They do not always pay headers and footers. I know that on my particular account, a certain percentage of lines are taken out to account for those. For example, my line counts are usually about 10% less than what I count up myself in Word, and I was told that a percentage is deducted for headers/footers. And yes, a large amount of work is going to speech, and some accounts are also shared with offshore entities.
