Most MTs really do not recognize errors they produce sometimes. I was surprised when I started grading, thinking that the instructions given to the testers were wrong or incomplete or something. I asked to see the instructions given to them and listened to the dictations myself. Sure enough, the testers really did miss those items. When I would tell testers their scores, I was called all kinds of names, accused of lying and cheating them out of a job, etc. The truth was that they were making errors they didn't even realize they were making.
