Subject: Slant-back procedure on bilateral halluces.

Halluces is plural for hallux.

slant-back is a type of nail avulsion.

http://users.erols.com/ibeiser/foot.htm#Ingrown%20Toenails%20(Onychocryptosis)

"1. Slant back nail avulsion. This may be done if the nail is not deeply embedded. This may be just a temporary measure and is often done without anesthetic. In this procedure, a nail clipper is used to create a tapered edge of the remaining toenail as the ingrowing portion is removed."

Maybe it is in regard to the patient's weight?

'The patient is tipping the scale at 58.'
And 58 being his BMI, morbid obese.

Just a wild guess, sorry!

The doctor dictates:  "The patient states his pain is 2 out of 10 on a scale of 0 to 10."

OR
