That's always ben a puzzle to me too, but I think that either way might be acceptable. The qualifying adjective (bilateral) suggests an "augmentation" was performed on both sides, and more often than not, MDs will dictate singular nouns following bilateral. Sometimes, though, if it sounds strange, I will use the pleural, i.e., one of my docs says "bilateral lower extremity" which I change to extremities. Would be interested in further input or if AAMT has a directive on this.

Evidence of old TBC. I'm thinking he means tuberculosis. Is that right. Also under discharge medications he has Rifamate, Theo-Dur and it sounds like he's saying prinsom or primson and taper down over two weeks. Any ideas? Thanks a bunch.

apparatuses or apparatus.  Have never seen a doc that didn't try to make up his own form of word from time to time either though...so if your account states strictly verbatim without the ability to correct grammar even when you know it's incorrect then I would type what he says.

For "small and large bowel" and for "lumbar and thoracic spine" do I think of it as pleural or singular?  Would I type small and large bowel are or small and large bowel is? Lumbar and thoracic spine are or lumbar and thoracic spine is?  It gets me every time!
