Hi,
I'm looking for some advice and direction. I am looking for possible options for a digitaldictation/transcription system for a multioffice practice on a Citrix network environment with remote transcriptionists. Several years ago I worked for a web based transcription service that used Chartnet, but that was several years ago.I don't 'really remember any big issues, but I don't need the payroll, production aspect ofa system. I am lookingmore for tracking, in having control of knowing incoming and outgoing jobs with a system. Our practice is integrating with EMR; however, several of our physicians continue to choose to dictate and I am looking to other transcriptionists to possibly point me in the direction of software that they have used so I can research and try to find a product that will fit our practice - we are still on micro tapes.I am looking for a system that of course would be Transcriptionist friendly, but also afford me the ability to manipulate the jobs, i.e. assigning easily, producing reports and software that has good tracking abilities and of utmost importance,excellent sound quality for the player.As a transcriptionist, Ipersonally would look for a system thatI will not spenda lot of time in the download function and the complete function. It isn't real effective to me ifthe timeuploadinging and downloading per job takes longer thanactually transcribing the jobs. We have a large quantity of work.Key to me is having control of my work, meaning the physicians being able to identify dictation with a patient number and my being able to track jobs downloaded. Forgive my rambling!! I would appreciate anyone responding to me with any suggestions or telling me of your experience with software.

I also constantly worked in Meditech creating macros, which we weren't given permission until we begged after waiting so long for that company to put in some canned text AND I NEVER GOT A RAISE. It's all part of your duties. Get over it already!

Most likely they'll laugh at you for asking for a raise to do these other duties as required. Look at your job description and see what it says in the fine print.

PT position requires Lanier dictaphone but I have VDI 3006 Digital SuperStation [2008-07-28]
Can I use a VDI 3006 Digital SuperStation when the job calls for Lanier dictaphone? I read a post that said Super Station could be used in place of Lanier but not the other way around.
And, if not...Do you think I should purchase a Lanier Dictaphone in order to get a PT job which sounds promising? It would be a fast return on my investment and I could use it in the future.
CHEAP recorder, awesome sound! [2008-07-22]
My doctor uses an Olympus ws110, which he paid less than $70.00 for. He dictates, it comes apart and has a USB connector inside. It plugs into his computer, he downloads the dictations, and he e-mails them to me. I type them, e-mail them back. You can get digital signatures in e-mail for around $20 each (one for you, one for her) and no one can open the e-mails but you two. I love the whole set up! Best of luck to you.
Computer/digital phone system question - SM [2008-07-22]
I currently use a Philips digital transcriber system, which I call the number and get my recordings through the phone line. I would like to get a laptop to start doing my work and was wondering if there was some way I could move the recordings to the laptop and just work strictly with the laptop and foot pedal.
If anyone has any information or knows if this can be done, I would appreciate it.

I need some advice on digital dictation systems [2008-07-21]
I am talking to a psychotherapist about possibly doing her transcription. She does not dictate presently, but is interested in getting started. I am thinking of asking her to go digital, but I don't know the first thing about getting started, what equipment we'll need, etc. I currently work for a national, so they sent me the PC and footpedal, and that was it. Any advice would be so appreciated. I don't wanna sound like a dummy when we talk...thanks y'all!
Digital handhelds easiest, or a cheap [2008-07-21]
You can buy digital handheld recorders at decent prices. If dr. is computer literate, dr. or staff connect to PC and send you files from recorder either through encrypted e-mail or secure FTP site (you'll probably have to set up). Or you could look for a cheap call-in system. Dr. calls it, dictates, and you can transcribe using Express Scribe or another player. A lot of docs like those 'cause they are used to them. You may need a new pedal if yours only works with your current co's platform. Check ebay first or classifieds here even, maybe you can get deal on used equipment to see what dr. prefers.
please, need help with startstop opening windows sound recorder. [2008-07-18]
When I open a file from within startstop, it wants to open windows sound recorder, and then it wants to open a file located somewhere else on my hard drive.
Sometimes it will open the file I want to listen to, and the windows sound recorderflashes on the screen a couple of times.
Is anyone having that problem? I've emailed and called startstop but no response to email and no return call from startstop tech support.
Any info you have is greatly appreciated. I'm on version 9.8.7

I am always startled at how much tallying line count every hour or two keeps me focused. Some days just spread out vaguely and my line count suffers as I get distracted or restless.
The info your platform provides should be enough ... to keep some sort of a running tally.
I trust almost no one is still doing manual logs (best kept in Excell, imho), but yes, even with manual logs, a bad hour total can provide the seat-in-chair motivation to perform better next hour.
Taking breaks is a good idea -- but I admit I still spend a lot of time playing catch-up.

I have used Express Scribe for years but ever since I restored my computer it will not play .dss files any more. I have downloaded every codec out there, etc. I have Gearplayer also but cannot stand it. What does everyone else use? Thanks

Converting voice files to digital files [2008-07-08]
I have a Voice Solutions dictation system that will happily convert DS4000 voice files to digital when they are downloaded (wave files). However, it will not recognize any other files for conversion. I had a physician download .WAM files from his DS-30 and my dictation system would not convert it. Anyone have any suggestions? I would greatly appreciate any input.

Her doc wants her to transcribe and he records on an Olympus digital voice recorder. When I look up the specific model she said he has, it's this http://www.aaaprice.com/olympusds330.html. My questions are this? What does she need to be able to do this work? She has a .wav pedal. I don't do this type of stuff working with these particular types of voice files from a single recorder so I'm just trying to help here. Right now he's given her JUST his little recorder to transcribe from until his software comes in (had to reorder it). He thinks she should be able to load the software onto HER computer and be good to go. Is that so? So right now she either awaits the software or has to sit and transcribe from his recorder w/o any headphones or foot pedals (which will be painstaking, LOL). Am I right? Any thoughts are appreciated.
Digital dictation setup to use with express scribe (sm) [2008-07-05]
I can't find the post that tackled this subject a month ago. I'm looking to get one of my clients set up on digital dictation. I have on my end express scribe, the headphones and the foot pedal.
I'm either going to set up an upload function on my webspace or have the client email me the voice files.
Can someone recommend an inexpensive recorder that I can purchase for him (I can't afford the $200-$300 recorders) that can have the files uploaded to his PC so they can be sent to me and I can transcribe them on express scribe?Thanks in advance!!

He bought an Olympus WS110 digital recorder that just plugs into his computer to download the dictation. You can use digital signatures, send everything back and forth using e-mail without having to have an FTP site. You can also password protect documents in Word. His digital handheld was less than $75.00 and the sound quality is awesome. I use GearPlayer software with a computer foot pedal to play his files, type them in Word. I hope this helps.

Can someone point me in the directionto find information on purchasing equipment for a doctor to try digital dictation for the first time? He's currently using tapes and is interested in digital dictation.
I have express scribe and a wav pedal, I realize I'll need to get FTP set up on my webspace but I'll need to purchase him the handheld, etc. and I'm really not sure what the best setup is. Being only one person, I can't afford thousands of $ but would like to be able to offer that type of service.

I've always used the metal Lanier type headphones and they've been great, but I just tried a pair of Sony digital reference headphones for listening to music. They are a little heavy on your head, but the sound quality is wonderful.. you hear things you couldn't before. Having the speakers on just seems to muffle voice for me and I tend not to hear as well. So, next to the Sonys, I would use the metal earphones.Hope this helps.

First of all, I have worked for several nationals in the past few years, and equipment has always been provided. I currently have a f/t position with a national that I use my own personal computer for, they just put their software on it. I had what I needed already, as far as foot pedal, Expander program, etc.
I have just been sort of rolling ideas around in my head about possibly starting up something small perhaps on a p.r.n. basis just to bring in a little extra income. As I am still just mulling this around in my mind, my questions are: How do I go about getting started? Do I try to purchase equipment first, or start out using tapes? I am worried about spending the money and not having any takers, so I am thinking, I should send out some sort of letters first; but, without having the equipment already, how do I tell them I plan on doing their transcription. Maybe mention tapes first, and tell them I have plans to go digital after I get up and running? These are all just things I wonder about. If anyone has any ideas, please feel free to email me with any advice.
I have been an MT for 16+ years, but as I said, I have always had all of the stuff provided for me and just don't know how to start.

I have my biz cards ready to go. I have my letter partially ready to go, with sticky notes that I’ve designed as a promotional giveaway to insert into my packets. I need some advice/suggestions for startup. If I get ‘that’ call and the customer wants digital rather than tapes, who pays for the handhelds? Do you all use an FTP site for return of documents, or some other way? What wav player do you suggest or use that works for you? I’ve tried GearPlayer, the trial download, which is very nice. I have had a lot of trouble with Express Scribe, so that is out. The audio skips and is just overall very bad. I have never had good luck with Express Scribe.

What are you using for a computer? I think in the past you said a laptop but I am not sure. Also, if it is a laptop, did you have to add extra memory, etc., to load all the software programs you need? I am very interested in what works as I have such back problems I had to let a lot of business go and am sorry with all this economy stuff, had I had your brain and done it your way, it would have been worth the investment. I just worry some won't accept a laptop, etc. Appreciate your comments, you are truly a helping person, that was the reason I chose this profession but not too many want to part with help. Kudos to you.

now because you need ULD and no phone company offers true ULD. I worked with a company where I had to use a C-phone and I had digital phone service through my cable company. I no longer work for the company, but I used it without any problems whatsoever.
question for laptop users sm [2008-11-03]
Some of you say you can work from anywhere, etc., have heard of people using them in your car. It seems to me there are very strict rules about hours you absolutely have to work, so how can you manage this and travel as well? I would love to find some work on laptop as my back is killing me, but I don't think I could be on as rigid a schedule as some require. I would love to work when I can from a laptop, is that a possibility? Are there any flex time positions, which is a silly question in this economy - seems like there are almost no positions left at all! Love to hear from you. Hate to give up, also it seems you can't have as much memory on a laptop as a desktop. Perhaps I'm looking in the wrong places and not savvy enough as to equipment and time schedule requirements.
mangled tape [2008-10-28]
I was able to take the cassette apart to get the tape inside unmangled, but now I'm trying to figure out how to roll it back up as tight as I can so it doesn't happen again, and I don't know how to do that. I have a few feet of tape hanging out and I can't roll it tight by hand unless I want to spend days on it, and it still may night be tight enough around the spool. The tape is intact so I don't think I need any splicing. I'm think right now this is just ridiculous that I'm having to do this and yes, the doctor is technotarded. He kept erasing his dictation accidentally on the digital recorder so went back to using cassette tapes.

If you have a business machine company in your area who sells the transcribers they may, out of the goodness of their hearts, splice the tape and fix it for you. As said, you will definitely lose a bit of dictation but you won't have to lose the whole tape. I had this done before. I have not spliced a tape myself but if not an office machine supply store, perhaps there is some other kind of business that would take this on and you could pay them a few bucks. I've been there, done that and found an angel somewhere. I know people splice but don't know how they do it. It's a big, big problem and I understand how you feel. Good luck with it, let us kow. You will get a lot of flack for not using digital but I know some docs absolutely will not switch.

Or they hold the recorder right on their mouth, or 10 feet away from them so it is too loud or too quite. I totally agree about the meds and labs though, I have dictators who do that exact thing.

this is a post I just read:
My Microsoft Digital Media Pro keyboard is quite nice. The keys are all attached to the front of the keyboard. Just unscrew the back and the front lifts up with all the keys. On the bottom part of the keyboard remains the rubber/silicon and circuit board, which can be sprayed off. I then submerge the top keyboard in scalding hot soapy water to clean it.
fwiw: I use plain 0 Vicryl and #1 Vicryl and 2-0 Vicryl [2008-10-16]
I don't recall how I settled on that but I've never had QA challenge it (or not within memory) ..

The hospital only offered a flexplan, which if you don't use the $$ by the end of the year, you lose it. With an HSA, the funds rolls over year to year.

I've been in this business for 20 years. QA? Never heard of it! We took it straight to the dictator. These were the old days of working on-site. People didn't have computers.
Now with all the digital components and can work from anywhere on your PC, there needs to be QA before the document can go the client so the company looks good. We usually had each other when we were on-site to help with a s/l, and if we couldn't figure it out, then it went to the dictating physician. Never had a problem.
QA is highly over-rated in my opinion. You can probably thank AHDI for that fiasco. Who else should know the answer but the dictator????? JMO.

Packages are smaller, some things cost twice as much as they used to and the higher-priced supermarket chainsand pharmacies have only a few cars in the parking lot. New England is feeling the crunch and we are facing winter heating costs. I usually use Comstock cherry pie filling, just barely covered the bottom of the pan, I noticed the difference from the norm, then read the label, which said use two cans. Geez!Making more things from scratch, getting back to basics, forgetting about diets yet careful about fats and sugars. If things continue, we won't need to go on a diet, we will HAVE to cut corners. My DIL was complaining of this smaller packaging also this weekend. We went out for a special occasion this weekend --family style chicken dinner, and the restaurant purposely served the salad after they brought out the meal andIt was a small bowl for 11 people. We didn't say anything as we had 5 kids who don't eat salad anyway.The usual dessert which goes with the family style meal was eliminated. Everyone is hurting and holidays are coming. Doing my own baking, no more canned cherries!
"Listening ahead" doe NOT mean "stopping." [2008-10-11]
I'm not sure what you mean by listening ahead, but if it's the listening ahead mentioned elsewhere here as one of my recommendations, it does not mean you stop dead and listen, then rewind and type. It's not even listen-then-stop-and-type. That would be a waste of time, as well as unnecessary.
It means that you listen to the upcoming dictation WHILE YOU TYPE what you just heard. You listen far enough ahead (about half a sentence) that you can understand the context and can catch changes.
It is much more efficient to transcribe it correctly the first time. By listening ahead, you avoid typing the nonsensical material that typically results from listening and typing one word at a time.
Older MTs working on typewriters with carbon packs with old Lanier transcribers that had no auto-backspace were forced to avoid any errors at all due to the difficulty in changing anything. Literally, the entire page would need to be retyped for anything more than a small error, and you did not want to be unrolling the carbon pack in order to scrape the errors off every page. (You had to use a scalpel or a very sharp knife.) You also had to listen far enough ahead to stop in a pause in the dictation, because every time you stopped, a few words would drop out between where you started to stop and where the tape finally got going again.
When computers and digital transcribers came along, it was so easy to correct errors and so much easier to transcribe without those words falling out that MTs began learning to transcribe in an inefficient manner. Now, it's very common and it looks as though people are teaching it that way.
I am not old enough to have learned on typewriters, but I did have the unhappy experience of having to use a ancient Lanier transcriber for a while. The light dawned on me when an employer installed an electronic document system that had no medical spellchecker, would not allow use of my productivity software, and which took an ridiculous amount of time to make even the smallest correction. It cut everyone's line counts in half. The facility did not adjust our productivity one whit to compensate. In self-defense, I decided to try to learn to listen way far ahead as I'd known some really ancient MTs to do and to type so slowly that I made NO errors and thus had to make no corrections.
Within days, I had not only returned to my former productivity, but had increased it by 200-400 lines per hour . . . without an expander. I was typing at a leisurely, relaxed pace, too.
A lot of MTs have tried this and most report that it helps them.
Suggestion [2008-10-11]
It's not impossible to learn. I did it and we've had students and graduates learn it successfully.
Try it when you are not working, so there won't be pressure to produce. If you take it in stages, it's probably easier and you'll discover the technique.
You have to learn to hold about half to a whole sentence in your mind while you type it AND then listen ahead at the same time. Really, it's not impossible.
First, try just learning to remember entire sentences. Listen to a full half of a sentence, then stop and type it from memory. Increase that until you can do whole sentences. You might have to work on that an hour or so for a few days.
Once you can do that, I think you will begin to see when you can start listening ahead for the next sentence. It is usually about 2/3 or 3/4 of the way through that first sentence.
The key to getting this right is to do it with the expectation that YOU CANNOT MAKE EVEN ONE ERROR because you CANNOT FIX THEM. Obviously, we know you can fix them on a computer, but pretend that you cannot.
This does require a lot of concentration and focus. You'll be holding one sentence in your head while typing it AND listening ahead for the next.
Some people can't do it, I think, but if you can, it's worth the effort to learn it. Email me and let me know how it's going for you.

Headphones? [2008-10-07]
I have to order a new USB pedal, so I was looking at the headphones. Anyone have any headphones they just love. I do not want to spend a lot, so I was looking either at the Spectra headset USB by Infinity that has the true volume digital control and am wondering if that is really not needed and might be over doing it, orI was also looking at just the standard Spectra headset with a plug. Any opinions greatly welcomed!!
