You are so right. There isn't much you can do when the dictator is just plain bad.

This will probably eat up valuable time, but it might be a good idea to document why the report is being sent to QA. Just jot down a note to yourself. When they come back on you for high submission rates, you have documentation to prove it wasn't just not taking time to look something up or whatever, but the fault of the dictator (chewing, yawning, slipped finger off the phone button...)

One other thing that is helpful, although you may already do this. As you transcribe, leave a blank immediately for anything you can't understand. When report is completed, jump to each blank, listen again, and do the research at this time. Then do one more listen to each blank. I cannot tell you how many times this method has saved me from sending to QA.

Hang in there. There's only so much you can control. Just know we've all been there and it sucks.
