching hospital and I used to have to train the new residents each year on the dictation system and I used to beg them to dictate slowly and clearly and I would bribe them with promises of food. I would stock the chart completion room where most of the dictated from with all kinds of snacks and treats.
Docs are so sweet when they are residents. Then they turn ugly as the years go by!

Good ventilation! [2008-11-23]
This is a good reason to make sure your office -- and home! -- is well-ventilated. Keep your tower as far away as possible and downwind. As plastics heat up, they do outgas. Flat-screen monitors are supposed to outgas less, so that's a help.
That awful smell from photocopiers and laser printers isn't exactly safe, either. You should use those only in well-ventilated areas.
Let's not blame everything on computers, though. I'd bet that many here have homes that have toxic soup for air.
One allergy group I know has taken to sending a staff member to the home of pediatric asthma patients when the usual control methods fail. They nearly always find the homes tightly closed up, always running either heat or air-conditioning with no ventilation. The homes are often very clean, too, with moms using lots of cleaning products, powdered carpet deodorizer, toilet bowl deodorizers, fabric softener, scented detergents, bathroom sprays, potpourri, fragrance candles, and insect sprays. In most homes, the big offenders were those plug-in air-fresheners and scented oil diffusers. Some homes had carpet that were shampooed constantly with non-professional units, so that they were soaked with dried detergent and growing mold underneath.
The moms felt that they were doing good to clean everything to keep away the dirt that was causing their child's asthma and they didn't understand how something that smelled nice would make their children sick. They replaced the carpet in the kid's bedroom with linoleum and thought that was enough.
Very few were willing to stop smoking or even to go outside to smoke. They were in utter denial that their behavior was harming their children, and even if they admitted it, they didn't care to stop even when offered free medication. Nearly all of them had ashtrays and cigarette butts in the house, even though they told the doctor they didn't smoke there. Most of them burned air-freshener candles thinking THAT was going to help, when all it did was add MORE toxic chemicals to the air and make their children sicker.

The only other way possible I can think of, and someone above already mentioned it, is have another Transcriptionist train you. That's how I got into this field. However, most transcriptionists aren't in the situation to train to nor do they probably want to. If you do choose to do online courses, I would personally recommend CareerStep... just stay away from PCDI! I have two friends going through CareerStep and they both are enjoying the course.
Otherwise, this isn't a job you can just walk up to and start without any knowledge. If you want an easy job like that where you need no training, or training on the job, try a gas station or fast food restaurant. Sorry, I'm not trying to sound harsh, just truthful. You need a lot of training before working as an MT, as another poster said, it really is like a second language! You have to learn terminology, pharmaceuticals, anatomy, etc. However, I wish you the best of luck. If you *really* want to become an MT, then you can do it!

What is the best website to buy Instant Text from? I have been considering getting it for months. I have been using AutoCorrect in Word this whole time, and I think I'm ready to graduate to Instant Text. It's worth it right? I was there once. Where I lived at the time was a small town. Those commercials make it seem like it is so easy to get work as an MT. Not 15 years ago in a town with 1 small hospital and maybe 6 docs it wasn't.

She does have to be on a high-fiber diet. They think this might be related to H-pylori, even though she tested negative for it. I told her lets not worry until we have to, easier said than done, but she is a worrier, and yes, I have inherited that from her as well:) I was reading up on this MALT lymphoma and if that's what she has, it looks like it is pretty treatable if caught in the early stages, so we will pray. Good or bad MT [2008-11-08]
Can someone tell me how I to know if I'm a good MT. I have tried looking for some free online tests, but I can't find anything. I'm constantly worrying about losing my job because I don't know if I'm any good...I just recently got put on a new account and I'm always having to blank now. I am wondering if this is just how the MT world is or if I suck at what I do....please HELP!

I liked working there, but no time off allowed. One day an MT co-worker of mine got a headache and got in trouble for calling out that day even though she gave plenty of notice early in the morning and they had all day to distribute the work. This is when often-times there was no work at all. I left because after hearing that, I realized I would not be a true IC. Why, as an IC you are supposed to make your own schedule aren't you? And if something comes up isn't it true that you are allowed flexibility? They say it is a small company and everyone is expected to be there for them when the hospital decides to send in the overflow, yet with no notice to the IC. It was too confusing, in may ways worse than employee when at least you know when you are expected to work. Not an IC job at all. Sorry for your experience, but I and many others I know have had the same experience.

I've seen some MTs on here that say inhouse in their area make less than $10 an hour. Our local starts newbies (providing they pass the tests) at $16.79 an hour.
Does anyone have good text expanders for vital signs data? [2008-11-05]
Do you any of you use text Expanders for the numerical portion of vital signs for example, 130/80 ? I have lots of expanders but none for the actual numerical data. It really slows me down either having to look at my number row or switch to the keypad. I do have some, but nothing consistent. Anything ideas would be great, thanks!!

As MTs, it's not our problem so why worry? As long as we do our 8 hours, why worry about what the doctor is or is not doing. I just wish that the powers that be remind them not to be eating while dictating and also be sure they are talking into the microphone and not off in space somewhere.

I do this and it helps with the volume.
It is amazing how poor the sound quality of most dictation is. I experimented by listing to videos on the Internet. Those sounded like movie theater quality compared to the dictation.
I think the quality is just as good when plugged directly into the computer, but the volume capacity is higher if you use speakers. You don't even have to have good speakers when using them this way.
A couple of things come to mind.. [2008-12-03]
Years ago while doing my OB rotation as a nursing student I developed an axillary rash after shaving. It got really ugly and spread, and my theory was that it was a staph infection I must have gotten from a baby in the newborn nursery. I figured I was holding or feeding one of the babies and s/he was breathing bacteria from his/her nasopharynx into my irritated armpit, where it was nice and warm and moist. I treated it with OTC triple antibiotic ointment and it went away quickly.
The other experience with underarm rash was my husband's. He came to me with a flaming red bilateral rash extending down his sides and asked if I knew what it was. It took me about 30 seconds to figure out he was having a reaction to his deodorant.
A fungal infection is probably a good guess, so if you want to self-treat you could try clotrimazole like the other poster suggested (I would use cream). You should see an improvement pretty quickly if you're on target.

my daughter developed a horrible rash under her arms that started spreading over her sides, went to the doctor 3 times and got creams, antibiotics, and steroids, nothing helped. we finally discovered it was caused by aspartame (Splenda), and went away when she stopped using it in her coffee. researching on the internet, this appears to be a common reaction to aspartame.

Okay, last Thanksgiving weekend, my new doctor gave me a flashdrive and I did all the patients, and then she told me nine of them had been done, how did I want to handle that? You all were very kind in giving advice. This was after a girl walked away and left them not knowing up or down on who is done and who is not.
Today, she called me and asked me questions about dates and patients. She said she is busy and doesn't really have time to see what is actually on the flashdrives. We're meeting in the morning and she is going to give me another one (or two or three). Should I volunteer to listen and make a patient list, or should I tell her I will type all the patients and expect to be paid for them. I don't want to spend my time listening and not getting paid, honestly. But then I want to try and hold onto this job, having just got laid off from an office with more than one physician (thanks to Cbay). You guys are experts and thank you.

I can type 180 lph hour doing straight transcription and I can edit at an average of 600 lines per hour when I edit reports via escription. I make a lot more money editing than I ever would doing straight trans. This is also from 5 different accounts (big accounts) with 2 different employers. My average is relatively the same with each account. It really depends on the person though. The potential is there but it isn't going to work our for everyone. Good luck!

especially if you did not charge the first time for the duplicate reports as you have been down this road before. Definitely do not WASTE your time by listening through first and making a patient list. You might suggest that the doctor separate his reports out in teh future as he dictates and then the office staff can make a list and give to you with the flashdrives. That way, if there is a patient on the list and you do not get you, you can let them know.

very sorry, I should have looked it up before I wrote that. but it WAS splenda she was allergic to and apparently a lot of other people have reactions to it. I'm not knocking Splenda, just thought I'd mention it in case the OP had recently started using it. I thought my daughter'sexperience might help someone else, since she went to the doctor 3 times and we ended up figuring it out on our own.

Dictation speed question. sm [2008-12-02]
As a rule (obviously, it varies based on typist and doctor), but 30 minutes of dictation would take 90 minutes based on the 1:3 rule, right?
If dictation is 30 minutes but took 3 plus-hours to transcribe it, this dictation was done in double speed, right? Looking for some sort of chart online that shows this. It was a .dss file.

for the docs who are horrible dictators. I do progress notes for an ICN nursery almost nightly. Thank GOD for those nurses. There are 2 docs right from the old country who can't string a sentence together in English though good docs they may be. We're talking 7-8 page very detailed reports on some of these babies. The other night, one of these gals dictated the first part, God love her, and actually handed the phone to the doc to give his 2 cents worth, awful dictator. Hey, the best thing some of these docs can do is let their PAC's do the dictating!!!

It is one of the most user-friendly platforms I have worked on. I have 2 accounts that the speech rec is very good on and 1 account with lousy dictators that require more editing, but it is often entertaining. The sound quality is very good, and there is little to no distortion when you speed up the audio file.

I had posted this q. on the Gab Board before, but got no response, maybe because I worded the subj line wrong or maybe it was just the wrong board. Has anyone had an experience with an underarm rash that may be a form of tinea? This started w/me having an irritation 3 months ago from wearing an ill-fittingbra in hot, humid weather. Since then it has gone from red to a brownish rash that is spreading to areas under my arms that was not originally irritated. Any OTC treatment I could try before I make an embarrassed visit to my doctor? TIA!

Sometimes the less said, the better, but of course, only you know what you did and I don't think you should broadcast any personal info on here or the Web. You will have to decide for yourself, if you have the skills, maybe they will give you a chance; maybe they won't think it matters. Only you can decide. I can't give you advice and doubt anyone else on here would either. We're used to everything being confidential, etc., professional people here, at least most of us anyway! Good luck!

stay in this business is if the tax breaks are ended and the loopholes closed for companies that outsource what used to be good American jobs. Ending offshoring entirely would be nice, but I doubt it'll happen. But it should cost less for companies to hire American workers at a decent, livable wage, than it does to send it offshore. Offshoring should be something they do in a pinch, not as a standard practice. Yeah, so what if they're making more money the way they're doing it now. Doesn't make it right. They're greedy and they're un-American when they put thousands of hard-working American MT's in the poorhouse, or out of a job.

I am a SE and I rearranged my schedule and worked Thursday through Sunday. As a matter of fact, so tired of working through the holiday I am resting today. Not all US employees are like that. Maybe you need to interview people differently so that you can find people who will stick by you. I am not saying I would want to work every holiday and holiday weekend, but if it meant bread and butter on the table for my children, my family and I make sure to support the decision to do it. Please don't close your doors to US MTs. There are some of us out there, although weary, who appreciate a good job.

Want to be able to offer clients a platform like Bayscribe or Vianeta, etc. However, I am looking for one with low cost, I love Bayscribe, but if you are low volume, which we would be starting out, their per line rate is very high. Does anybody know of a good platform with a low per line rate and no or very low startup cost?

that disorganization does not always come from the old MT that you've replaced. You can type for free all you want. That's your business. If this person wasn't smart enough to know that there were dictations already transcribed and just handed it over to you on a holiday weekend to sort out the mess, and now doesn't want to pay, that's a bad first impression of the person handling the work that is supposed to have a handle on things. I guess she should have checked, but then again maybe she was in a rush to get to her holiday and not worry about it.
I'm sure they'll pass the word all around that Lisa works for free; try her out! What would it cost for them in all honesty to just pay you for the trouble since it was their mistake and not yours? Honestly? But, hey, if you like working for free, that's your business.

You are all assuming the old MT was the bad one, when in fact it is probably the office; I think it's horrible to have this girl type on a holiday weekend and then come back with well we don't want to pay you know. Horrible!
No, I'm not a doormat. It is actually MTs that type for free that give us all a bad name! Gee, I wish I could afford to type for free, but I can't, so I won't.
Do you think a patient goes into thisphysician's office and says I've heard this all before from another doctor, so I'm not going to pay you? Doubt it!
