If you have a good range of dictators...sm
I do radiology and acute care for a facility.  There are some doctors I am glad I get paid by minute and there are some that I wish I was not so it all equals out.  I read an article some time back about being paid by the minute of dictation is really the more fair way for Transcriptionist to be paid.  I have 3-4 docs that I speed up so I do their lines in half the time but still get paid for the whole time.  It works for me.

MDI, less work? good dictators disappearing?

Is is just me? or am I getting less and less work, more and more difficult dictators, seems the account is seriously over staffed, transcribing reports on the doctors tails, and the reports I do finally get are only the MOST difficult dictators it seems....


Is our work going to India after all?


Good dictators end up on ASR so it can only get worse. My accounts are all picked over.

Ummm...No, large account I'm on just has good dictators....
It's on EditScript, and I was system receiving alerts of higher priority jobs coming through and I accepted them. The dictators were by and large very good and many of the reports required minimal editing.

It's luck of the draw when it comes to being placed on an account. I am very grateful to have been hired for the account I'm on.

Sorry you apparently aren't as happy. I hope your day gets better.


Unlike the poster below, I did not find the ESLs good dictators at FN. sm
In fact, in my over 20 years of experience, they were the worst I have ever heard.  We got way more than 3 e-mails a day, too.  More like 10 to 15, beginning for help and sometimes bordering on threatening. If you are really good with ESLs, you MIGHT do okay. But who wants to put up with that?!?  No bennies either.  Glad I am long long gone.  The grass really is greener.

Offshore is not a good solution
These are people who come from a completely different culture. They have very little regard to human life and therefore it is hard for them to comprehend that we do and have it extrapolate out to providing a conscientious medical record.  Please do not think that I am criticizing them for this. It is just a fact. I know a little of what I speak because I have seen the product that comes out of the offshore community and India and Pakistan to be specific.  Did you see the movie Slumdog Millionaire. Did you see the screenwriter's analysis of life in India? He said everything is extreme over there. If it is sweet, it is very very sweet. If it is spicy, it is very very spicy. If it is poverty, it is grinding abject poverty, living underneath active outhouses. If you see the movie, there is a segment there where it portrays a call center and the lead character tells the customer that he is just down the street from her and he is looking at a map of her area. But then he messes up and says the wrong thing. This is the type of culture with which we are dealing. They think nothing of lying through their teeth just to keep their jobs. Do we actually think the clients here in the US even know that their precious heretofore confidential medical reports are even being sent overseas? I think maybe 20% of them know and the other 80% are blissfully unaware. That will change.  
I try and try and can barely do 200 lph with TT. Used to do 300+.

and it's not because they're all ESL.  Something else and I don't know what.

mention spouting all the facts, and I know most others on my team have no clue either, so I again am amazed at your talent for spouting facts that most of us don't know anymore, since things changed so much. I work with a lot of great smart gals, so I don't think its just us. Glad you're one of the few on the ball, but most of us aren't.  Confusing enough figuring out how to change our personal health choices, let alone knowing what the entire company offers everyone.

Barely any work
I have to work for 2 companies at the same time, because of the lack of work.  Has anyone else faced this problem?

lack of work. Looks like we all need a 2nd job. They have always overhired for the 5 or so years I've worked there.

barely making a living
Used to make a decent living but lately (past 2 years) have not. Major pay cuts in all 3 jobs (per diem to hourly and page count to line count) and big expense of benefits for single mom and 2 kids.

I sort of lost all my incentive and feel very used. Thought my salary would increase over the years (or at least stay the same), not decrease. Keep trying to get motivated but can't seem to get there.

I am currently employed by a national, benefits are $550 a month and I have no control over how my lines are being calculated, plus it is on a 74 character line, not even 65 so my lines in my pay are way lower that what the company is calculating already.

Recently tested and interviewed with another national but they pay per page (which was way less than my page rate 10 years ago) and beneftis were more than $600 a month.

Any suggestions and ways to get motivated again would be appreciated.

You've barely scratched the
surface with one of those match box schools. You would still need Andrews or MTec, but like some others, I think this is a risky field to invest in at this point. I believe coding has more of a future. Andrews teaches that.
Oh, but its OK that the MTs barely make living wages?
Shall we feel sorry for the poor MTSOs now?  Please...buying meat past the expiration - most of this winter I had to hit the FOOD BANKS, and while working full time.
Not so. Can barely get 9 cents directly from the doctor!
Doctors see all these Indian companies offering 6 cpl, balk at 9 to 10 cpl, and they want it printed, free courier service, ect.
9 cpl barely passable if an employee with bennies, but -
if you're an IC, don't forget you have to buy your own health insurance, no PTO days, and you have to own & maintain your own equipment. If it were me, I'd be asking 12 cpl, might (just MIGHT) settle for 11 cpl for the right employer, if the accounts are good line-producers.
My accounts have barely any work today.

I've managed 2 small jobs so far today.  My account had picked up for about 2 days and actually had a good backlog, but then it was bombarded by MTs instructed to go there, so completely wiped out and nothing left for the primaries.  And I see instructions for other MTs to come here for part of their lines again today as well, which means the primaries on my account have to fight them for work again.  If my husband hadn't just had his hours cut also, I'd consider just taking a week off and getting stuff done, since I'm mostly wasting time babysitting my computer that I could put to more productive use elsewhere.


They have the accounts set up so that we can BARELY get our minimum lines required in.
If I want to camp out for 15 hours a day as I wait for work to trickle in I can make more. :-(
My husband has been on STRIKE for almost four months and we are barely getting by!! The union made
big promises and all I can say is we have lost our job!  We are struggling to get by!  If you go on strike and do WITHOUT you will see what I mean!

Dictators
Actually, yes. The dictators are very good and the variety was also a good mix.
How many dictators a day do you do who
speak clearly, enunciate each word, isn't eating, chewing gum, or flipping through the chart while they are trying to dictate, who does umm, umm, umm every third word?   I don't think VR will be a threat any time soon, if ever.

The majority are good. After a few times of transcribing them and with sample reports, not a problem

Hispanic Dictators
Hopefully its not De Tar Navarro. That's an awful account.  They have quite a few of "mush mouth" RN's who dictate for the doctors.  You don't get samples, so you pretty much wing it.  At least you don't get the defy's (QA markers at MQ) counted against you!!  De Tar is all progress notes and a few discharge summaries.  You're lucky if you can get 3/4 of your required line count!! Other than that account, the others were pretty easy (with the exception of those bad quality sound files that is).
They are in a folder as are they for a ton of dictators.
Unfortunately, there are TONS of them that I have to scroll through. Even though they are alphabetized by dictators last name, it is very time consuming.
Difficult dictators?
So MDI-FL and WebMedx both have decent health insurance for a decent price? And what do we have to do to get that? Are they all ESL? Or do they truly have decent dictators where you can make some money.  I have done very few ESL. What typing speed are they looking for?

Depends on who the dictators will be.
At Spheris, I got paid 0.09 for acute care but only 0.075 for ER work.

I could do nearly twice the lines of ER work, though, because of better dictators.

So, I pushed to do as much ER work as possible and made considerably more money.

Awful dictators

Awful dictators.  They cannot keep people on this ortho account.  The sound is so muffled, you cannot understand even an otherwise very clear dictator.  They do not have a website.


We could say the same thing about their ESL dictators...sm
who are making a 6-figure fortune off of our backs...while they pay us pennies.  Learn the language, at least!

and the account specifics would make you nuts. specifically, they wanted tables edited that would take about 15 mins/per @ 4 cpl. also we kept running out of work, so yea, at $7/hour I quit and am doing just fine now back on clinic work.
Stuttering dictators

Just wondering what you all think about this:  Should a doctor who obviously has a terrible problem with ssssstuttering be dictating?  Seems like it might be an invitation for errors on the poor patient's medical record.  What do you think?

who dictate like this then YES it is fair to test you on those.  It does no good to give you a good dictator (whatever you deem to be good) and then most of the dictators of of the caliber you had on the test.  It is very fair and honest to the potential employee.  What happens if you test on a dictator that is good and then your account has the really bad ones?  Is that fair to you or the company?


It use to be that folks in the MT profession accepted this as part of the profession and none of this was ever a point.  Seems now MT has come down to lines, spaces, cpl and all the rest and forget the real reason behind this profession, THE PATIENTS!


Dictators getting worse all over.
More and more ESLs. Many younger doctors not as 'literary' as older (retiring) doctors, may have opted out of language and literature courses in school in favor of science courses. In some cases good dictators requiring less proficiency to transcribe may be sent offshore or routed to VR. Seems to me that some younger doctors don't care much about documenting their work and don't spend any time preparing a dictation in advance, then fly through it just to get it over with.
