presents for evaluation of chronic cough with peripheral eosinophilia and abnormal CT of the chest, per report (thickening of the bronchial wall).  The constellation of findings is suspicious for chronic eosinophilic pneumonia or allergic bronchopulmonary aspergillosis.  However, the patient does not have a prior history of asthma and per report, the chest x-ray or chest CT did not show any pulmonary infiltrates.  She does not have an extensive history of travel to suggest “ham et ic” infection.  However, she traveled through Brazil many years ago.  The IgE levels are elevated but not the point to suggest allergic bronchopulmonary aspergillosis.

She states that she is unable to eat, continues to lose weight, and is having bouts of vomiting.  She did have her 48-hour pH test completed and extensive *physiatry* to view the findings which demonstrated that on the first night when she had vomited after her procedure, there were some episodes of breakthrough acid; however, the following day when she was tolerating a diet and had her AcipHex, her pH was well controlled, above a pH of 4, essentially the entire time.  Therefore, I discussed with the patient that her chest burning does not appear to be correlated with breakthrough acid of any significant degree and suggests the possibility that a lot of her chest pain and burning may be related to her anxiety and depression.

The patient did have laboratory studies performed in February 2008, which revealed normal MTHFR, protein-C, protein-S as well as antithrombin III, negative Factor V Leiden mutation, anticardiolipin antibodies being normal with PAI-1 Stage 4G/5G being heterozygous.  The patient’s had a normal homocysteine level of 8.2 as well.  In April 2008, the patient had other studies performed including a prothrombin 20210A mutation, which was negative with anti beta-2 glycoprotein antibodies being negative as well.

Can't make out this word.  Sounds like ace-ee-iss with the stress on ace.  Here's the whole sentence.  "A CT of the abdomen with contrast revealed nodular [ace-ee-iss?] entry into the head of the pancreas and the omental fat, which was felt to represent adenopathy or recurrent tumor."  Any ideas?  Thanks!
