Okay, I'm transcribing a SOAP note and this word that I'm trying to figure out is under the ASSESSMENT AND PLAN part. She says the word three times and I am now thinking she is saying something like "number next or number next is." What do you think?

(Normonaxis) Anemia may be caused from a recent epistaxis. He will start an iron pill daily over the counter called Fergon. Next visit we will check a hemoglobin, iron studies. (Normonax) He was in for right shoulder pain in the past. This was resolved. (Normonax) Recent qualm was treated with doxycycline.

Also, I was told to transcribe this using a straight SOAP format, but the doctor dictates things like "Review of Systems" and "Exam." I didn't think those headings or subheadings were in a SOAP format. She never dictates "Subjective" or "Objective," only "Assessment And Plan" as one heading towards the end of the report. Any advice?

References are inconsistent as to Kitner versus Kittner for the surgical sponges, clamps, etc.  Anyone have a reference identifying the ACTUAL PERSON these are named for?

(And please don't just respond telling me how your reference book spells it!  I have plenty of my own, thank you!)

Dear Oldtimer:

Thanks so much for the scissors advice. Indeed that is what she was saying. Her last name is Johnson, so there is no excuse for her poor enunciation of words!

I appreciate your help.

There's a plastic sheath that they use initially to place the sling and then they place scissors or a clamp between the urethra and the sling for support. I would think they would need to remove them, although some of them don't (insert embarrassed face here). I did some surgical reports, my first year working, for a hospital and they did lots of transobturator and tension-free tape procedures. It seems like they always "removed" the scissors. Never heard of an incisor in this procedure. :-)

Doc is doing carotid endarterectomy. Statement is below. Cannot hear kind of scissors, the most I can get out of it is Fox, but it is very, very low. I know there are Fox scissors, but I don't know if they are used for an arteriotomy. Any help is appreciated.

The internal, external and common carotid arteries were then clamped. An arteriotomy was done in the common carotid artery and carried upwards towards the internal carotid artery with _____ scissors.

I have been transcribing for 15 years now, and most MDs use bandage scissors for uterine incision because they have a rounded edge and are not pointy, thus cannot injure the baby...just cut tissue between blades.

Procedure:  Excisional biopsy of a left earlobe mass.

Surgeon dictates what sounds like the "canum" scissors were then used to excise the skin lesion.

What is the name of the scissors?

I use google first and get to healthgrades dot com if I know the state and then I only click on the CACHE button thru google and only the MD I'm searching for will highlight in all the states.  I hope I made myself understood in that sentence.  I also use anywho dot com and 411 dot com, 411locate dot com, dr-411 dot com, and last but far from least:
