In the upper tibial area, there is a 35 x 5 cm eschar in a linear fashion oriented superiorly to inferiorly.  I did put __________ lidocaine on this area and tried to debride it, but it was too tender.

but it still sounds better if you use "was" instead of "were." Just as you wouldn't say, "2 cups of water was spilled" but instead, "2 cups of water were spilled." It's a matter of what sounds better, in some cases. :-)
5 mL of 1% lidocaine WERE injected...

In response to Mark's question, "5 cc of 1% lidocaine was or were injected", the text from the grammar website below, from the topic, "Subject-Verb Agreement", explains why "WERE" was the right word there.  Per that text, the "5 mL" (but not the "lidocaine") becomes the Subject, and the Verb should be in plural.

many times a contrast material will be injected after when they do the epidurogram to check the appropriate placement, usually Omnipague-240. Sorry I cannot be of more help here with no s/l.

I have had that happen to me before, and I was told by QA to not move things around in the report, maybe that is the same for you, but if I were typing this report, I would type it in the order as is dictated, even if that means typing it under the heading PLAN.  Also, his vitals were probably taken at the end of the exam as they were repeated, so it would make sense to type them again under that heading.  Hope that helps!
