PET SCAN 03/08/08:  Reveals intense hypermetabolism corresponding to the mass in the left mid lung field posteriorly, abutting in the left major fizure predominantly in the superior segment of the left lower lobe.  This same type of metabolism corresponding to the right middle lobe pulmonary nodule previously noted activity in the lower neck anteriorly (06/28/06) has since resolved.

S/L askute??? result can greatly affect the mean, but the median is not as affected.    Any clues?  This is a horrible dictation.

At that time, it was decided to explore (s/l trileptory) concerning for resection of small bowel. The patient went to surgery on that day for exploratory laparatomy, small bowel resection of primary anastomosis, and was discharged on 03/18/2009.

Patient has ear pain. Has had myringotomy tubes placed in the past.

The right TM was a little dull, with loss of landmarks and either a fluid s/l wave or some fluid related to the eardrum. There was no evidence of a myringotomy tube in the ear canal.

Fluid level? Does that work in this regard? Thanks.

She received a total dose of 45 Gy to the primary tumor area, regional lymphatics at 180 rads per fraction x25 fractions.  Then she received cone down 1800 Gy to the primary tumor.  The total dose was 63 Gy.

Doctor is describing a patient's finger pain.  He says:


"At her last visit she did have a maintenance interscalene block and reported several months of improvement in pain to a level of 3/10 when used in combination with her other conservative therapies.  These have included Lac-Hydrin applications, paraffin wax applications, and (s/l "co-bayne") and protective (s/l "cutlets") to the left ring finger.  The patient has dystonia and extreme difficulty in bending at the PIP joint from a functional standpoint.  Numeric pain score is an 8/10..."


First word sounds like Cobain or Cobayne, pronounced just like the last name of the late lead singer of Nirvana.  Second word sounds just like cutlets, like veal cutlets.     


I have no idea at all what these two words are, and he repeats them in both the subjective and in the assessment.  Can anyone save me from leaving four blanks? I'm sick and exhausted with a sick four year old at home, and I'm feeling a bit fussy and frustrated and would be SO thankful for the help.

I am stuck on a word,  maybe two words..not sure! This is for cataract closure, the procedure is phacoemulsification and peripheral iridotomy.


"A superior corenomy was performed and hemostasis was achieved with wet-field cautery."


It actually sounds like "supear corenomy" but I thought he may mean "superior". But I have no idea what the next word should be.


Any thoughts?

I'm a little confused, the doctor dictates "He is at a point where he is disabled by the anal fissure pain, and I have recommended surgery in terms of an anal fissurectomy and lateral internal sphincterotomy.


There is no such thing as an anal fissurectomy but I swear that is what he is saying, and I would think it would be a fistulectomy but through the whole dictation it sounds like he is saying fissure.  I am a little confused with this.  Any ideas or help would be greatly appreciated.  Thanks.

    This one's stumping me after 15 years of experience... medical student dictating a report on social history:


    The patient drinks one martini a day.  The patient denies tobacco use.  The patient denies ___________ (s/l  - no kidding - really  - "beetle nuts.")


    Diagnosis is squamous cell carcinoma of the mouth - ENT tumor board report.  Thought it could be something to do with chewing tobacco - something of that nature, definitely not saying "drug abuse." 


    Any ideas?  I did leave a blank, but I'm hoping someone might put my mind back on the right page... beetle nuts?  Give me a break.

    I'm pretty much clueless with regular clinic work. I basically went from school to typing ortho, so there's a lot of things I learned in school that I've never applied, and this is one of them.

    At any rate, I learned it was mmHg, but with the doc dictating just mm, it threw me a little bit and I was wondering if I should type it as mm or mmHg, or does it matter...

    Sorry I was so vague in my question. I typed it out fast, as the phone was ringing.
