Doc is very hard to understand... i've been doing this for a long time, and I have always thought that cor belonged in heart exam.  This doc speaks very low, but s/l he continually says on physical exams, ...


lungs cor rsr.  I figured lung exam was separate and i was missing something he said, and then i should type .... Cor, RSR (for regular sinus rhythm).  Can somebody please clarify for me?  I saw another transcriptionists report that said ... lungs cor rsr.  That does not look correct to me and many of these other MT's make a lot of mistakes.

Active Tuberculosis
To diagnose TB of the respiratory tract, 3 to 5 sputum specimens are collected first thing in the morning on different days when they are most likely to contain the most mycobacteria. Jugular veins end at bottom of the neck to subclavian veins, so how in the heck are there jugular veins in the lungs? He has to be in the neck!!!

I question "Cardiac size and cardiovasculature" and also have a blank. 


"Cardiac size and cardiovascular" are normal.  No infiltrate or pleural fluid is seen  ___________ trachea is midline.  There is no hilar enlargement.
