Now some are saying it's q. 4h.??  UGH, YUCK!!  I have also worked on accounts where it had to be "every 4 hours," and we had to spell out p.r.n. (as needed for).  At least we got more characters. 


First rule:  It is all what the "client/account" wants, not always what the BOS says it is.  I realize that language and its usage is constantly changing, but I've seen so many BOS and client flip-flops on so many other things in nearly 3 decades that it's laughable.  What isn't laughable is when we get penalized for not knowing what they want, even when they, themselves, don't know or are wrong.


Oh, and I still prefer the old no spaces:  q.4h., q.4-6h., q.12h., and so on.....

As we all know this can get to be somewhat boring, but learning from what I transcribe as well as from other transcriptionists keeps me going (money is not bad either). It becomes a challenge. But I can't thank you enough for the kind words.

From what I understand, most doctors prefer shotty as opposed to shoddy. I don;t know if this helps or not, but maybe

Shotty: Resembling shot or pellets of lead, shotgun pellets and, hence, hard and round. The term "shotty" was in use in the 19th century. It is now generally obsolete but it is still in medicinal usage.

Shotty is very commonly used in describing the feel of lymph nodes (the lymph glands) when they are palpated (felt) through the skin. "Shotty" lymph nodes are ones that are not only hard and round but also small and surely of no consequence.

No consequence is not always the case with shotty. The pustules (the little pockets of pus) in smallpox are said to feel shotty, according to D.A. Henderson, the director of the World Health Organization (WHO) Smallpox Eradication Unit from its inception in 1966 to 1977. "Smallpox pustules have a dimple, a dent in the center. Doctors say that the pustules have a 'shotty' feel, like shotgun pellets. You can roll them between your fingertips under the skin," Henderson recounted (quoted by Richard Preston in The New Yorker, July 12, 1999).

When I worked for a previous transcription company I used to put shotty and was constantly corrected and told that it was shoddy.  So that is what I did.  Oh well.

I thought I was losing my mind when I heard "beetle nuts." Now that I have this information, I'm writing it down for future reference. We have a lot of Asian patients in this practice and it is probably what he was dictating. That's what I like about this business - you learn something new EVERY day! That's also what's great about our MT boards - MTs willing to help each other continually learn.

I have a doc that keeps dictating "dermatomy" when referring to skin incision.  While this makes logical sense to me, I can't find this in any medical dictionary.  Not sure what to put or just flag and bring it to her attention.  Any ideas?
