I was taught to never change a transcription, even if it's blatantly wrong.  I would leave as dictated and leave a note for the doctor (if you have a place to do it)

The context doesn't give much away either.  One is the problem list, one is the past medical history, and one is:


There is a history of ... and he had a PEG placement.


Not even sure if the two go together, LOL, she tends to run her sentences together.

any time they spell it you can pretty much count on the spelling to be wrong. guess atleast they make the effort? lol better than the speedy ones who don't care enough to even attempt to make sure you know what they mean.

I hate it when the doctors/PAs prounounce diabetes MELLitus as diabetes mellITUS like it's some kind of inflammation or something.

First typed it back in 1970s. Then again in the 90s and the doctor used all abbreviations. Almost lost it that time!!!  They all have their little "docisms". keratinaceous is used by a lot of them. For real!!!

If the QA is insisting ".5" after weight instead of "1/2", wouldn't they also insist on typing "A 2.5-year-old child" instead of the obviously correct "A 2-1/2-year-old child?"  It just doesn't jive with me on some level, but then again neither did Aqua-Net hairspray.

Pes plano valgus with gastroc constracture, forefoot varus, left side, is correct. Technically, in OP report headings, I was always taught to spell things out like "gsstrocnemius" even if the doc abbreviates because it is a main heading, but it also goes with your account. Hope I have helped!!

It was not a medication, rather an "environmentally friendly" fly repellent used on livestock and pets.  Here's a link to one of the product sites I found it on:

word here. Waddell's test is an exam used to determine if someone is faking pain. If the patient has exaggerated complaints of pain it is a positive Waddell's.

What is FUV1, used along with FEV1??  Never heard of it and cannot find.  Also he also states FVC as being forced ventiltory capacity, which I know is called forced vital capacity; wonder if I should transcribe a non-word.

as to what a doctor MAY be dictating. Doctors make mistakes all the time. It is up to the individual MT to check out the validity of any info they ultimately transcribe. That's what I would do. Any MT who transcribes a "suggestion" should not be working as an MT. ;-)

I've gone to every medical website I can find. I have "asked the experts" on various medical websites. Even doctors can't find it! Thank you all for trying. If I come up with anything before I'm 99 years old, I'll post it.
