I'd just make another subheading (flush left,  if that's how the CP says to format and it is not a verbatim account....

well, if the doc is doing abdominal exam, he would be looking for guarding, tenderness, rebound, etc

abdominal and pelvic region to rule out either a renal (kidney pain is mixed with back pain), or peritoneal or or intestinal pain, which can radiate to back. So if you hear it clearly, I think it is right, many docs use certain phrases lots of times!

I keep getting the same dictator who I believe it is during the abdominal exam although he is speaking 3 million miles a minute:

While transcribing the back exam, the doctor says what sounds like "no rotation or scoliosis noted."  I'm not exactly sure if he is saying "rotation," but would that make sense?  I've never heard that used in a back exam before.

I learned from from a QA's report.  How are we to know there are changes on things that were done one way for so long and now done another?  geesh
