There is an instrument, and I think it either a clamp or clamp-like, that is refered to as a Davis & Geck in various procedures I have seen.

case I think it is incisors, because you are right, the scope goes directly into the mouth, down the throat, throught the esophageal sphincter, etc etc.  I was thinking of the "wrong end!", and thus incision!
