We had a discussion, about 30 minutes long, outlining the treatment options from primary surgical to primary S/L: NEAR-AGIMET modalities, which I recommended.
Patient has poorly differentiated adenocarcinoma of the EG junction or GASTROCARDIA. (is this right?)
This doctor wants to treat her with chemo and radiation.

The wounds were dressed and wound care instructions were given.

This patient has osteoarhritis of her right knee.
The patient is a 61-year-old female with a long history of right knee pain. She pursued conservative treatment <s/l al-er-d-es> without significant relief. She proceeded with a right total knee arthroplasty.
I can't for the life of me think of what this could be. I thought maybe it was just suppose to be "conservative treatment without significant reflief" but it sounds likes she's saying something there, but I can't figure it out.

When transcribing oncology reports, does it matter how you spell out the cycles, such as: The patient has received 2 cycles of chemo. Or, is two cycles of chemo. Or ..... does it matter?
