The hospital I worked for went to the electronic medical record about 2 years ago.  We still did the transcription, it just wasn't printed out.  It went straight into the electronic record through macros.  We typed in Word, checked it, hit 2 keys and it was off!  No, I do not think transcription will ever truly be eliminated.  Computers can only do so much on their own, they still need a human hand and ears and mind working behind the scenes.
