he pain of the anesthesia injection itself can be minimized by injecting the agent slowly and steadily.[4,5] The addition of 8.4 percent sodium bicarbonate to the lidocaine (ratio: 1:10) appears to decrease the burning sensation during infusion, presumably by raising the pH. The correct angle for a subdermal injection is 30 degrees, which will smoothly raise the lesion from the skin (Figure 1). If the injection is too deep, the lidocaine will diffuse into the fat, and a wheel of anesthesia will not form.

Under the definition of wheal, the round, elevated area of skin must be different in appearance from the surrounding skin (like in hives). So, when this surgeon has decided to do a suprapubic catheter on this gentleman who had a misplaced Foley, he elevated a "wheal" of skin (he spelled w-h-e-a-l). I'm not sure about that. Opinions?
