The clinician said 'transferred,' she typed "transferred." Verbatim or not, sounds like something an ESL phrase and I wouldn't have changed it to 'transfused' either.

She transcribed what was dictated. I see no reason to fault her.

Had she taken literary license and changed it to 'transfused,' and the clinician dictating the report called it into question, said MT would get a major ding for changing what was dictated.

IMHO, the MT did nothing but her job and there's nothing to support zapping her.
