Oncology query...irradiated or radiated

This is from a transfusion reaction report for my oncology account:  She was receiving the second of 2 units of leukoreduced irradiated red blood cells on March 20, 2007, for a hemoglobin of 6.6 g/dL and developed skin flushing 15 minutes after the unit was started.  The transfusion was stopped, and she was given 25 mg of diphenhydramine orally. 


I can't honestly be sure that she's saying irradiated or if she's actually saying radiated.  I got a sample from the office for another transfusion reaction report by the same dictator, and that report had leukoreduced irradiated platelets.  That's influencing my hearing, I think.  This is above my realm of knowledge, and I'm honestly not sure which it really should be, and I don't want to totally trust the other MT's report when I know nothing about her or her knowledge base.

I get Cytoxan but am questioning the other 2 because I cannot find any references. Can anyone help? TIA!!!!!

The report comes back as malignant melanoma, superficial spreading type.  Breslow thickness is 2 mm.  Clark level is 3, and it is positive for the epidermal ulceration.  Melanoma in situ focally extends to the s/l int or aint lateral resection margin.

Trying to type a lab for an oncology patient and have no idea where to look to determine what he is talking about.

This is what I hear Blood chemistries are obtained for S-peptin and _______quantification, immunoglobulin quantification and beta 2 microglobular.   I have been unable to verify this or to fill in the blank.

I'm doing an oncology report by an ESL on a patient with an abnormal platelet and white count.  She says they have sent investigations for molecular diagnostics including BCR-ABL translocation.  Then she seems to back track and correct herself and say BCR-ABL and translocation and jactermitation to evaluate for myeloproliferative disorders.  Any ideas?


Meanwhile, I'll go back to trying to research this on Google.

No, I don't think it's hypermutation, but I believe I've got it. I believe she is saying BCR/ABL translocation and JAK2 mutation.

I think it is my weakest specialty. Scared of oncology! I can usually get through the rare reports that I get, but they are difficult.

Have tried everything I know of. I do not hear intracranial in this. It is decidedly interferal, intrafural, interfural, etc. I've tried antero, entero, inter, intra, you name it.

HORRIBLE ESL dictator, you know the kind... the one everyone else skips in the queue.
