or a portion of it specifically to induce sterility.  It absolutely is not correct to leave vasectomy in a female patient's report.  It is wrong!  Nowhere in any of my reference books can I find vasectomy referring to anything other than the vas deferens for male sterility, NOWHERE.  I don't care how you break up the word, you can not change its meaning as a whole.


The QA person who filled it in is a boob who is simply filling in blanks for the sake of sending a report with no blanks.  The report simply should have been sent with the blank and a note drawing the physician's attention to the discrepancy.

If you don't have Stedman's Medical & Surgical Equipment Words book you should get it. This book is amazing. Pretty much any piece of medical/surgical equipment is in there. It is worth its weight in gold. If your company doesn't offer discounted books, you can purchase it a Amazon.com either new or used, and they have great used book. I bought my entire collection of Stedman's words book used. All of them are in at least good condition.

that doesn't mean the surgical excision of a lymph or any blood vessel could not correctly be referred to as a vasectomy. It may not be common, and it may not be the textbook definition, but the term does not have to exclusively apply to the vas deferens. If you were going to refer to the surgical excision of a lymphatic or other general vessel in a single word, what word would you use? Anyway, I'm not arguing whether or not the dictation was accurate, I'm just saying if that is what was truly said and context did not support a more general use of the term, then the real boob is the physician who dictated it, and if that is what he/she said, and it is clear that is what was said, then I would send it through as well and let the physician confront his/her own potential error in proofing the report and correct it then if it truly needs to be corrected. Patient safety is not involved in this case, so if it is an error and the physician does not take the time to do their job and proof read the report and correct it, then the egg is on their face, which is where it belongs anyway.

Hi, I'm a semi-new transcriptionist who has had no formal training, and I'm having some problems figuring out a couple spellings.  One is a medication called *Inderon* (not sure how to spell).  The other is a phrase that I'm not sure what exactly the doctor is saying.  It sounds like *bill-raw-2 anastomoses*  Thanks SO much for any help!!!

- Glypenitin? Some kind of medication. Don't know what it is for as this patient has many diseases.
- Sounds like "see-ko" syndrome... I think. I can't understand her!! Fecal syndrome? LOL Maybe it's sickle syndrome.. I don't know.
- Sounds like "smith". As in positive ANA, positive SSA and SSB (I can't tell if it's SSA and SSB or just one or the other GRR!!!!), positive RNP, positive __________, positive rheumatoid factor.

I'm going to try to finish this report. Wish me luck. :/
