question. Now it sounds like disabilities?  Before it was suntab, then suntaf, and desmilitis.  Is this the third time that the doctor has said it, or are you hearing 3 different things?  Is there a reason you cannot leave a blank?

On the psych evaluations I get, and this is probably account specific, but we show these as headings. Not that it is required, but the dictators really seem to prefer that type of formatting. There is even one who asks for many of these same terms (including things like "he had an Full Scaled Score of..." within the body of her text. So, I always just capitalize or put things in ALL caps when she says to.

Big ditto on siren's post. Even if you could please EMAIL me, would really like to know where you work and for what facility. I LOVE psych work and both my major primaries which were ALL psych work were lost to other companies (left the one I am at) and I miss it. Glad to help though and see above post on your blank before status! Bye!

It's hard to judge these things without hearing them - I went with what was closest to the sentence provided. While the patient could have been "dyspneic-looking like a patient with COPD or sleep apnea," that just didn't sit right with my gut.

Some have even typed Gram's stain, like Alzheimer's or Crohn's (no flames please, we all know  some believe in omitting the apostrophe s)...but if hyphenated, it's gram-negative, gram-positive (as in rods).
