Slit lamp: Cornea shows endothelial pigment bilaterally. It is not in a pattern but it is just uniformly pigmented on the endothelium of the cornea. Anterior chamber quiet. No cells or flare. Lenses are clear. No sign of any s/l sign-ick-e-a.

Physical exam of a diabetic in ketoacidosis
Subject: Physical exam of a diabetic in ketoacidosis

You all are always such a great help.   Discharge Summary of a patient in diabetic ketoacisosis.  On their physical exam, the doctors is dictating about their eyes I believe, they say:  "No nystagmus."  then right after this it sounds like "No lid lag optosis", followed by periorbital edema".  Help!  Can't find anything to replace "No lid lag optosis!"

I just pasted it on as she dictated it.  No idea what it means or where to use punctuation, etc.  Not sure I'm even hearing it correctly.  She barely speaks English.  Any help is appreciated.  I don't normally do this doc.  Thanks!

Neuro - not sure if I have this right - sensation testing of diabetic sm
Subject: Neuro - not sure if I have this right - sensation testing of diabetic sm

Usually this doc says something along the lines of monofilament testing is intact, which is also how the other docs have dictated it to one degree or another.

However, this time he says, "a nylon string test reveals s/l 5 point of sensation on both feet."

Don't know if it should be 5 points of sensation, 5-point sensation, or if it's simply 5.0 sensation. Is anyone aware of how to type this?

Thanks for any help with this.
