DSG

They use DocQScribe and are a small company but growing.  You need your own equipment but they supply a USB foot pedal at a cost of $80.00, taking out $40.00 x2 paychecks.  Only bad thing I noticed was that they are awful at responding to your application or subsequent e-mails which put me off tremendously, taking too long a time to respond.  Also, they have direct deposit and sometimes post positions on MT Jobs and if you scroll that board, you might see their ad if it's still there.
