As an IC you invoice the company, count your own lines, and receive a paycheck.

Without starting a war, how much time is reasonable to spend on reasearch (sm)
when paid by the minute?

I have access to the patient's electronic chart and often use this as a tool to look up doctor names, diagnoses, etc. Often, these charts go back for years. Documents are both scanned into the patient's chart by the physician's office(a 5-page stress test would be 5 separate scanned entries) and typed directly into the patient's chart (a 5-page document would be 1 entry).

We are paid by the minute, no matter how fast or slow the doctor talks. So, if they manage to speed talk, not take a breath, and slur an entire H&P into a 1-minute report, we get paid for a minute's worth of work regardless of typing time.

Having given the above information, what would you consider a reasonable amount of time to spend on research,i.e., looking up "Dr. Smith" with no first name and no specialty or indication why the patient saw "Dr. Smith", or things of that nature.

(I've got my fingers crossed that this does NOT start any type of MT versus QA argument!!!)

anyone paid by the amount of time dictated?

I am being paid by the line right now, but my office is switching to a completely new system that counting lines would be a nightmare.


The doc has suggested maybe paying me by the amount of time dictated.


Has anyone done this before, and if so what are the pros and cons, and what is a reasonable rate??? I have NO idea!

I have been with them for seven years this September and never raised my rates, which I brought up to him while speaking about the new system saying I would hope whatever we decide is more than I make now!

Yeah, I feel your pain. I shop for as much groceries as possible in the Dollar Aisle at a local Kuhn's grocery store. Hopefully you have something like that around you.

My answers:


1.  Missing 1 paycheck would hurt but not devastate me.  Mising 2 or more, yes - it could be dire.


2.  Yes, most definitely.


3.  Started one and slowly building it up.


 


My sis-in-law did not do well enough on the final
she just started applying everywhere anyway. Needless to say, she got a job and is doing QUITE well. She is making more than me actually!! Go figure.

I worked for them for about 7 months the first 5 with direct deposit was fine but the last couple where they mailed checks was bad!!! My last several checks were 2-3 weeks late. Several people have left here. The "captain" is crazy. You would not believe some of the crazy & nasty emails he sent me. I would definitely get out of here unless you like waiting 2-3 weeks for a check and then are not sure if it will clear. Yes I heard a few checks bounced. Sorry I could go on and on about this guy but I think I have said enough. Good luck to you.

It usually takes a full billing cycle for direct deposit to get going in my experience.

Yeah the MTSO was responsible in that if they had payed her when it was agreed upon she could have paid her bills and wouldn't be in a financial bind. If you have your paycheck you can pay your bills. You work to pay bills, not for the heck of it. Yeah, you should have a cushion but they should pay on time. You think a sick child is an excuse you go pay a few doctor bills a see how much "cushion" you can keep in the bank. Don't judge people without knowing the circumstances. You don't know what is wrong with the child or how frequently they have to go to the doctor and it costs money not to mention the medicine. They have an agreed upon paydate and she has every right to be angry when she doesn't get her money. People work to pay bills. Alot of people don't have a big cushion in the bank.
No paycheck

I started with a company about three weeks ago, and here is the situation.  This is my first transcription job, so I think I was a little too eager to jump at the job offer when it was given to me.  They charge a "set up fee" of 100.00 that is taken out of the first paycheck.  I am only getting paid 5 cents/line, so I did not make that much more than the 100.00 (I think about 40.00).  The pay period ends on the last day of the month and checks are issued on the first of every month.  They do not offer direct deposit, so I have been waiting for the last nine days for my measly 40.00 check.  I really just want it to have documentation that I have paid the stupid set up fee (which I am embarrassed that I even agreed to).  I have emailed them 5 or 6 times with no response whatsoever.  They continue to send me work, and will (usually) answer questions regarding work, but NOTHING about my requests to see where my check is. 


I have since found a much better and higher paying position with another (wonderful) company, but I do not really want to put in my 2 weeks' notice until I have that check.  Any ideas??

I have a very old version of IT and ran the compilation for VR (helps improve accruacy), but there is no way I could remember all the different phrases they have. I'm lucky I remember the traditional chfx = congestive heart failure.

FINAL QA COMMENTS

My main complaints with QA are when the individual QA's opinion regarding commas and such affect my QA scores, with the threat of deductions to pay, withholding of raises, benefits, etc.  I did actually receive a payroll deduction once, but it was due to the QA's error.  Thankfully, I discovered their error and was able to prove it.  It was extremely inconvenient, however, because I received no advanced notice that my entire paycheck would be reduced by 15%, and I had to wait for the next paycheck to receive the correction.  I also resigned that position based on the rude treatment I received from the payroll department and the subsequent hassles that I experienced while proving the QA's error.


Again, it seems that QA has very little to do with patient care or document accuracy.  Recently, there was a big controversy with our QA regarding the proper way to indicate military time.  QA had decided that a colon was required and deducted points because I did not use a colon for '1925 hours', while the BOS indicated that a colon was not required and the customer specs didn't specify anything about it.


I have some Stedman books, but it galls me when a company says that Stedmans is THE SOURCE.  Most of the time, I have found answers on good Internet sources; accuracy is easily confirmed.  My books are not easily searchable, and I have not invested in the electronic versions.  Also, I have found discrepancies/inconsistencies between different Stedmans books.  I just purchased my BOS Second Edition 3 years ago, along with the PDF searchable version.  I'm not anxious to purchase the Third Edition so soon.


Again, I believe that all this stuff is more about control than about providing accurate reports for the benefit of the patients.  IMHO, there is more than one accurate way to form a sentence, and this QA grading process is a bunch of unfair baloney.  If the dictators were grading according to a similar QA process, it may seem more fair.


Maybe the persons who began the QA points/grading process were just bored and needed some drama.  Or maybe the objective, again, was simply to devise a method/excuse to reduce MT pay and prevent payout of benefits?

I work in 1 of my bedrooms with a big window facing the driveway. I have done this work for 30+ years. The office also has my husband's computer in it. I have absolutely no problem in keeping my mind on my work, the pay alone is the answer, no work, no pay and the more lines the better I feel when I get that check. I never see things I should do around the house because I have a housekeeper that comes every other week and takes care of things like that for me. Housework is about the last thing I want to do!!

But, you might consider drawing up a "contract" that states the terms and conditions ... the dotor takes the ultimate responsibility ... your initials are only a reference ... and so forth. You sign it and they sign. Throw in a notary.

To get the paycheck I want, I work
5 days a week and I type 1500 lines per day. I am radiology and therefore this is easy to achieve and for the most part, I am done in less than 7 hours but I will put in 7 hours.
No paycheck, need advice
I have worked for a company since April 21 of this year.  I began asking about my first paycheck around the beginning of May, because it was my understanding that I would be paid for what I worked in April on May 1.  After asking MANY times, one of my bosses (they are husband and wife) finally responded to me (saying that he had already responded many times) only when I stated that I would not continue to submit any more work until I got an explanation of where my paycheck was.  He told me that I had misunderstood when I was hired, and that I would be paid June 1st for the pay period of March 5 - April 29.  I decided (stupidly) to continue working, and the same thing has happened this month.  I have been given three different explanations as to when my paycheck was mailed.  Today, I finally stated, again, that I would not submit work until I received either the paycheck, or a faxed copy or emailed copy of something that proved that a check was issued and mailed to me.  He was very rude, and basically said, he would not do it, and I would be "shut off" from work.  The wife emailed me and said she did not want me to quit, and that the check had been resent the previous day.  I subsequently gave my two-weeks' notice, and stated that I would complete any and all assignments given in the next two weeks, but wanted to quite thereafter (which I have to do according to the contract I signed).  I live in CA, and the company is located in PA... and I already submitted a back-wage complaint online, but do not know what else to do.  I have read recently that other people have had this problem, and am in contact with someone else that works there currently, who has only received one paycheck, and has worked for them since January.  They also charge a "start up fee" of 100.00, so the check I am waiting on is only for 17.00!  It's not the money, It is the fact that I have worked very hard for 2 months and have not been paid!  He was also very rude and brought up personal things about myself and family in his emails.  ANY ADVICE is MUCH appreciated.  Thank you.

I cannot figure out the hourly because I get paid by the line and that varies from hour to hour.  DocQScribe is the platform.  I just did 180 LPH for my first hour of the day which comes to $16.40 for that hour but again, that varies from hour to hour.

Radiology often has a lot of normal reports, so with normal macros from each doc its quite doable.  Also there are many repetitive phrases they all use so you can make your own macros and speed production that way.  As a newbie I'm not sure you could get up to speed that fast, it really depends on how many normals are already in place and how easy the docs are.  A seasoned MT would try to bring her macros with her, but as she already knows their lingo and can anticipate what's going to be said, its pretty much the same stuff on each type of report.

FINAL CONSULTANT POST

You are right Fingers/Carole, I am not an MT.  I am a consultant for many big businesses.  All businesses are the same, in it to make money.  The bottom line is that the MT biz is going downhill consistently.  Those of you who are happy with your jobs, benefits, pay scale.... GREAT!  But what about the tons that are not.  Do you not care for your fellow American!!  It won't be long and you will ALL have the same problem.  Pay cut and fixes other VR and off-shore work.  I did not come here to start any war, only to open eyes, that it is imperative to stop the AAMT type who do nothing for you and organize yourselves to get accomplished what is necessary to keep your pay and benefits going up, NOT down and out of the country or to VR with you doing the corrections.  There is nothing wrong with making things easier with VR, but you should still get paid for your expertise while transitioning to this.  VR will NEVER be able to take the place of the qualified MT. 


I hope in some small way I did something good here.  My intentions were in the right place.  I wish you ALL well and full pockets!!

I don't feel in control of my paycheck

For example, I start out on a roll, going along at a really good pace.. get half my production in a few hours, then all h - - -  breaks loose. Here comes the bad stuff...All of a sudden, its not about skill... its about detective work...hunting foreign referral doctors names and addresses, etc, many times I can't even understand what the name starts with. I have to search and search for what seems like hours just to get what might or might not be the right name!  OOPS...there goes that production. I'm at the mercy of my "pool". I have no control over that or the paycheck from that point on. Any suggestions?


Self-employed even worse getting paycheck
Bill three doctors at the end of the month, and still waiting two weeks later, sometimes three. I even stay up all night and do a 12h turnaround time. You'd think they would see I was paid in a reasonable amount of time!

That sounds like a fair assessment, what confuses me are the ones that say they can get 500 lines an hour...... In 15 years the best I ever did was around 325 per hour.

but as an employee the company can dictate your hours. As an IC I work a split usually and do better this way. However, as an IC, the company cannot dictate your hours according to the IRS. Companies now days promise too short of a TAT for the MT, IMO. What happened to the 24-hour TAT for H&Ps, consults, etc. and longer for discharge summaries? Not to mention the # of reports at the end of the month when doctors who haven't dictated decide they should dictate rather than be put on suspension or not allowed in the OR until they get their act together.
What is the best reasonable VR program available?

I am an IC and considering buying a VR program, but I would like to know which is the most accurate, and also cost is a factor too.  Does anyone have any recommendations, good or bad?  I hear a lot about Dragon, but I am hesitant because I have heard it takes a long time to train it to your voice.

I've gotten all of my equipment from Transcriptiongear.com and have never had problems with the quality.  Reasonable depends on your definition, though.  I never thought their prices were outrageous.

Cherrypickers complained about the difficult doctors, TAT went haywire as they struggled thru 'em, so MQ "unlocked" the "send back to pool" key. So victory for the C-pickers. I knew it couldn't last!
