Oh! The arrogance!
I've been in the medical industry just as long as you have. Try that on for size!

I have done QA but the other posts have it - QA is nothing more than an MT who watch dogs other MTs. QA is not more important, or critical in the trancription process . . . but they definitely are more full of their own puff and stuff. PFFTT!
humility is more of an attribute than arrogance...sm
pride goeth before a fall.

chew on that for a bit.
This type of arrogance and presumption ...

is exactly the type of attitude that motivated the post by the OP.  Most times when an MT is forced to leave a number of blanks that high in a document it's not because they didn't research the item;  it's because they couldn't understand what the physician said despite repeated attempts at listening.  In short, their 'ear' isn't as developed as yours.

QA people are in QA without question because their skill level is higher than the rest of the MT population in the company.  They get compensated on a different level, and justifiably so, than everybody else because of it.  You SHOULD be able to hear things that other MT's don't, and you aren't doing the MT a favor by filling in the blanks.  YOU GET PAID TO DO IT.

It's no wonder that as more and more time passes there exists more and more of an adversarial relationship between MT and QA.  Most MT's welcome constructive feedback on their work as they continue to learn and refine their ear.  What they DON'T appreciate is nastiness and having to bear the brunt of YOUR frustration in doing the job that you're getting paid to do.  Sure, there are bad apples in the bunch who simply rush through and pass the work on, but I sincerely doubt that the number of MT's who do that is as high as some would have us believe.

true, true - but what you see as arrogance...sm

I think a lot of people go the extreme -

so, if I had to be one way or the other (being a humble extremist myself...*LOL*), I'd rather have too much self-esteem than not enough (been there, done that the first 30 years!!! *L*).  And I am a truly humble person, been recognized for just that.....but because of *been there, done that* - and all sorts of therapies (not drug therapies), some end up the other way.  Oh well, this was a fun couple of posts - so I thought.

Gave me much to chew on :)  Have fun!
