Contact the MTSO and let her know she has 48 hours to  get you a cashier's check for ALL monies owed to you.

If she fails to meet the deadline, contact the doctor's office in a professional manner.  Let them know you've worked their account for years but are now being told they are not paying their invoices, so therefore you are not being paid.  Ask to verify if there is an issue with the invoices and offer to do the transcription at 2c more per line than you are being paid now :)
