so no comma is correct. The elderly patient is pleasant. Not pleasant and elderly.
Per AAMT says to hyphenate adjectives such as this

(notice I left the casual word "say" out.)

They actually are not fragments as they contain necessary parts, but punctuation shows how they are related to the other thought.

Capitalize first letter after a colon. Forgot an apostrophe to indicate possessive case,as the cough belongs to a smoker. Forgot serial comma after nose.

As to format, if they let you get away with each system starting on a separate line it will be clearer and give you a higher line count.

Finally, to quote my medical editing instructor at Cornell Medical School, "The addition of "al" to an adjective does not make it any more "adjectival" -- I always got a laugh out of that line. I've been correcting docs and transcriptionists for years about neurologic and hematologic.

SKIN:  Lots of seborrheic keratoses.  He does have an elevated 9 mm papule on left side of the face, which has some vascularization and central core.  It almost looks like acanthokeratosis, but basal cell carcinoma cannot be fully ruled out.  No other suspicious lesions.  He does have some miliary rubra on the back.

Assessment:  Nevus s/l: roo-let-ee s/l: tip-e-er midabdomen.  (Nevus ruletti tipiar midabdomen)?? AND:


s/l: Dermatified (or dromatified) broma right shin.  She uses this phrase in a previous sentence:


On his right shin he has superficial change that appears like a dermatified broma; however, it is not as nodular beneath the shin, but it is definitely excoriated or irritated on the surface. 


I just cannot find any info on these two - sorry this is so long.  I am covering for someone and would prefer not to send to the editor!!  TIA

Study showed "fluid collection in the upper anterior midline abdominal wall was again noted with a similar appearance" (much thought was given to blah, blah, blah). 


Do I have the second quotation marks in the right spot or should they go at the very end after the end parenthesis and period?

I have heard different things on this issue...


Does punctuation, whether it be periods or commas or whatever, always go inside parenthesis or not?  BOS shows they don't, but I've also heard that punctuation always go inside the end parenthesis.

Was or were 40 minutes?  Should it be punctuated with a semicolon?

Browsing this morning, I read original post and thought I had had "servant" as "cervatte" or something before -- looked up a couple different spellings, and could not find, so figured I must have been wrong and moved on. However, this one would have bugged me enough I would have spent more time searching later...

since telantiectasia is one of the symptoms/signs of rosacea I believe these sites are okay and reliable. See below. Sort of like saying papulo-pustular, which in the old days when I typed derm was always hyphenated etc. Just new derm-speak I believe
