Not sure what you mean by weird heart murmur.  That should have been checked at least with echocardiogram.  I'm thinking mitral valve prolapse which can cause chest pain and exercise symptoms but is generally benign. 


At  your age and being female it is highly unlikely it is cardiac disease.  They would probably do an EKG while you're having the pain and check enzymes. 


Chest pain is very very common and often is not cardiac.  It's cause is unclear, sometimes costochondritis, sometimes GI, sometimes just unknown.


You still need to see a physician very soon to determine if NSAIDs or other simple meds (Protonix) are the answer or more tests are needed.

The only ones I capitalize are --sm
the ones with person's names, like McMurray test, etc. I have never been asked to change that or had any problems with it. To me, it would take way too much time to capitalize every single test name, unless put into an expander. this is just my opinion.
Would you capitalize the name of a form the doc's use?

I wanted to move this discussion back to the top so I started a new thread.  She asked if I share any websites or information with the MTs I QA and my answer is, I do.  


For instance, when looking up a doctor's name, did any of the MTs here know that most states have a state department for professional licensing where you can look up MDs, DOs, physical therapists, nurses (both LPNs and RNs), etc?  So in addition to using the AMA, website which is great because you can do a sound alike search, you can also go to each state's licensing website.  I should add that no one ever told me this when I was an MT.  I just discovered it on my own because I cared enough about the work I put out to make sure I exhausted every resource possible.


Another nifty, self-taught, trick of the trade... I have the website for every facility for every account I work on in my favorites.  A lot of times a facility will have a doctor finder on their website.  In addition, they may say the name of a ward or department that you might be able to reference on the website.  Not to mention, it helps to know where the facility is located for future reference such as when a dictator mentions a place of employment in a patient's social history.  If you know where your facility is located, then you can look at the yellow pages online for that area.


I always share any great terminology or drug websites I come across.  There's a great website that will list most every hospital in any particular state and a link to that hospital's website.  All I had to do to find that website was do a Google search for "hospitals in Arizona or California or Texas" (whatever state you need) and I found theagapecenter.com website.


No one person ever sat me down as an MT and gave me a step by step on how to use my reference books or the internet.  I taught myself because I CARED.  That's the problem with a lot of the MTs I QA.  They don't care.  The say the MTSOs only care about the bottom line, well the MTs only care about the bottom line too and the end result is poor quality and subsequently loss of American jobs.


But so what?!?!  I'm just a dragon lady and most QA people are just big meanies anyway.  MTs can keep blaming everybody else for their low wages and offshoring, but it's time to step up and take some responsibility.

Through those many years we had the luxury of time for research and education. These MTs today must concentrate on getting as many lines as possible within as short a time as possible. There is just not the luxury of time for research we enjoyed. Its a whole different ball game now. These young robots need every bit of patience, encouragement, and help we can give them in their struggle to maintain a living in our chosen field. They memorize quickly and do not forget the things they must learn on the fly.
Would definitely capitalize "Merry Christmas and Happy New Year" However,
happy holiday season is nondescript and would put in small case.

this Productivity Talk board and I e-mailed the "moderator" of the board (and who seems to be the only person in the world who knows how to do this) two weeks ago and I've gotten no response, so that may just be a waste of time, but you could try it.  I have had to manually re-enter my auto corrects yet again.  I went from SmarType on YOG to DQS to new job in Word all in two months.  I so hope I don't have to do this again for a very long time.

Platforms are referred to when you work live
via a VPN or through the internet in some fashion -- when you work directly from their voice server and upload back to their server through the software they use.
Unless a facility name accompanies the phrases you hear/type, you should not capitalize.

One of my instructors referred our class to them a few years back.

ExtText by Dictaphone -- Word 2000 - won't capitalize when making a new paragraph or after lists?
Hi. I am new at a job. The Word 2000 running in ExtText (Dictaphone) will not capitalize at the beginning of a new paragraph (never had that happen before), nor after numbers in an automatic list, and sometimes even just after plain periods, not just periods used in numbers (like hemoglobin 7.5).

What is going on? It works fine in the Word version on my computer, but when it ties with ExtText, it "forgets" to capitalize. It is especially annoying with lists, and also at the first of paragraphs, I never had to babysit that before.

Any help? TX

Need help from Cardiology MTs
I was just put on an account that has tons of cardiac reports, lots of echos and cath reports that I have never done before.  Can anyone suggest good references or websites that can help me with this?  I'm having a hard time verifying guidwire and stent names.  I have 2 surgery books and Stedman's Cardiology, but I can't find everything I need to look up and I'm so overwhelmed.  Any suggestions would be greatly appreciated. 
Is anyone out there transcribing for Cardiology?
Just wondering if it is fairly easy to catch on or a pain in the butt? I might take a position but I'd like to know if I will be able to handle it as a newbie? What do you think?

Capitalization of Cardiology
I was taught (and it is required by my company) that you capitalize a word not usually capitalized when it is used as an entity as it is in your example (used to mean someone in the cardiology dept). A good example is when a doc says "An orogastric tube was placed by Anesthesia" you would capitalize anesthesia, as it refers to a person and not "the thing" of anesthesia. If not capitalized, it calls to mind some kind of anesthesia (endotrachial, perhaps?) placing the orogastric tube. Capitlizing it adds clarity.

Cardiology is my favorite specialty (sm)
I can't say why, though. I just like the terminology and it just seems like it's easier to look things up maybe?

Be sure you have a good cardiology book. It will be helpful, but if you do caths and PCIs, etc. there are new instruments and stents that come out like it seems every minute, so having a few good cardiology sites and knowing how to search the web is a big help.
