detailed than a SOAP report, often with all the medical codes in there. I have one doc who does the occasional one. Usually he does them for Workers Comp or some retirement agency. There are a lot of lines in them. My guy is really clear so it is kind of fun to do and get all those lines. I have mostly seen them in the orthopedics area.
