Companies With No Line/Hour Commitment

Anyone know of companies that do not require a certain number of lines/hours per day or pay period.  I'm looking for a company that just needs help without a commitment.....maybe overflow, p.r.n. or fill-in work.   I work outside the home during the day and I recently had to give up a great company because of the daily line count.  Thanks!

Line Commitment
There is a $7.50 fee, but if you commit to a certain number of lines and meet it the fee it is waived. They have a seperate fee that is pro-rated if you do not meet your commitment, but I have never had to pay that. It was explained to me that they have that because so many MTs contract with 3 & 4 companies and don't meet their commitment, it's to keep people who are not serious about working away. If they are not paying you by the hour, but BY THE LINE, it is actually none of their business when you take breaks as long as you meet TAT.  That is a problem I am perceiving with some of these places:  expecting you to sit at your computer for a fixed amount of hours EVEN IF THERE IS NO WORK.

Please explain this to me.  I am not new and not naive, but I really would like someone, preferably an MTSO, to answer a simple question for me.


Why is it that QA, for most MTSOs, still get paid by the hour, but VR gets paid by the line?  Are they not basically the same thing? 


Am I wrong? 


Would you prefer a higher cpl for Editing VR or an hourly wage for Editing VR? 


Before VR, the MT would type the report and leave blanks for words/phrases the MT could not understand.  Those already typed reports would go to QA so that another ear could try to decipher the blank and send a nearly 100% correct report to the client.  QA was always paid by the hour because the reports they worked on ranged from one blank to fill in and little listening to the voice file required, all the way to listening to the entire voice file and practically retyping the entire report for a newbie Transcriptionist or a totally unintelligible ESL dictator.   Now, doing VR, the “Editor” receives (supposedly) already typed reports.  The Editor then has to listen to the entire voice file, and sometimes there are very few changes, while at other times, the entire report gets thrown out and typed from scratch. 


Also, I think VR is dangerous when paid by the line.  As every MT has been told since transcription began, “if you leave a blank, DO NOT leave a sounds-like word – just leave the blank.”  Everyone knows that if the Editor is not really paying close attention, the word typed on the page can look/sound “reasonable” when compared to what is heard because the suggestion is there that the word is correct.  If there is a list of Diagnoses or medications with no context of what symptoms and past history the patient has, it is very easy to assume what has been typed is correct, whereas, if no suggestion was made, the MT/Editor may hear something different.  There is not an MT on earth that has not raged against QA because QA changed something that the MT swore they heard right and typed right the first time.  I vividly recall nearly being fired for insubordination because I was livid with one QA.  It was an ESL dictator doing a discharge summary.  The patient was on over 20 different medications.  I heard and typed “q. day.”  The QA heard “q.d.”  The account specifics stated that “q. day” was to be typed as is, but “q.d.” was to be changed to “daily.”  No change in content, but I was assessed an error for each and every change QA made because of “not following account specifics.”  Do you think an Editor making 3 or 4 cents a line cares if the dictator is saying “q. day” or “q.d.” and will bother to change that, whether or not the account specifics requires verbatim transcription.  There are many words that sound similar that a tired or stressed Editor will skip over because it sounds/looks acceptable.  Unfortunately, there are also probably whole paragraphs that get little scrutiny because the Editor is trying to get their lines done to avoid losing benefits.  I would never say these Editors are trying to get paid for not doing their jobs, it is just physically difficult to concentrate for extended periods of time at the same intensity, and unintentional errors do get by.  Again, we have all probably run across finished reports in medical records with blatant errors that we swear would never get by us but which the MT who transcribed it felt they gave 100% of their effort and produced a 100% accurate report.  None of us intentionally would send back to a client a report full of errors, but it does happen.    


If QA can be paid by the hour for doing the same thing, isn’t it more equitable to go back to paying MT/Editors by the hour?  The way things are now, everyone is stressed because they are afraid they won’t make enough lines to pay the bills or keep insurance.  Of course, there would be minimum editing lines per week, just like QA has to prevent Editors from taking their sweet time, but I think the MTSOs would benefit also because they would also have a more definite payroll month after month.   When I worked in-house and was paid by the hour, I never had that stress.  If I had a few really bad days with impossible ESL dictators, I knew it would not hurt me.  I guess I am not competitive, so I do not care if I type 400 lines an hour or edit 1000 lines an hour in order to make a $10 a day bonus.  I just want a decent paycheck that I can count on month after month. 


What is your opinion?  Would you prefer a higher cpl for Editing VR or an hourly wage for Editing VR? 



 


 


Commitment
Just to clarify, I was told that p.r.n. people don't have to have a set number of line committed. The recruiter told me part-time must commit to a minimum of 700 lines per day 5 days per week, and full-time 1200 line per day 5 days per week. Hope that helps!

Here are some that I know for sure pay by the hour; Probity, AllType, MxSecure, DTS America, Accel, New England Medical Transcription. There are more; I just can't think of them right now.

Yesterday there were some quite interesting posts concerning the trouble some of us seem to be having meeting/exceeding our line counts.  After giving this some thought today, having a need and a strong desire to earn more, I have formulated the following:


THE MT'S COMMITMENT TO EXCELLENCE


In order to deliver the best quality reports that I possibly can, I promise to: 


1.  Verify all demographic data. 


2.  Accurately transcribe all medications and dosages, spending a reasonable amount of time utilizing the resources available to me including my electronic drug book and the internet.  If uncertain, I will leave a blank.


3.  Verify the spellings of any unfamiliar words using my electronic medical dictionary and the internet, and again, if uncertain, I will leave a blank. 


4.  Spend a reasonable amount of time searching reference materials and the internet for names of surgical instruments, procedures, acronyms, etc., and if uncertain, I will leave a blank. 


5.  Quickly review lab values to ensure accuracy. 


6.  Do my very best to decipher what is being mumbled, partially pronounced, or mispronounced, and if uncertain, I will leave a blank. 


7.  Spell check and quickly review the report in its entirety, checking the formatting of the report and looking for any obvious errors on preview, prior to submitting. 


8.  Make an honest effort to locate on-staff physicians' name in the physician lists provided.


9.  Transcribe the reports in the order they are received, no matter what they are. 


10.  Submit the report only after it has earned my seal of approval. 


HOWEVER, there are limitations to the extent to which I will go from this point forward.  I will no longer: 


1.  Edit my own reports.  I get paid the MT wages, not the MT and Editor wages.  After spending a reasonable amount of time locating missing information, any blanks left in the report will REMAIN in the report and it will go to editing.  I will no longer rewind to the beginning and spending additinal time doing someone else's job. 


2.  Search for off-staff physicians.  For example, Dr. Smith in New York City.


3. Work overtime without incentive to do so. 


My line count has been so much higher today because I refuse to spend any unpaid time searching for elusive information.

even if you are only doing 1400 lines a day at 8 cents a line that is still $15 an hour...

How many jobs are you going to get outside of the home making $15 an hour, especially in this economy and most parts of the country?  That is only 175 lines an hour and most nationals expect at least 150 an hour, so that is not that much of a push.  If you do 200 lines an hour, or 1600 lines a day, look at the raise and that is $16 an hour.  This is also only making 8 cents a line, which by the time you are making over 1300 lines a day you probably are getting some sort of incentive, at least a lot of companies you do, and that would push this number up even more.


I used to be a graphic designer for many years and to this day pay for a good designer is only $12 to $14 an hour and you have to commute to the job, so how is it not realistic to make this type of money.  I do not see where your logic is and if you are making so bad then maybe it is time to re-look at your profession and find something that is more a fit to you and make the money you deserve.  I just do not see how making a minimum of $14 an hour is so unrealistic.  Geez.

Twelve-hour window companies (sm)

I know this has probably been asked before, but I'd like to know of some companies alow 8 hours of work within a 10- to 12-hour window?  I don't mind punching in and out, I just can't sit for 8 hours nonstop and produce like I need to.  I know other factors are important, but I think that's where I'm going to start this time.   BTW, no need to offer up MQ.  I work for them PT, just need a good FT right now.  Thanks!


Most companies go by the number of lines per hour,
adjusted for full-listens, etc. To try to go by the number of reports is idiotic. There are too many variables for that.
Did it explain HOW you get the 18 days? Is that for all 40hour/150 line per hour folks?

High line rate versus possible lines per hour....

I'd like your opinions...  I think it's more important to be able to produce a high line count than what your actual cents per line rate is.  $0.10/line means nothing to me if I have to struggle to produce 200 lines per hour because of their format or platform.  I'd rather make $0.07/line if I can count on producing 500 lines per hour. 


 


Just something to think about when you interview with a potential employer who tries to dazzle you with a high line rate.  I always ask about the average line counts and if they have auto-populated headings.  We don't get paid for auto-populated headings (which can make a significant difference in our line counts), yet I'm sure the service bills the hospital for them.

I have many, many years experienced and was just told a little while back, even though I am an IC, that I have to have a set schedule...And I am struggling to follow the set schedule due to family illness....

Any companies hire to transcribe with a 12 or 24 hour window
I am wondering if anyone can tell me of a reputable company who gives a window of like 12 to 24 hours to get your lines in (part-time), instead of an hourly schedule.  I'd sure appreciate if you'd share this info with me.  I've worked a long time in this business and I'm looking for something like that with flexibility.  I appreciate your help.

Lousy companies pay QA by line.
Reputable, better companies pay QA by hour. If offered line pay for QA, just say NO and look elsewhere or do MT instead. You're much better off and it will be a help to your fellow QA people. We all have to stick together on this and say NO to line pay for QA. I think confusion here is the words editing versus QA. Many places now call VR MTs editors. Not the same thing as QA. For companies that still call QA people editors, they need to change that title because QA people are SO not merely editors. ;)

IC companies line requirements

Stat-IQ Solutions has a 2500-3000 line per week requirement - work when you want without a committed schedule.  I worked for them a year ago before I went full time with my current employer.  They are hiring - see MTjobs.com.


How do companies pay PTO when we are paid by the line.
Do they use an average of what you make most days and pay you that when you want your day off, or do they have a little bank of money that you can dip into when taking a vacation? Explain this to me please?

I'm serious. Are there any companies IC or employee with a daily line requirement of around 800 or so? I seriously lose concentration after that but I love to transcribe.
Many companies manipulate the line count either with
headers or footers or some other way and blame it on the account/hospital. This is nothing new. The indians just caught on to it.

All programs can be manipulated.

My own personal opinion, I think that they are charging the client for the H/F and making the profit off of it, like other companies have done. This is nothing new in the MT industry, unfortunately!
It varies widely. Most companies pay per line.
I've seen it as low as 3 cpl.  Some companies pay per hour, some as low at $10, others up to $16.   The ones that pay the lowest is probably off-shore work and in that case is isn't enough because you have to 100% edit and correct half of it- could also be VR work.

In some of these postings (I'm talking mostly about the big nationals here), the recruiters spend LOTS of time talking about what they have to offer but are noticeably silent about line rate. If a company has so much to offer, it should be proud of the line rate it is offering. It always seems like they are trying to rope me in in some way & then will spring the bad news on me once they have a captive audience, like a bunch of used car salesmen.

If I don't see a line rate next to pay/compensation I just substitute embarrassingly low & move on to the next listing.

I've had the thought that the listings should be divided into Big National and MTSO --- and within that, willing to show line rate and not willing to show line rate.

Anyone else?

I'm an acute care Transcriptionist with 5+ years experience. I currently work at a hospital but am researching for a reliable company that only sets daily minimum line counts versus a set shift schedule you have to follow.  Any out there? Thanks for any advice.

I believe a "thank you" is warranted in many cases, but don't hold your breath for one.

companies with accurate line count systems
Does anyone know of a company that has a line count system that actually keeps up with our current line counts? 
Lots of companies pay 1.5 times line rate

for holidays, but unless you work all day it doesn't add up to much.  My company pays for us to take the day off and then if you work you get your line rate, plus get paid anyway, or you can take another day off in the same pay period.
