As a rule (obviously, it varies based on typist and doctor), but 30 minutes of dictation would take 90 minutes based on the 1:3 rule, right? If dictation is 30 minutes but took 3 plus-hours to transcribe it, this dictation was done in double speed, right? Looking for some sort of chart online that shows this. It was a .dss file. Any help is appreciated.

You have the wrong employees. I NEVER TAKE THE HOLIDAYS OFF unless there is no work for me to do. Don't tell me there aren't people in the US who have no work ethic because you are looking in the wrong places. I know lots of people who have a better work ethic than you apparently. Poor baby had to do four 18-hour shifts...if you OWN your own company and you only have 48 employees, this can happen when you um, like give them all the day off. Sounds like to me you have IC workers, not employees. Most employees I know work when employer says to work. If you need people to work holidays, why did you give them all the day off?

Do you pay a decent wage? On time? Do you value your employees? If you treat them right they would help you out when you need it. I too worked over the holidays. There wasn't much work but I was available. You have 48 employees and no one would work? Who's in charge? Get rid of them. There are plenty out there ready, willing and able.

cheap - and believe me, it gets worse. It will cost you about $400/yr. Why should YOU pay to work for someone else? I have worked for many MTSOs and only found one that require I pay my own insurance. It is NOT worth the write off and you lose money in the long run.

Acute care dictation [2008-10-18]
If all you do is office dictation, then you might lose it to an EHR. Hospital dictation, though, is much more difficult to automate. No field remains stagnant. You have to constantly upgrade your abilities to keep up and stay employed. If you can't do acute care dictation, why not consider upgrading to it? It will be around a lot longer and can enable you to switch into coding, documentation improvement and review, and other developing areas in health care.

to BOS3, hyphens are now not used with numbers--i.e., 2 cm mass, not 2-cm mass, etc. Also, they are taking the periods away from titles, i.e., Dr not Dr., Mr not Mr. It will be a fun time for all.

Searching for digital dictation [2008-10-08]
Hi, I'm looking for some advice and direction. I am looking for possible options for a digitaldictation/transcription system for a multioffice practice on a Citrix network environment with remote transcriptionists. Several years ago I worked for a web based transcription service that used Chartnet, but that was several years ago.I don't 'really remember any big issues, but I don't need the payroll, production aspect ofa system. I am lookingmore for tracking, in having control of knowing incoming and outgoing jobs with a system. Our practice is integrating with EMR; however, several of our physicians continue to choose to dictate and I am looking to other transcriptionists to possibly point me in the direction of software that they have used so I can research and try to find a product that will fit our practice - we are still on micro tapes.I am looking for a system that of course would be Transcriptionist friendly, but also afford me the ability to manipulate the jobs, i.e. assigning easily, producing reports and software that has good tracking abilities and of utmost importance,excellent sound quality for the player.As a transcriptionist, Ipersonally would look for a system thatI will not spenda lot of time in the download function and the complete function. It isn't real effective to me ifthe timeuploadinging and downloading per job takes longer thanactually transcribing the jobs. We have a large quantity of work.Key to me is having control of my work, meaning the physicians being able to identify dictation with a patient number and my being able to track jobs downloaded. Forgive my rambling!! I would appreciate anyone responding to me with any suggestions or telling me of your experience with software. Thank you in advance - I know that the first place I needed to go with this transcription question was to the experts - folks that use these systems and are familiar.

Again,i've used these programs and checked them and they are virus free. To confirm this, try your best antivirus and scan all files. I use Avira latest virus scan and it detected none. For those of us who can't afford to buy the original stedmans, this is a good alternative. If you have the money, buy the software! If you have them and want to share, share them by all means!

So, naturally, in whose favor would we expect it to be? Right!! 2 things I would get absolutely straight on a contract, though, is hours or schedule, if any is to be considered, and a definitiveline rate, not in vague terms.

but after working inhouse I know they the med recs departments know of the problem and do try to talk to the doctors about it. most doctors just think they are above some things. I also know that sometimes the doctors and med recs departments don't set the TAT, the CEO or board of directors do based on JHACO and other audits. They also do build in some requirements for physician's to get things done in a timely manner and physician do get cited but (at least where I worked) it meant little to nothing. The physicians had no problem taking the slap on the hand for being late on dictation.

I do this and it helps with the volume. It is amazing how poor the sound quality of most dictation is. I experimented by listing to videos on the Internet. Those sounded like movie theater quality compared to the dictation. I think the quality is just as good when plugged directly into the computer, but the volume capacity is higher if you use speakers. You don't even have to have good speakers when using them this way.

and listening to dictation, yes, it should be paid and she as an IC should set the amount she should be paid. I pick-up and deliver, and that's in my line charge. I also do that at my convenience, not theirs. A real office is fine with that and understands the meaning of IC. Lisa is a vendor like any other contractor. If I had to sit and listen and sort patient lists, etc. and spend more than 5-10 minutes tops at an office, then I would charge for that time spent at that office sorting through their mess. Let's face it. The office is a mess and Lisa is their scape goat. Since now they know she's bending over backwards, they will take advantage. Again, Lisa should not spend her time sorting out their mess. Furthermore, if they can't figure out what to give her and don't have time, then Lisa's response should be that she doesn't have the time either unless she charges them for that time. It just sounds like a real mess of a deal. I can't wait to hear if they actually pay her at all! Okay, I'll shut up now! Just the fact that she's posting again is hysterical. I guess I'll keep my mouth shut after saying from the last thread regarding this I told ya'' so. You all thought oh just this one time. Why do you think the other girl quit? Hello? and the office staff are extremely disorganized. Other than making a list of the patients you transcribed, I would not get involved in any pre-transcription listmaking. That's the office's job. What should be happening is that they give you 1 jump drive for every 1 or 2 days' dictation ALONG WITH PATIENT LISTS FOR THOSE TWO DAYS. They have patient schedules in the office or a sign-in sheet, I am sure. They can at least provide you with those. As far as duplicates, state very firmly that due to the state of the records prior to you doing their transcription, they are probably going to have some duplicates and that you expect to be paid for everything you type. This is simply a cost of doing business the way they have been doing it in the past (or associating themselves with) and they are going to have to eat that cost. It's not your problem. If they give you any trouble with any of this, I would drop the account immediately and let them find a sucker to do free typing for them.

Can someone please tell me how I get the little box to open that allows me to speed up or slow down the dictation? Sometimes it opens automatically and sometimes it does not. How do I make it open when it does not appear automatically?

We had Thursday and Friday as holidays, but since I knew there was a backlog and that it would truly be appreciated by my bosses, I dug in from time to time and we got the backlog caught up for today. I truly treasure my bosses and love my job, and all the co-workers I talk to feel the same way. We really do appricate and value a great MTSO and we prove it!!

Personally, I don't mind doing ESL doctors or more difficult docs. I can now run through them just like any other. However, others in my company are not the same way. On many occasions I have been requested by physicians (of course this burns the other MTs) to do their dictation. With that I have many recommendations and have been promoted for that reason. I think it really depends on the company and the people.

With DD, your bank statement is proof that the company paid you or not. With checks or prepaid credit cards, if it's lost in the mail YOU have no proof it wasn't sent. If it gets screwed up in adding it to the card, YOU have no proof the MTSO is at fault and you have to fight with Visa. The MTSO is correct in refusing to tell you which other MTs get Visa--they have a right to privacy. The main problem I can see with Visa is that it's just inappropriate. You'll be forced to spend every penny you earn AND spend it at an establishment that takes Visa. How do you plan on saving? How do you plan to pay your insurance and other expenses? What if your children need $10 for school? You also may be subjected to fees and/or expiration of the funds on the card. Why don't you talk to your bank about the safety of direct deposit? Perhaps if they can calm any unfounded fears you have, you can be paid in a more appropriate manner.

I had an account where a company of dictators (not doctors, but PACs I think) was hired specifically to do dictations of discharge summaries. There was a lot of shuffling of papers, but some of them did quite well, considering they probably never met the patient, and I was happy thatclear-speaking people werehired to do the dictations of a fewveryheavy ESLMDs. I don't know how accurate the dictation was, but it suremade my job a lot easier. I have to admit that I wouldn't want my own reportsas a patientdictated by an outside person, though.

have told me over the years how dictating is their least favorite part of their job. This is why so many of them suck at it. I used to the transcription supervisor at a teaching hospital and I used to have to train the new residents each year on the dictation system and I used to beg them to dictate slowly and clearly and I would bribe them with promises of food. I would stock the chart completion room where most of the dictated from with all kinds of snacks and treats. Docs are so sweet when they are residents. Then they turn ugly as the years go by!

Does Editscript V8 now with USB foot pedal? [2008-11-28]
I ended up getting a laptop for my version 8 upgrade since I use another program for another account and can't do both on my desktop. My 9-pin worked fine with a USB adapter on my desktop and the old version ES, but now won't work with V8 and the laptop. It is set correctly and can fast forward and rewind and stop dictation when I tap the center but won't play when I press the center key. I am stuck having to use Alt A to play audio and tap the pedal to pause, which has slowed me down considerably. I've tried to change the brand of the adapter to one of them suggested and it does the same thing as the one I was using. I wonder if it would be worth while to just get a new Infinity USB pedal. I notice when setting the port on Editscript options that it now has a USB option. Has anybody used this type of foot pedal with the V8 successfully? I'm getting very frustrated with this upgrade, especially with the expense at Christmas time and the hours waisted trying to get it to work, that I don't get paid for.

the only way I ever re-listen is if I have a word I didn't quite catch the first time and then will go back and listen to the word again after I type the whole report, but never re-listen to the whole dictation..I would be working all day and night.

protected by HIPAA, as we all know. I saw that ad too and was surprised to see it. Any identifying info is considered Protected Health Information, including diagnoses, tests, etc. This MTSO should have known better than to ask for that.

I never send in a report without relistening to it. I first type all the dictations at once and then speed up the dictation between 200% to 250% and go over everything. I leave everything to the proofreading, checking up words, referring physicians, correcting mistakes etc., unless the doctor is really slow and then I correct my mistakes as I go along, but still proofread. Since quick typing is so essential, I don't want to stop typing in middle to check things up and I found mistakes so many times (even small things like an extra comma, a missing letter, he/she, etc.) that I don't feel I can send it without proofreading.

I want to know what I can do to become the best supervisor possible.Personally, I have had only one great supervisor in all my years and what I found is that she displayed, fairness, objectivity, a sense of humor, consistency. Can you tell me what your idea of the perfect supervisor is?

Need feedback on VR (am feeling so inadequate) inside.... [2008-11-26]
I have not been able to double my lines. Most of the time there is no formatting, having to add a's, an, the, change sentences, add what VR left out even though heard correctly. When I talked to the manager, she say I am not embracing the software. I feel with so much editing even if the next report is better, I lose so much time on that report and others like it that even having several reports pretty good albeit bad formatting, like not capitalized headings, all clumped together and having to separate out, I lose a lot of valuable time. Idon't know how I can embrace anymore, I do use the tricks and tips but it doesn't help with the amount of editing. Is it just me, some people aren't cut out for it. When I pointed out at 600 lines I was actually being paid for 300 lines at regular rate but half at 600 lines and she was shocked I felt that way. That with VR it is so much faster. It seems when I compare the two, I am 500 lines a day short of straight dictation from previous. I know it is supposed to take time but how do you do that when your bills are still the same?
