the trainer on the day of her scheduled training session on the secondary account! The primary account is easy for her as she is just off of a major university and now is doing small town work and whipping them out faster than I can log in patient information.

This is not fair, it seems to me. When I contacted her via e-mail to see how the account was going, she simply said that she had not been trained on it yet when I vividly recall that her training date was over a month ago.

switch accounts all the time.
But maybe mine is the only one that thinks she knows it all. What a witch!!
I would list but make sure you put "do not contact."

And put your reasoning.  I looked around for another job and listed my present employer without instructing them not to contact and my supervisor had a fit.  She called and confronted me about it and wanted to know why I was looking.  I tried to explain I was looking for a part-time job to supplement, she told me it was against company policy for me to work anywhere else while I worked for them.  Then she refused to even give a reference for me.  She basically told me she wouldn't return the call to the HR rep who had called for a reference.  I was shocked at how personal my supervisor took it.

Just tread lightly and best of luck to you!

I would contact the bank, stop the auto debits, contact the lender...
Contact the lender, explain that you stopped the payment because of financial difficulty. Ask if they have a hardship plan. You probably have a 20-day grace period on your loan payment, (they report to credit bureaus when 30 days late) and you probably also have a grace period on your insurance. In the meantime within those 20 days of grace, FIND A WAY to make some cash. It seems, at least from my experience, that when you get behind, it is almost impossible to catch up. If you use next month's check to pay last month's bills, you never get out of the hole. I've been in that downward spiral and I understand what you're going through. I've sold things on Ebay, consigned clothing at consignment stores, and worked two jobs just to stay afloat at times. Every little bit helps. It can be done. But don't WAIT, call them today!!
You mentioned the IRS. Try calling the Taxpayer Advocate's Service toll free at 1-877-777-4778. They are independent from the IRS and their function is to help solve IRS problems that cannot be solved through normal channels.

I have been a member and not
I got nothing out of being a part of it except that it looked good on my evals to be part of an organization, but the raises I was getting didn't justify the cost. I have thought of getting certified, but see no reason are looking at it for a long time.

Reasons I was told to get certified is that you get hired faster if you are certified, especially by the outsource companies, but I do not seem to see people very happy with any company so I will continue to work fo the hospital I do. Plus, the outsource companies are sending work out of the country to get cheaper laborers and are not interested in paying top dollar for certified and competent transcriptionists.

To get hired for hospitals, etc., they are also tightening their belts and I will go with a good non-certified before a certified Transcriptionist and pay less.

This "AHDI" has made themselves "self-proclaimed" association for MTs. I could start one today and if I had investors to get it organized. We have the Joint Commission setting what is acceptable and making the industry unified in the way things are done and that overules ADHI anyway. I think someone is making lots of money off already poor MTs!

To Proud Member of AAMT: I think it's just as

Things in this field have changed for the worse over the past years.  Salaries have decreased, yet expectations have risen.  Benefits are contingent upon production, yet a good share of the time production can't be met due to complications outside of the MT's control.  When it comes to taking the losses due to changes in the field such as offshore outsourcing and technologic advancements, it isn't the clients or the service owners taking the major hit.  It's the transcriptionists.  Yet AAMT has done NOTHING to help the transcriptionists in general.  In fact, it has gone out of its way to PROMOTE offshore outsourcing and even to change some of the guidelines in the newly published, amended BOS to accommodate voice recognition and other technology geared to replace the MT or relegate transcription to other forms labeled "editors", which in reality involve retyping whole reports or major portions of reports at a cut in pay, of course.

All anyone, newbies included, needs to do is to look at the history of the medical transcription field, see the changes, and then see how much AAMT did to promote the worth of the Transcriptionist in this country.  The depressing message isn't coming from those speaking against AAMT, it is coming from the consequences resulting from the actions of the AAMT.  One fact alone can depress a newbie, and that is to look at the average line rate in 1995 compared to the average line rate now in 2005.   Thank you, AAMT, for promoting that.

to start holding hands with India. They are not out to help AMERICAN MTs. They don't care one way or another about American MTs anymore.

Their latest pitch is to the  MTs STOP transcribing verbatim. So not only will we have to be language specialists, we'll have to put a doctor's words in "proper English."

It doesn't matter what the docs want. It's what they want. Baloney. They are not our boss. Our docs are our bosses. If the docs want verbatim, that's what they'll get...at least from me (except for the JAHCO list of unacceptable terminology).

I'm so glad there are hospitals and doctors out there that don't give a hoot about AAMT. I sure don't. Oh, and if anyone noticed, usually buying products through AAMT doesn't save a dang nickel. In fact, you pay just as much, if not more, for the same products you can get by dealing directly with the company that makes them till all is said and done.

I'll save my money and spend it on more important things.

Anyone a member of AHDI/AAMT? (sm)
I'm interested to find out what benefits you actually get and use for your money, and if any employers are at all interested in the fact that you are a member etc. It's not a inexpensive prospect to become a member, that is why I wonder if the benefits are worthwhile to the working MT.

Thanks

So sorry for the loss of your precious family member -
My sister's lab has been attacked twice while on a leash in front of his own yard by a pit bull. The attacking dog seems to never been on a leash. Her lab had part of his mouth ripped open and required numerous stitches to reattach it, plus some bites to his back. Something needs to be done to stop people from breeding pit bulls with these temperments!
Tell that to my friend. They had a dog they treated as a member of their family. That is, until it a
Dogs need to be trained and need to know they're lowest in the pack. I tried telling my friend this, but she wouldn't listen and she continued to treat the dog as a member of the family instead of as a dog. And eventually that "member of their family" decided it didn't want any more competition for affection, and attacked their son. Thank God he survived, 92 stitches and some nasty scars tho. If you love your family and you love your dog, then TRAIN your dog. Don't treat him like an extra child.
Very good post - was getting discouraged as a member myself.

I have a question regarding a family member and possible cellulitis?

I went to visit my aunt last night.  Last week she fell and broke all the bones in her ankle.  She has plates and screws.  This is her fourth fall this year, but that is another story.  I looked at her cast and placed my hand on her leg just above the cast and it was warm.  I felt her other leg and it was cool.  The more I went up on the broken leg the less warm it got, but all the way up it was definitely significantly warmer than her "good" leg.

My mother is a nurse practitioner and she showed up while I was there.  I explained my concern to her (my aunt is also a diabetic) about a possible infection.  She wouldn't acknowledge my concern.  I repeated myself and still nothing.  Lately my mother and I have had a couple issues regarding family members and their heath.  I am not a nurse nor do I claim to be, but being in this profession you do learn about the medical field and somtimes I feel I see a different side of things than my mom.  I can't figure out why, but she just doesn't see any value in my thoughts regarding medicine, ideas, thoughts, alternatives, etc.  In every other way she is the best mom in the world and I love her so much, but I feel she is disrespecting me by not taking my opinions/thoughts into account regarding the field of medicine and our family.

Another example is she has trouble with the pronunciation of medications.  I'm not claiming to be a pro, but I hear doctors and nurses says these meds over and over and over and I feel the majority rules in this regard.  Again, she thinks I'm wrong and doesn't really listen to what I'm saying.  The only reason I correct her is because I think it would be embarrasing to speak with a doctor and not say a med correctly.  I think it is a reflection on her.  It also helps with dictation too (as we all know).

Any ideas.  I don't know what to do.  I tried talking to her about this a couple weeks regarding my dad's health and she really didn't listen to what I had to say.  With certain things you can just blow it off, but this is the health of my aunt and father and I just couldn't live with myself if something happened to them.

Thanks for listening and for any ideas.

If you are a member of the AAMT, how have you benefitted in her career? CMT? SM

I have been at this for about 15 years and have thought off and on about becoming an AAMT member and about getting my CMT, but always stop short because of the cost and basically the inconvenience of having to find the testing center that seems to be located in some obscure office building I've never even heard of.

However, I have recently started working for a facility that actually encourage their MTs to further their education, to network with one another and share information, and offer pay raises for the CMT credentials.  So, once again I am considering the pros and cons of membership and taking the CMT exam.

How do you handle a family member who is a hypochondriac???

Has anyone ever had a family member who they believed is a hypochondriac?  We have an individual who seems like he always has something wrong.  It is to the point now if he really does get sick I can't believe him anymore.  I can't even bring myself to calling him to see how he is feeling when he says he is sick b/c I think he is just making a mountain out of a molehill.  Am I being insensitive?  I am just so tired of hearing it.

I am sure someone is going to recommend counseling but he already does that.  Anyone have any other advice how to handle him?

TIA

Transcribing Report Of Family Member

I am wondering what the proper protocol would be if I ever came across a report that was dictated on one of my family members.

I just started doing transcription for a hospital in a really small town--the ONLY hospital in that town--and it just happens to be the same town were most of my family lives.  I know myself well enough to know that I won't break any rules of confidentiality.  However, is there anything in the *laws of transcription* that say I legally am not allowed to transcribe reports dictated on relatives??

I am afraid to ask my supervisor this question, as I really like this account and don't want her taking me off of it.  It's chopped full of *standards* and the line count/pay just doesn't get any better than this.

transcribing report on family member
When I worked at a hospital, I asked my boss, the director of medical records, if I was allowed to transcribe a report on a family member, and she said "yes".
huh? how? when someone says NFL team.SM

Please - don't look for a problem when there is no problem.  They said NFL teams - you know how many NFL teams there are????????????  Try 2 for each state.....American league, national league.  They said "celebs" - try a zillion.

If it ain't broke, don't fix it........nobody posting compromised any HIPAA laws or any patient confidentiality.

Give us a break, eh?

Drill team
My DH is a high school teacher and we go to tons of band competitions and football games  and I wasn't even aware that drills teams even exist anymore. There are cheerleaders and color guard, but drill team?  Are you sure that's what it is.  Maybe it's regional but in the east, no drill team.   Back when I was on drill team, it was a very large squad and the  moves were not "dance-like" at all.  We marched beyind the band and it was very marchy and military-like.  But then, even cheerleading has changed so much that it ALL looks the same to me, very "dancy."  My kids have taken  lots of  dance classes.  I would say that jazz or hip-hop would be fine, maybe hip-hop a little more.
I don't know what a drill team is but
where my kids grew up cheerleading was the school activity of choice and most girls were in tumbling, gymnastics and dance often as soon as they were out of diapers. Fierce competition!
I do know how to manage a team, obviously,

since I have my own service.  I know that you can't put someone with much less experience in a position over a bunch of people with a lot of experience.  It just does not work.  I not only have a lot of MT experience, but I have been a service owner for quite a few years.  Before that, I worked in a hospital transcription department with a lot of seasoned MTs.  Through the years you see a lot of supervisory changes and what it boils down to is the experienced MTs need/want someone who knows more than they do, someone they can go to for help, when they need it.  With someone less experienced, they can't.

One of the big problems people had with Medquist when it started was that the "suits" were running things - not people who actually knew what they were doing when it came to transcription.  You have to know both sides of it.  And I can tell you from years and years of experience, it doesn't work when you try to put someone with less experience over those with lots of experience.  The seasoned MTs have NO respect for someone who knows less than they do.

Sorry, but that's the way it is.

Are we a team or are we competitors

Two days ago our supervisor came into our office because Dr. Neuro had dictated a report.  It was obviously an EEG. She did not know what template it went on.  Well, co-worker (super MT) just jumps up and said “I don’t know, you have to call home MT 1 and home MT 2 because they do cardio”  Well mind you, super MT has only been there a year.  I have been their 8 years and I have done ever single report that hospital offers including cardio, especially cardio because that is the first thing that pops into my Q.  (Doc dictate EGGs under cardio, it is up to us to put it in the template).  I found that insulting.  Then yesterday one of our male MTs needed help deciphering what a doc said.  Well super MT jumps tall cubicles to help so I just kept on typing.  She could not understand it either so she gets someone else who is not even an MT to listen.  It is petty but I find it insulting. I don’t do her that a way.

I am more of an introvert.  Super MT has a lot of friends that come in our office just basically gossip about the other people who work next door. I try to tune them out because I only have 8 hours to get my lines in because I have to be out by 4:30 to pick up my two children.  I cannot stay late like they do.  I used to go to lunch with super MT and her buds but stopped lately because I joined a gym and I go work out with another girl who happens to be on the top of their gossip list.  They don’t approve of her lifestyle, which it is questionable but it is her business.  She is a pleasant person to work out with we just happen to have a common interest in exercising.   All they wanted to talk about at lunch is talk about some TV show I have no clue what it is because I don’t have time to watch TV.   I have 2 small children and when I get home I have to hit the ground running with supper, dishes, grocery store.  I also have also turned down their outside parties, 1 because I could not find a babysitter (alcohol is involved at their parties) and the last one was because I was running on fumes until payday and a $20 meal just for me was way out of budget.

I have explained most of the above to her about babysitting, interest in loosing weight, tight budgets and needing to get my line count up which is why I don’t talk as much and go to my introverted ways (they called me an introvert and super MT is an extrovert), not that I don’t watch TV and could care less what the people next door are doing and really could care less what my workout buddy is doing in her spare time.  Then again I may have told them that.  Should I be bending over backwards to be in their crowd?  I didn't know being their best friend was part of my job description.  I don’t think they are all as great as they think they are but I don’t see why we cannot at least respect each other as professionals.

My team lead is not even an MT!

She never understands anything I am talking about. I have to explain things to her like she is a 2 year old, and even then she just doesn't get it. Why would a company put someone who is clueless about transcription in a position over other MTs!!!??? It is just another frustration to add to an every increasing list of frustrations. But, like you, I am keeping my mouth shut. I just don't get it!!

I have been AOL member since 1999. Can you bring link to this story to board?

so its cold here! Actually it broke the 15, the night before we left for Disney on a Make-A-Wish trip for my daughter Jenny....(we had a blast)...got back 2 days ago, it broke again that night (21st). DH got it running again, but the heater guy disassembled it last night and has not returned yet to put it back together and fix it. Luckily we have a propane fireplace or we'd be in big trouble, still chilly though especially since it goes down to 25 at night, brrrr!---so any $$$ if we are included or entitled would be wonderful...but I doubt we will get so lucky since it was so long ago that I was a "member" of AOL. Guess I will just have to work even more to pay for the repair, fun.

I was sent a courtesy copy of the office notes for a family member once.
This was soon after I became an MT. I was so appalled at the sloppiness of the typing and the rampant use of inappropriate abbreviations that we switched doctors. At least I knew what the abbreviations were for, not sure the typical lay person would have.
Dog doesn't carry a gun, none of his team does. sm
His story is actually very interesting. I know he is abrasive at first adn the wife is a real piece of work, but they really do try to help the people they round up. Dog is a born-again Christian, and he tries to help the people straighten their lives out, isn't in it just for the case. In fact, his own son is in jail for 20 years because of drugs, and so he has a personal motivation for getting drug dealers off the streets. I never miss the show, I find him inspiring because he found his nitch in the world to try to work for good. Unconventional, but he really sincerely tries.
I think my team leader is bipolar.

She's all sunshine and happiness one day with kind words of praise.  The next day, she's sending out shrieking emails contradicting what she said the day before.

How do you select your team leaders? When did you
lasst post a promotional position open? In 3 yrs I saw this 1 time, January I think, but I see lots of new team leaders. You spend a lot of time on this board, but how many times have you taken enuf time to write e-mail to the MTs you have to let them know what is going on. When did you get promoted? Is Jason still around? Did you ever tell us when you have new accounts so we can change ifwe want to? No, youjust hire more MTs from outside to fill those jobs. Sorry, but your team leaders still have only 1 answer when we need more work and that is they don't know what accounts are behind. So what you did is what somebody else said. you added another layer of management to answer with I don't know. Yeah, you sent out an e-mail last week and a few other times about this 1 account that is behind, but what about the rest of the time?

I started coming to this board when I started looking for a second job so I could make enough money to survive after you sold to Transcend and started this big hiring binge so we don't have enough work.

I won't call you,I won't write you, I just am tired of the empty worthless responses I get. I will sit iin my corner and type during my shift, but I am not happy. The communication is still horrible in the company and team leaders just add to the massive confusion.
Team leaders tell you to find another job too.
Don't expect any help from that end.   I can't see what the team leaders do other than send out account status reports twice a day if they happen to be home or harrass you about petty crap.
Yes, and his team members were supportive, which will help. sm

I've been trying to think of where I saw Kelly LeBrock before -- wasn't she the one who did those commercials for the shampoo, saying "Don't hate me because I'm beautiful?"   I might be thinking of someone else.  I guess I'm showing my age, just by mentioning that commercial ... oops.

I never hear from my QA/team leader. sm
and last time I called into the office for something, they answered it and said BYE LYNDIA! and hung up. Rude, but true.
I look at it this way...I am part of a healthcare team.
There are other members of this team for a reason. I knew an MT who told me that if she could not put down a word with 100% certainty, she would not leave a blank. She instead would jump through all manner of hoops including pulling and reading the patient's chart, talking to the nurses, or waiting and even talking to the doctor.

In my opinion, this is not the best use of anyone's time, especially hers. While she is wandering the hospital looking for resolution, other reports are waiting and patient care is being compromised. Obviously at the end of the day, no one else is going to sit down and transcribe for her.

A blank is not a failure. It is simply a request for clarification. Some members of the patient's healthcare team are better qualified to provide that information than I am, so I leave it to them.

I don't guess at addresses. If the HIM department at the hospital feels that is information critical to my job, they will provide it for me. That is their job. Once I have that information, I will gladly and accurately type it every single time it is requested.

I don't guess at what *@$%#*@! means in a report. Again, once the interpretation of that is provided to me by the kindly physcian who garbled it, I will gladly and accurately type every single time it is requested.

I honestly don't consider turning in a report punched full of blanks to QA or even the hospital a failure on my part. If the doctor is eating, the report is dictated 30 feet away from the phone, or Mr. In-A-Hurry thinks I can understand his speed mumbling, I still have done my job to the best of my ability, and now it is someone else's turn to work on that record.

What I would really like is the marketing executives or the used-car-salesman-type recruiters that constantly tell you that "of course line rates aren't going up" to have the pleasure of working on these reports. They low-ball the line rates to get the work and then pass that crap on to skilled MTs making it look like Christmas. You know darn well that if they did not ever have the prospect of a raise, they wouldn't be working for this company yet they can spoon-feed you all the reasons why you can't have one.
My new training team and trainer are in the US. I believe
right now they are looking for people to be on the USA QA team, as they are ramping that up for USA accounts which do not want to off-shore. At least that is from what I understand. But best thing is to get it from the horse's mouth. I am pretty confident in my decision to join the team, and am looking forward to it. Good luck to you.
Never call a ROM or team leader if you need an
answer. You need to have it in writing. Anything verbal is rapidly changed and they will never admit they said it the other way. They are experts in evasion
