This is the theory. It is happening now with the EMRs but 99% of the time is not correct. The diagnosis is entered by the provider (MD, RN, NP or PA), who knows nothing about coding rules. It was a short experiment as facilities who tried it found they lost big, big bucks for claim denials due to coding that did not meet the rules. We no longer pay attention to the stuff the providers enter but still read the charts and do the coding from that info.
