The patient has chronic s/l mydromitus, causing early tear film break-up time.  She probably also has decreased aqueous production.  I suggested she use an antibiotic-steroid lid scrub twice per day for the next month.

How does the sentence read and what type of report are you typing?
neck is rigid but s/l puretic.

flaccid quadriplegia but the neck is rigid but s/l puretic (patient in a coma from CP arrest).  I don't think pruritic makes sense.
