AAMT BOS does not specify layout for this either, so if the account is "as dictated," then just transcribe it as the author dictates.

For your info.

--I haven't done ops in more than 15 yrs.

--I was hired to do discharge summaries. Instead, I was started on this new job tonight with 5 dictations to start. The first 3 up were--OPS!!!.

--There was no one to call at the company to look into or correct this situation at this hour, so I decided to forge ahead and do my best regardless.

--I posted asking for help, not for someone to be snide and to tell me how wrong I am and how they're not going to type it for me.

--In future, do us both a favor: Don't reply to any of my posts. I certainly won't be reading or replying to any of yours and I promise not to miss you--regardless of whether you have the guts to sign your real name or not.

I know ~ that's why I posted. I was sure there had to be something else.

On the psych evaluations I get, and this is probably account specific, but we show these as headings. Not that it is required, but the dictators really seem to prefer that type of formatting. There is even one who asks for many of these same terms (including things like "he had an Full Scaled Score of..." within the body of her text. So, I always just capitalize or put things in ALL caps when she says to.

Big ditto on siren's post. Even if you could please EMAIL me, would really like to know where you work and for what facility. I LOVE psych work and both my major primaries which were ALL psych work were lost to other companies (left the one I am at) and I miss it. Glad to help though and see above post on your blank before status! Bye!

It's hard to judge these things without hearing them - I went with what was closest to the sentence provided. While the patient could have been "dyspneic-looking like a patient with COPD or sleep apnea," that just didn't sit right with my gut.

by the administrator??? I am giving my point of view and no I am not repeating anything. I am saying it differently and it cannot hurt. Is it okay for posters to give WRONG ANSWERS? why are you not telling them enough already and give it a rest???

How come when you see the same answer on the help board over and over again, why are you not piping in telling them they are giving redundant anwers?

This subject must have hit a nerve with you.

I will not give anything a rest. Thank you. Have a nice day.

...Dictator simply gives visual acuity statistics in prior sentence, then states "The ________ visual field exam is intact." I got two of these reports one right after another, first time ever doing this dictator or this specialty. While researching for the second time I actually waited to get the detail report on the prior chart so I could see what QA had entered - waste of time! They left it blank too.

Of course this dictator also lists drugs like "Actone" (probably Actonel) and grossly mispronounced several others, so who knows? Motalakian, tularakian....weird!

questioning whether either was 1 word, but if they had 2 separate meanings; i.e.; patellAfemoral versus patellOfemoral.  I've seen it written both way on a Google search

From what I've just been reading, patellAfemoral syndrome relates more to knee conditions with: cartilage damage; menisci, ligament and capsular damage; a sequelae after patellar fracture, dislocation or subluxation; sequelae after knee surgery; quadriceps tendinitis; iliotibial band friction syndrome; pre-and infrapatellar bursitis; and pesanserinus bursitis.

Thanks again for your interest.
