Alpha I (inflatable penile prosthesis) is also correct according to Stedman's GI & GU Words. Can't find the "OGE" though. The abbreviation "IPP" stands for inflatable penile prosthesis. Maybe doc saying IPP instead of OGE? Hope this helps.

Subject: In LAB portion: "a free T and a free T4"

I cannot find any just free T lab studies to be performed, and I am thinking he is leaving out the number after the T.  I could be wrong though, so if anyone knows of there is a lab test of just a free T please post back and let me know.

Thanks so much in advance.

The patient will be seen for followup in a week.

    Followup one week.

The patient came in for follow up of her acne. (I believe this can be hyphenated to follow-up.)

The patient will be seen for followup care in a month. (This, too, some MTs hyphenate to follow-up but I do not.)

The BOS has this very issue - Book of Style by the AAMT 2nd edition  - discussed in it.

Subject: We learn every day -- don't be afraid to ask your QA why? ..see msg

I'v been doing this for quite a few years, and last night had a report where I heard "Shoreman kyphosis," researched and found quite a few websites spelled this way, didn't really check to see source of websites, just that there were a LOT of them. (slap me with a wet noodle, I was tired and brain-dead because I know better)

Anyway, QA send me an email this morning with correct spelling, "Scheuermann kyphosis." citing Dorlands online.

You can bet I created a shorthand entry for that spelling immediately, and of course, sent of an email to my QA person letting her know how much I appreciated the feedback -- I never have questioned my research without the feedback!

    Have a great day.

He can say whatever he wants to in court. It may sound harsh, but that's really not your problem. If you accept a contract to do a verbatim account, then that's what you do - a verbatim account. I'm not here to argue the point with you of whether verbatim accounts are a good idea or not, or whether the insurance company, lawyers, or the man in the moon will like it. I gave the correct answer to the OP. A verbatim account is just that - verbatim. If that's what the client wants, that's what the client gets.
